Annual Monitoring and Inspections
Former Naval Complex, Adak, Alaska
1.0 INTRODUCTION
Naval Facilities Engineering Command Northwest is acquiring environmental services for the purpose of conducting environmental sampling and institutional control (IC) inspections as required by the Adak Operable Unit (OU) A and OU B-1 Record of Decisions (RODs) (References 1 and 2) and approved decision documents for 14 petroleum release sites (References (3 through 8).
2.0 BACKGROUND
The Navy, the Alaska Department of Environmental Conservation (ADEC), and the U.S. Environmental Protection Agency (EPA) signed a Record of Decision (ROD) for OU A in April 2000. In addition, the Navy and ADEC signed decision documents for 14 petroleum release sites. To implement the annual monitoring program, the Navy has prepared a Comprehensive Monitoring Plan (CMP) (References 9 and 10), which serves as the project work plan for implementing the monitoring and inspections required by the OU A and OU B-1 RODs and the 14 petroleum release sites decision documents (References 1 through 8).
3.0 OBJECTIVE
The objective of this task order is to conduct the annual monitoring and IC inspections and report on the findings per the requirements of the CMP and other requirements as further defined in this statement of work.

The SSHP shall comply with the U.S Army Corps of Engineers Safety and Health Requirements Manual EM 385-1-1 (Reference 11) and all applicable current NAVFAC NW SOPs (Reference 12). The SSHP shall be submitted in draft and final versions in accordance with the schedule listed in Section 5.0 Deliverables. With the final submittal, the contractor shall provide a written response to comments provided on the previous version. The SSHP shall be considered acceptable to the NAVFAC NW Safety Manager, or assigned representative prior to beginning any field work and the Contracting Officer’s Representative (COR) must give written approval of the SSHP before the contractor can mobilize for any on-site fieldwork.
4.2 Waste Management Plan
The contractor shall prepare a waste management plan (Reference 13) to cover all on-site activities planned for completion under this task order. The waste management plan should include the ADEC approved method for on-site treatment of petroleum purge water. All investigative derived waste (IDW) generated during the execution of this task order shall be disposed of off-island by the contractor. It is the contractor’s responsibility for disposal of the IDW and other waste generated under this task order. Only household waste generated within the housing rental units can be disposed of on-island at approved disposal facilities. The waste management plan shall be submitted in draft and final versions in accordance with the schedule listed in Section 5.0 Deliverables. With the final submittal, the contractor shall provide a written response to comments provided on the previous version.
4.3 Annual Monitoring
The contractor shall conduct the annual monitoring in accordance with the groundwater and landfill SAP portions of the CMP (References 9 and 10) including attachments or as updated. The contractor must complete the appropriate inspection forms for each area/site visited. Data collection and reporting shall follow the standard operating procedures as specified in the CMP including attachments. An independent third party not involved with data collection, analysis, or reporting shall validate all laboratory data. Data will be validated for all analysis performed by the laboratory with exception of waste characterization results, natural attenuation parameters, and water quality parameters. The contractor shall summarize in the report the results of the free product recovery (found in Reference 4) at monitoring wells identified as having free product during the annual monitoring event.
The contractor shall work closely with the laboratory to ensure there are no hold time exceedances and the detection limits for landfill sediments meet data quality objectives (DQOs). The contractor shall notify the RPM immediately if any hold times are exceeded or if sediment results do not meet DQOs.
Well caps without locks will be secured with plastic lock ties following sample collection. This applies to all wells sampled or measured under this task order. The contractor shall compile a list of wells that require maintenance including a description of what repair work is necessary. The repair list shall be provided to the RPM per the schedule listed in Section 5. The repair list shall be submitted via email in tabular format.
During all visual inspections (monitoring wells, sediment & surface water sampling and seep & shoreline), the contractor shall document site conditions with photographs that are date and time stamped.
4.4 Annual Monitoring Reports
The contractor shall prepare the annual monitoring reports in accordance with the requirements outlined in the groundwater and landfill SAP portions of the CMP (References 9 and 10) and will follow the format and content of the previous monitoring reports. The annual groundwater and landfill monitoring reports shall be prepared and submitted separately.
The groundwater and landfill monitoring report (References 14-16) shall include the sites listed in the groundwater SAP portion of the CMP and the landfill report sites included in landfill SAP portion of the CMP. The reports shall include a description of field and analytical methods, field observations, deviations from the approved work plans, trend analysis (as appropriate) and additional requirements included in this statement of work. Analytical results will be summarized in tabular format and compared with cleanup levels to assess the effectiveness of the remedies. All flagged analytical results shall be explained in the body of the report. Appended material shall be included at the back of each report. The appended material will be in electronic format (native file and/or portable document format) on CD/DVD(s). The annual reports will be comb bound. Three iterations of the combined groundwater and landfill report are required: internal draft, draft, and final. Line numbering shall be used for the main body of text in the internal draft and draft versions. All deliverables including quantity and type will be submitted in accordance with the schedule listed below in Section 5.0 Deliverables. Data presentation and format will follow previously submitted and approved annual reports. The contractor shall prepare response to comments with each applicable version (internal draft and draft). The response to agency comments submitted on the draft report will be included with the final report as an appendix. The appended material will also include field notes, field logs, inspection forms, sample deviation forms, data validation report, data summary tables, historical analytical results, labeled site photographs (time/date stamped), complete laboratory analytical results including field & laboratory QC, sample chain of custody forms, data validation reports, IDW disposal documentation, field sampling parameters, free product measurements and recovery data, and other information required to be reported in the planning documents . The contractor shall include water level and product depth information in both tabular and graphical format. The contractor shall review the agency comments and Navy responses on the previous year’s annual reports and incorporate any agreed upon requests or recommendations. The appended material shall be in electronic format (native file and/or portable document format) on CD/DVD. The appended material shall include all inspection and evaluation forms, field notes, annotated maps or figures, site photographs (time/date stamped), deviation forms, excavation notification requests and other pertinent information. The IC documentation shall be submitted in accordance with the schedule listed in Section 5.0 Deliverables and Performance Requirements. Data presentation and format will follow previously submitted and approved annual reports. The contractor shall prepare response to comments with each applicable version (internal draft and draft).
