The Defense Logistics Agency Energy is seeking contractors to provide rapid deployment and complete support of fuel requirements including diesel, gasoline, propane, and jet fuel on a regional or statewide basis to support disaster mobilization, emergency response, and recovery efforts for the Federal Emergency Management Agency and other authorized entities. Contractors must have the ability to deliver fuel and provide complete support to emergency relief areas as needed in response to fuel shortages or other validated requirements. The contract will establish contingency contractors in each FEMA region to purchase and transport fuel to sites designated by FEMA officials, with priority given to supporting FEMA regions. Interested parties must submit a completed past performance questionnaire by October 12, 2018 to be considered for this award.

ATTACHMENT 5
In addition to the Past Performance Questionnaire Survey attachment, each offeror must also complete the questions below and return the responses with its offer.
Responses should be specific and provide concise, straightforward descriptions of the offeror’s capability to perform the contract. Responses shall be limited to three (3) single-spaced 8 ½ x 11 pages per question. No smaller than 10 point font may be used. Responses pertaining to past performance shall address the following (any areas you do not address shall be identified as “Not Applicable (N/A)”):
1. Address your ability to conform to previous contract requirements, specifications, and standards of good workmanship by providing quality product and service. Please provide specific examples.
2. Address your ability to meet delivery schedules, respond to administrative issues in a timely matter, and to complete a contract. Please provide specific examples.
3. Address your commitment to maintain an acceptable level of customer relations through performance and satisfaction. This includes the offeror’s history of reasonable and cooperative behavior, participation in problem identification, resolution, and preventive measures. Please provide specific examples.
4. Describe your general trends in contract performance. Specifically, provide the number of orders placed on each identified contract and the number of times you under delivered for each order.

5. Address problems encountered on the identified contracts and the ability to correct these problems. Demonstrate specific actions taken for each problem and the outcome of each situation.

Performance MEETS contractual requirements. The performance of the areas being assessed contains minor issues or concerns, for which corrective actions taken by the Contractor were effective.
MARGINAL
Performance MEETS SOME contractual requirements. The performance of the areas being assessed includes significant problems, issues, or concerns for which corrective actions taken by the Contractor were only somewhat effective.
UNSATISFACTORY
Performance DOES NOT MEET contractual requirement. The performance of the areas being assessed includes serious problems, issues, or concerns for which the Contractor’s corrective actions were ineffective.
