About this file
This document is a medical examination and history report form and instructions for conducting Customs and Border Protection (CBP) medical examinations.
The instructions section establishes requirements for administrative/clinical staff and examining physicians conducting CBP medical exams. Administrative staff must ensure candidates complete all medical history questions with dates and explanations for affirmative responses, verify that candidate name, last four social security number digits, and date of service appear on every page, document VA and non-VA disability information with rating breakdowns, and obtain candidate signatures on the Medical History Form. Vision testing requirements include depth perception (documenting test type, correct responses, and seconds of arc or percentage), peripheral vision (temporal and nasal results for both eyes), visual acuity (corrected and uncorrected, near and far), and color vision using HRR 4th edition with recorded candidate responses and examiner signature. Audiology testing must document calibration type and date, plus canal/external ear and tympanic membrane results. Additional requirements include documenting body measurements (height and weight) and vital signs (with pulse and blood pressure retests if initial readings exceed specified thresholds). Examining physicians must document all orthopedic injuries or illnesses, explain abnormal flexibility evaluations, co-sign paperwork if exams are completed by nurse practitioners or physician assistants, and provide printed name and signature with credentials. All testing and examinations must be completed before the applicant leaves the clinic, as incomplete findings or improperly prepared paperwork may result in non-payment. Completed examination paperwork with authorization must be faxed immediately after the appointment.
The Medical Examination and History Report form itself requires candidates to complete pages 1-5 before the medical examination, with failure to answer questions or disclose known medical conditions resulting in potential disqualification. The form collects candidate demographic information, current occupation and employment details, and extensive medical history across 92 questions covering employment refusal due to medical conditions, surgeries, hospital admissions, military service rejections, VA and non-VA disability claims, medications, inhalers, allergic reactions, diabetes, thyroid disease, orthopedic injuries, cardiac conditions, mental health treatment, and the candidate's ability to perform the physically demanding CBP position. The form includes sections for vision assessment (depth perception, peripheral vision, visual acuity, and color vision testing), audiology testing, body measurements, vital signs, orthopedic clinical evaluation, and a comprehensive clinical evaluation requiring physician/NP/PA signature and summary of significant findings.

COLOR VISION TESTING: HRR 4th EDITION
PREPARATION:
Keep the test book closed when not in use.
Do not touch the plates/pages with your fingers; turn the pages by the tabs.
Seat the examinee so that their eyes are kept about 30 inches from the book when it is placed on the table. Record all answers on the provided score sheet.
Turn on the light source and exclude from the room most extraneous light that would fall directly on the test. Test binocularly (both eyes viewing at the same time).
NOTE: Color corrective/tinted glasses or color corrective/tinted contact lenses are NOT PERMITTED.
DEMONSTRATION:
The demonstration plates/pages (1-4) are used to show the examinee what to do and to explain how the test is done. Please complete plates/pages 1-4 with the examinee. Show these four plates in sequence saying; “I am going to show you some colored symbols. Without touching them, how many do you see? What shapes do you see? Where are they?”
Say, “For example, your answer may be – two symbols, a Circle and a Triangle, the Circle is upper right and the Triangle is lower left." Unless the examinee is malingering or totally color deficient, two colored symbols should be seen on each of the first two demonstration plates (O, X and △), one colored symbol (O) on the third plate, and no colored symbol on the fourth demonstration plate.
Tell the examinee that these symbols may appear in any of the four corners of the page.
Ask the examinee to trace the symbols they see with the small brush. Then say: “The test itself is made up of just these three symbols with two, one, or none on a page. Some of them will be harder for you to see as they may be less strong in

SCREENING:
NOTE: These instructions are specific to this particular client, COMPLETE ALL 24 plates/pages. Be aware the plate/page numbers are on the BACK of the corresponding page.
Ask “How many colored symbols do you see on the page? What are the symbols? Where are the symbols?”
Obtain a quick response from the examinee but take your time carefully recording the response (using X, O, △) in the box provided for each plate on the Scoring Sheet. Record the exact symbols seen in the location indicated by the examinee or place a check in the small box if no symbol is seen.
INCORRECT RECORDING IS A MAJOR CAUSE OF RETURN TO THE CLINIC and DELAYS IN PROCESSING.
