About this file
This is a Past Performance Questionnaire template for use in evaluating contractor performance on CBP Medical Support Services contracts. The questionnaire is designed to be completed by government representatives or past clients who have worked with prospective offerors on similar medical services contracts. Respondents are asked to provide candid assessments and detailed comments regarding the contractor's past performance across six primary evaluation categories: Quality and Accuracy of Medical Services (medical examinations, testing accuracy, and regulatory compliance); Timeliness and Schedule Adherence (meeting deadlines and responsiveness to urgent requests); Security and Confidentiality (protection of personally identifiable information and health data); Communication and Reporting (interaction with government representatives and accuracy of reports and invoices); Contract Administration and Management (adherence to contract terms, staffing, and subcontractor/facility management); and Overall Performance (strengths, weaknesses, and notable accomplishments). Respondents rate performance using a six-point scale: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Unknown.
The questionnaire collects administrative information including the contractor's name, point of contact, contract number, awarded amount, current contract value, contract type (FFP, T&M, CPFF, CPIF, CPAF, or other), period of performance, and location of performance. It concludes with a question asking whether the respondent would choose to work with the contractor again, with response options ranging from "Definitely Would" to "Definitely Would Not." This questionnaire supports the Phase I evaluation process for Solicitation 70B06C26R00000017, CBP's nationwide medical support services requirement covering pre-employment and employee medical examinations, fitness testing, drug testing, and related services with historical annual volumes exceeding 40,000 medical exams, 50,000 fitness tests, and 45,000 drug tests.
