Summary: A/E Design Narrative - 100% Submittal for IR Cath Lab Renovation
This document is an Architect/Engineer (A/E) Design Narrative for a federal construction project involving the renovation of an Interventional Radiology (IR) Catheterization Lab at the West Palm Beach VA Medical Center (VAMC). The project, designated 548-CSI-139, encompasses approximately 2,000 square feet on the Second Floor of Building 1 and includes enlargement of the existing IR Cath Lab to accommodate new Philips monoplane procedural equipment and Baxter ceiling-mounted booms, along with supporting spaces including a semi-restricted supply room, enlarged control room, equipment alcove, scrub sink alcove, and a new ICR equipment room. The design follows the VA Design Guide PG 18-12 specifications for a Monoplane Hybrid Operating Room (ORHY1) and incorporates lead shielding per VA physicist requirements.
The project will be completed in three sequential phases to maintain operations of the adjacent Cardiac Lab throughout construction. Phase 1 involves modifications to the Cardiac Lab entrance and creation of new semi-restricted access areas, Phase 2 encompasses demolition and renovation of the IR Cath Lab space, and Phase 3 includes final finishes and systems installation. Key specifications include: VA-approved HVAC systems requiring replacement of existing AHU-60 with increased capacity for epidemic mode operation; fire protection modifications compliant with NFPA 13 and 72 (2022); plumbing extensions for medical gas, domestic water, and sanitary systems; electrical distribution from existing panels with new isolated power panels for imaging equipment; structural reinforcement for C-Arm mounting; and communications infrastructure including Cat6A UTP cabling for Voice over IP systems. An environmental assessment confirmed no asbestos-containing materials or water damage in the scope area, though presumed lead-containing materials may exist in perimeter wall interiors. Construction requires strict coordination with the VA Contracting Officer regarding scheduling, phasing, and infection control protocols (ICRA/PCRA), with temporary 1-hour fire-rated barriers and negative air pressure maintenance mandatory during all work phases.

9. ACI 318-19 Building Code Requirements for Reinforced Concrete
10. AISC 360-16 Specification for Structural Steel Buildings
Existing Building Construction:
The West Palm Beach Veterans Affairs Medical Center opened in 1995. Existing contract documents for the structure dated August 20, 1990, prepared by Wolfberg, Alvarez and Associates were provided to Above Group for review. The structural set is Volume 3, which includes sheets 1-
S1 through 1-S128.
The existing structure is a concrete column and beam frame with a concrete joist and slab with topping deck. The structure is a proprietary system combination of both cast-in-place and prestressed precast concrete. The concrete joists and the bottom of the concrete beams
(referred to as “soffit” beams) are both prestressed precast concrete, which then has a cast-in place remainder of the beam and/or deck poured on top. Typical slab construction is a 3” concrete topping on 3” concrete deck with 16” deep precast concrete joists spaced at 4’-6” o.c.
A typical bay is 27’-0”x27’-0” with 24” wide x 28” deep beams including the precast soffit at the bottom. Columns are typically 24”x24”.
Due to the nature of the existing structure, the capacity of the existing beams and joists cannot be determined by the original structural contract documents. The delegated design precast soffit beam and joist shop drawings are required. A portion of the existing precast concrete shop drawings were located and scanned as part of another project, but the beams and joists specifically for those in the area of work have not yet been found. Structural capacity is based on limited data for the design loads for the existing precast concrete joists and design moments and shears for soffit beams.
Design Loads:
• Dead Loads: Refer to calculations in Appendix.
• Live Load:
o Support Area Floor: 100 psf o Interstitial: 25 psf (15 psf when combined with floor live load)
• Seismic Load o Assumed Site Class ‘D’ o Seismic Design Category A
• Risk Category IV (Critical Facility)
2nd Floor Monoplane Hybrid Operating Room (ORHY1):
The proposed monoplane hybrid operating room will include the removal and replacement of catheterization lab (cath lab) equipment in replacement of the existing in room 2B-234 and associated support spaces. The cath lab table and other floor mounted equipment will be supported directly by the 2nd floor, while the C-arm and other ceiling hung equipment will be hung from the ceiling system. Due to the weight of the C-Arm, structural steel framing will be required to span between columns to support it. The combined weight of the C-Arm with the additional hanging equipment exceeds the design capacity of the existing precast concrete joists and steel subframe will need to be provided. The subframe is located at the lowest elevation possible to avoid conflict with major existing utilities and will need to be spliced at several locations to permit installation in the interstitial space. Some minor utilities will need to be rerouted to permit installation of the steel framing. Final ceiling system support loads need to be provided in construction and final design of the steel structure will need to be performed by the Contractor.
