About this file

This is a Performance Relevancy Questionnaire (Exhibit A) that establishes the evaluation criteria and submission requirements for past performance documentation in response to solicitation W50S8R-26-R-A005 for Building 403 CYBER Repair. Offerors must submit no more than three (3) completed questionnaires describing relevant projects completed within the past five years and at least 70% complete if ongoing. Each questionnaire requires detailed information on the project title, contract value, completion dates, specific work performed, subcontractors and key personnel involved, cybersecurity requirements (particularly ICD-705 compliance), and similarity assessment relative to the proposed effort. The questionnaire also requires contact information for government references and mandates submission of Exhibit C packages to obtain third-party verification of past performance claims.

Part 2 of the questionnaire addresses company-wide compliance requirements applicable to all offerors, including documentation of small business subcontracting utilization in accordance with FAR 52.219-8, disclosure of problems encountered on relevant projects with corrective actions taken, and disclosure of any liquidated damages or contract terminations within the past five years. The underlying solicitation is an unrestricted, firm fixed-price procurement for commercial construction services valued between $10,000,000 and $20,000,000 with a 540-calendar-day performance period. The project involves cyberoperations facility improvements at the 179th Cyberspace Wing in Mansfield, Ohio, with mandatory compliance to ICD-705 standards and CMMC Level 2 certification requirements for access to controlled unclassified specifications and drawings.
