About this opportunity
The U.S. Customs and Border Protection (CBP) Office of Human Resources Management is soliciting comprehensive Medical Support Services through Request for Proposals 70B06C26R00000017, a firm fixed unit price Indefinite Delivery/Indefinite Quantity (IDIQ) contract supporting both employee and pre-employment medical services nationwide and at select international locations. Required services include fitness-for-duty evaluations, return-to-work reviews, medical documentation reviews, independent medical examinations, pre-employment medical examinations, pre-employment fitness testing (PFT), drug testing with specimen collection and laboratory analysis, Medical Review Officer (MRO) services, and medical and psychiatric consultations. The contractor must establish and maintain a nationwide network of HHS-certified clinics and facilities capable of providing services within a 60-mile radius of employee and candidate locations, with capacity to support annual volumes exceeding 40,000 pre-employment medical exams, 50,000 fitness tests, and 45,000 drug tests. The two-phase evaluation process prioritizes Technical and Management Approach (most important), followed by Oral Presentation, Prior Experience, Past Performance, Small Business Utilization, and Cost/Price (least important), with award determined on a best-value basis. The Phase I proposal deadline was extended to May 5, 2026, with Phase II oral presentations and cost/price proposals to follow approximately five business days thereafter. Contractor personnel must obtain favorable Tier 4 or Tier 5 background investigations and comply with comprehensive DHS/CBP security, privacy, and IT requirements.
This solicitation does not include a small business set-aside. The incumbent contractor is identified as Comprehensive Health Services, LLC, though the government issued amendments through April 29, 2026, indicating an open competition. The estimated contract value ranges from $400 million to $600 million over the 60-month period of performance, which consists of five 12-month ordering periods commencing August 1, 2026 and concluding July 31, 2031, with a minimum order value of $500,000 on the first task order. Performance of services is required throughout the United States, including Alaska, Hawaii, Puerto Rico, the Virgin Islands, Guam, and Saipan, as well as at select international locations, with the primary place of performance identified as Washington, DC 20229. Key performance standards mandate scheduling examinations within two to five business days, completing medical exams and fitness tests within ten business days, and delivering final consultations within ten to fifteen business days, with penalties for late deliverables including 75 percent payment if submitted within 30 days late, 50 percent if 31-45 days late, and no payment beyond 45 days overdue.
