PAST PERFORMANCE QUESTIONNAIRE SOLICITATION SPE605-18-R-0234 ATTACHMENT 5
In addition to the Past Performance Questionnaire Survey attachment, each offeror must also complete the questions below and return the responses with its offer.
Responses should be specific and provide concise, straightforward descriptions of the offeror’s capability to perform the contract. Responses shall be limited to three (3) single-spaced 8 ½ x 11 pages per question. No smaller than 10 point font may be used. Responses pertaining to past performance shall address the following (any areas you do not address shall be identified as “Not Applicable (N/A)”):
1. Address your ability to conform to previous contract requirements, specifications, and standards of good workmanship by providing quality product and service. Please provide specific examples.
2. Address your ability to meet delivery schedules, respond to administrative issues in a timely matter, and to complete a contract. Please provide specific examples.
3. Address your commitment to maintain an acceptable level of customer relations through performance and satisfaction. This includes the offeror’s history of reasonable and cooperative behavior, participation in problem identification, resolution, and preventive measures. Please provide specific examples.
4. Describe your general trends in contract performance. Specifically, provide the number of orders placed on each identified contract and the number of times you under delivered for each order. Please provide specific examples.
5. Address problems encountered on the identified contracts and the ability to correct these problems. Demonstrate specific actions taken for each problem and the outcome of each situation. Please provide specific examples.
6. Address your effectiveness of Project Management (to include use and control of subcontractors).
7. General Comments. Provide any other relevant performance information.

II. EVALUATION: The following questions pertain to the contractor’s record of past (within the past three (3) years) and current performance. The information that you provide will be used in the awarding of a federal contract. Therefore, it is important that our information be as factual and accurate as possible.
