1. Contracting Activity: Department of Veterans Affairs, VISN 10, Louis Stokes Cleveland Medical
Center will enter into a construction contract that requires the procurement of this item which requires justification for other than full and open competition. A purchase request number is not yet available.
2. Nature and/or Description of the Action Being Processed: This project is named “PACT
Realignment”. Existing space will be renovated and augmented to enable team‐based care aligned with the principles of PACT (Patient Aligned Care Team). Meeting space will allow for daily team huddles and optimal flow of patients throughout their clinic visit. Basement space will support upgraded mechanicals, while the second floor will house clinical exam and support space. In addition, this project addresses several outstanding FCA (Facility Condition Assessment) items.
Patient experience as measured by SHEP scores are expected to improve materially as a direct result of this project.
This project provides an additional 12,000 square feet on three floors by an extension on VA owned land on the east side of the ambulatory care addition. Project also renovates 12,000 sqft of existing space to create the necessary clinical support space to properly implement the Patient
Aligned Care Team (PACT) model of primary care delivery to nearly 30,000 Veterans annually and while incorporating designs that support VHA patient access goals including same day access to
Primary Care.
3. Description of Supplies/Services Required to Meet the Agency’s Needs: The project requires a complete nurse call system which will not only need to be compatible with the existing nurse call system at the end of the project, but also at the end of each of the phases. The system will include annunciators, patient stations, duty stations, call and dome lights, and remote locator receivers.
Extending the existing system must be done with the same manufacturer (Hill Rom) in order to have a complete operational system. The total estimated cost for the Hill Rom Nurse Call System portion of the project is $10,798.
4. Statutory Authority Permitting Other than Full and Open Competition: 41 USC §3304(a)(1), as implemented by FAR 6.302‐1.

5. Demonstration that the Contractor’s Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): While there are other nurse call systems available, the Medical Center utilizes Hill Rom beds that plug into the existing Hill Rom nurse call system. Continuation of the existing system is the only logical solution to continue a campus wide nurse call system.
6. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: This procurement will be competed and synopsized on (www.fbo.gov) as an
Invitation for Bid (IFB) and will indicate the requirement for brand name specification for the Hill
Nurse Call System. Since the construction contract is being awarded as a firm fixed price contract through competitive sealed bidding, the bidders are incentivized to locate the supplier that will provide the item at the most economical price.
7. Determination by the CO that the Anticipated Cost to the Government will be Fair and
Reasonable: While FAR 8.404(d) states that GSA has already determined the prices of supplies under schedule contracts to be fair and reasonable the Contracting Officer intends to seek further price reductions. As the Navicare System is the preferred system to integrate the remainder of the
Medical Center the Contracting Officer has determined the procurement to be the best value to meet the Government’s needs. In addition, it is anticipated that two (2) or more bids will be received through the sealed bidding process. If two (2) or more bids are received than the cost of the entire project will be considered fair and reasonable to the government based on competition.
8. Description of the Market Research Conducted and the Results, or a Statement of the Reasons
Market Research Was Not Conducted: Since this is the existing campus wide system and patching in another manufacturer to it is not possible, no additional market research was conducted and the decision was made to sole source the existing system. (Market Research will be conducted for the entire construction project).
9. Any Other Facts Supporting the Use of Other than Full and Open Competition: None. See Item 5 above.
10. Listing of Sources that Expressed, in Writing, an Interest in the Acquisition: None. (A Market
Survey will be completed on the entire construction project).
11. A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to
Competition before Making subsequent acquisitions for the supplies or services required: None as it would be impractical to operate multiple types of call systems within an existing operation.
12. Requirements Certification: I certify that the requirement outlined in this justification is a Bona
Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.