Some medical equipment manufacturers have specific vibration requirements. Once the vibration requirements for the C-Arm equipment have been determined, a vibration consultant can be hired to determine if the existing space is capable of meeting the requirements. The vibration consultant will perform testing in the existing space and provide the vibrations present under normal working conditions. If the existing space does not meet the manufacturer’s vibrations requirements, structural reinforcement/remediation will be required, or a different space will need to be selected.
2nd Floor Penetrations/Anchor Bolts:
The existing precast concrete joists and soffit beams utilize prestressed strand reinforcement. If a prestressed strand is cut, the force in the strand is released and serious hazards and damage to the existing building can result. Therefore, it is critical that coordination of any new holes and anchors through the structural floor slab be coordinated to avoid cutting through the joist and girders. Notes are included on the drawings to this effect and the Contractor is to perform GPR scanning to show any reinforcement prior to cutting any holes in the existing slab.
3rd Floor Anchor Bolts:
The existing structural system is the same as the 2nd floor, however, GPR scanning can be avoided by attaching anchors for equipment to the side of the precast concrete joists and beams in lieu of the bottom of beams where strands might exist.
2nd Floor Air Handling Unit (AHU) Platform:
Due to the increase in size of the space being served, the AHU serving the space will need to be replaced, refer to Mechanical. The existing unit is supported on an elevated steel platform in the 2A-304A Mechanical Room. As a result of the change in AHU, the platform will be extended to accommodate a longer or wider unit.
1st Floor Roof Exhaust Fan Penetration:
The new exhaust fan penetration is to be located such that it fits between existing precast concrete joists. The ductwork to the fan will need to be anchored down for hurricane wind resistance through threaded rods with eyebolts through bolted to the existing roof slab as the existing 4” thick concrete slab is too thin for use of post-installed anchors.

1.0 INTRODUCTION
OHC Environmental Engineering, Inc., (OHC), was contracted by RDC/JPA Architecture, to perform a comprehensive asbestos and water damage/mold assessment on IR Cath Lab at the West Palm Beach VA Hospital. The survey was performed on 02/28/2024 at the location described above.
2.0 BUILDING DESCRIPTION
The West Palm Beach VA Hospital is a multi-level building with a basement and interstitial spaces. The IR Cath Lab, interstitial space above the lab area, and mechanical room housing the A/H units supplying the area encompassed the scope of this survey. The walls of the lab area are drywall with a dropdown non-suspect cellulose ceiling tiles above. The flooring of the lab area is VCT and sheet flooring with yellow mastic underneath. The interstitial space has concrete walls, columns, and flooring. A/C duct work, water lines with TSI, and electrical cable trays comprised the interstitial space above the IR Cath Lab area. The mechanical room had two separate A/H units which supplied the area, both were inspected and deemed to be clean and in proper working order.
3.0 EXECUTIVE SUMMARY
3.1 Asbestos Survey Results
Based on the results of the Polarized Light Microscopy (PLM) laboratory analysis, asbestos-containing materials (ACM) are not present within the scope of the survey.
3.2 Lead Survey Results
No suspect lead paint was found within the scope of the survey; however, it is possible that the interior of the walls around the perimeter of the Angio Lab contain a lead lining to protect against radiation, because of this the interior of the walls are considered presumed lead containing material (PLCM) as indicated in
Table 1 below.
3.3 Mold and Water Damaged Material Assessment
A visual assessment was conducted throughout the IR Cath Lab area which included a nurse’s station, holding rooms 1 through 3, a storage room, a restroom, a control room, the Angio Lab, an Angio Lab storage room and scrub room, and the corridor connecting all these separate rooms. A moisture meter was used to assess the moisture content of surfaces and an IR camera was used to scan all surfaces in the area to determine if any moisture could be detected within the walls, flooring, or ceiling. No readings of high moisture were detected with the moisture meter and no abnormal readings were found with the IR camera. Note that no core sampling was done on the walls in the IR Cath Lab area to eliminate any dust created which would have compromised the sterile environment.

4.2 Asbestos Sampling & Analytical Methods
Asbestos sampling was conducted in accordance with NESHAP (40 CFR 61, Subpart
M) protocol for the commencement of demolition/renovation activities. The surveyor conducted a visual inspection of every safe and reasonably accessible room and space of the building. Materials sampled were grouped into Homogenous Sampling Areas
(HSA) based on the texture, color, use, age, condition, and every other visual appearance to identify suspect ACM. Materials containing asbestos are grouped into the following categories:
• Regulated ACM (RACM): Refers to friable manufactured ACM or a
Category I non-friable ACM that has become friable; or a Category I Non-
Friable ACM that will be or has been subjected to sanding, grinding, cutting or abrading; or a Category II Non-Friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition or renovation operations.
• Category I Non-Friable ACM: Refers to asbestos containing packing, gaskets, resilient floor coverings (including mastics), Galbestos, and asphalt roofing products containing more than 1% asbestos.
