ADDITIONAL INFORAMTION ON FACE MASKS/COVERINGS
The following are procedures for face covering on Fort Gordon. Please ensure that your personnel follow the procedures. Failure to follow could result in removal from or denial of entry onto the Fort Gordon installation.
W9124920R0020
(1) All individuals on Fort Gordon will wear face coverings in all Fort Gordon Facilities. This applies to all DoD facilities on Fort Gordon, and includes but is not limited to, wearing face coverings in entrances and exits, hallways, elevators, bathrooms, concessions, and ATMs. Personnel may remove face coverings in one's individual work space. This requirement does not apply to private on-post residences.
(2) Service members will also wear face coverings when off the installation, and outside of the home or dwelling, to include when riding as a passenger in a motor vehicle with any person who is not a regular member of the household.
(3) Commanders and supervisors in the grade of O-5/GS-14 (or equivalent), may approve exceptions in writing for individuals who are unable to, or for whom it is impractical to, wear a face covering, i.e. - a documented medical need exists, when wear of a face covering is medically unsafe, or when precluded by mission requirements.
(4) Failure to wear face coverings as required in paragraph (1)-(2) above may result in punitive action for Service members, or removal from and denial of entry onto the Fort Gordon installation for all others.
Fort Gordon Base Access Entrance Permits: Per Secretary of the Army Directive, all Army installations will now vet all non-Department of Defense (DoD) employees, volunteers, transportation providers, etc. All non DoD ID card holders will be required to have a National Crime Information Center (NCIC) III background check. The NCIC III background check will run a full criminal history of the individual. Any felony conviction within the past 10 years will automatically place the individual under a suitability review for access denial to the installation. If the individual chooses to appeal, guidelines set forth by the SEC Army Directive for appeals, can only be submitted by the Government COR/sponsor (not the contractor). The Government COR/sponsor must endorse the appeal;
otherwise the appeal will not go forward. Effective 1 Oct 2014, those personnel who have not completed a background check or have not submitted consent forms will be denied access to the installation.
i. All submissions for background checks must include the following:
1. PS Form 190-6 Consent form (signed by the COR/sponsor)
2. Pass request spreadsheet (multiple submissions only)
3. Copy of driver's license (due to erroneous submission of DL#s and illegible writing)
4. Copy of social security card (due to erroneous submission of SSNs and illegible writing)
ii. The Contractor is responsible to provide complete information for each individual requiring access to the Site Visit. The information needed must contain the following:
1. Full Name
2. Social Security Number
3. Identification Number or valid picture ID
4. Duration of time this individual needs access to the jobsite
iii. The ID card that is issued as a result of this action is accountable and must be turned in before final payment can be processed for this contract.
iv. ID Cards will be confiscated if an individual tries to use another person’s card. This office supports termination of the individual that uses another person’s card.