• Category II Non-Friable ACM: Refers to any material that is not a
Category I Non-Friable ACM that contains greater than 1% asbestos.
Bulk samples of all friable and non-friable suspect ACM were collected, as well as a representative number of samples from each homogeneous area following the EPA's simplified random sampling method (EPA560/585-030a). Good Industrial Hygiene practices were followed when collecting bulk samples to minimize fiber release.
Every precaution was taken to prevent asbestos exposure to the surveyor, the building occupants, and the public. All sample locations were logged with an appropriate description and the locations were marked on any available drawings. A unique sequential numbering system was used to identify each area. Each bulk sample was placed in a labeled bag, which was immediately marked with its sample number. Strict
Chain-of-Custody protocols were followed and signed by the receiving laboratory personnel who handled the samples. The samples were analyzed via PLM EPA-600 by EMSL Analytical in Orlando, FL. Quality control samples were analyzed by
EMSL Analytical in Tampa, FL for Quality Control procedures confirming the analysis of asbestos content. The purpose is to monitor the performance of the laboratories where samples are being analyzed to provide competence and reliability assurance.
4.3 Regulatory Requirements
Demolition: According to NESHAP, 40CFR61 Subpart M, demolition is defined as the wrecking or taking out of any load-supporting structural member of a facility together with any related handling operations or the intentional burning of any facility.
The final NESHAP Rule provides classification for regulated asbestos containing material as follows:
• Friable asbestos material.
• Category I non-friable ACM that has become friable.
• Category I non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading; or
• Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by forces expected to act upon the material during demolition or renovation operations.
If the total asbestos content is determined to be trace to 10%, the owner must either assume that the material contains greater than 1% asbestos and treat the material as regulated asbestos-containing material (RACM) or have the material verified/quantified by point counting. If after point counting, the material is quantified as 1% or less, it is not regulated by the NESHAP. The Occupational
Safety and Health Administration (OSHA) considers material that contains any amount of asbestos as asbestos-containing and requires compliance with OSHA regulations. The demolition of a structure with materials present that contain any amount of asbestos is considered by OSHA as an asbestos abatement, and all applicable OSHA rules must be complied with during the demolition.
Notification: Notification is required to the local regulatory agency:
1. Ten (10) working days prior to a demolition.
2. Ten (10) working days prior to a renovation operation, if the amount of asbestos material removed or impacted is greater than 160 SF on all building components (i.e. floor tile, mastic, GWBS, etc.) or 260 LF on pipes.
3. One (1) day prior to demolition if the building has been condemned and is structurally unsound as determined by the appropriate agency.
Notification must be sent by certified mail with return receipt or hand delivered to the Florida Department of Environmental Protection. The demolition contractor must wait ten (10) working days (Monday – Friday) from the postmarked date of mailing or the date of hand delivery to commencement of demolition. Any change to the start date of the demolition requires notification to the agency by phone, followed by a written revision to the Notification Form.
4.4 Statutory Requirements
The regulatory agency responsible for the oversight of the rules pertaining to asbestos-containing building materials (ACBM) is the Environmental Protection Agency
(EPA). The regulations require that prior to demolition or renovation a facility survey must be conducted in accordance with section 40 CFR 61-M National Emission
Standards for Hazardous Air Pollutants; Asbestos NESHAP Revisions, Final Rule.
Enforcement of these rules was passed on to the states. In the State of Louisiana, they are enforced by the Department of Environmental Protection (DEP). Some counties have developed an enforcement division to carry out the responsibilities of the DEP and have developed environmental and asbestos ordinances with which compliance is required.
5.0 LEAD SURVEY
No suspect lead paint was detected in the scope of the survey; however, it is possible that the interior of the walls around the perimeter of the Angio Lab contain a lead lining to protect against radiation, because of this the interior of the walls are considered presumed lead containing material (PLCM).
6.0 OBSERVATIONS
• Lead lining in walls of Angio Lab not sampled and are considered PLCM
• No suspect paint found in the scope of the survey therefore no samples for lead paint were collected
• No condensate coatings found on sinks
• No mirror mastic found behind mirrors
• No suspect electrical materials found
• Ceiling tiles not sampled due to the sterile environment however they all appear to be non-suspect cellulose tiles in the dropdown ceiling grid
• Sampling avoided as much as possible in Angio Lab 2B-234 due to the elevated sterile environment needs of the space
7.0 CONCLUSIONS
Based on the results of the PLM analysis, ACM is not present in the materials sampled.
No suspect paint was identified within the scope of the survey but the interior of the perimeter walls of the Angio Lab are considered presumed lead containing material
(PLCM) due to the potential for a protective lead lining.
No water damage or VPFG was idnetified in…
