CAF ADAIR “BLUE 1” CONTRACTOR EVALUATION
INSTRUCTIONS: This evaluation supports AMIC’s Program Management on the CAF ADAIR Contract. This form is filled out by the Contractor Flight Lead, for the entire formation, during the debrief. Blue 1 must validate all responses and Contractor Flight Lead Concur/Non-Concur with Blue 1’s validation. Then Contrator Site Lead concurs/does not concur and provides the completed, signed form to the GFR. This Evaluation form becomes UNCLASSIFIED//FOUO whenever data is inserted.
This form is an integral part of the contract surveillance program. Data collected here is utilized in both Program Management Reviews and Award Fee Boards for program oversight. This form is designed to be user friendly, and expeditious day-to-day. Firewall “Yes” answers equates to contractor meeting performance requirements and should be the norm. A “No” answer doesn’t mean failure to perform unless there was negative consequences to the blue formation and/or blue training. Blue Flight Leads are encouraged to provide subjective feedback whenever contractor performance is “poor” or goes “above and beyond.”

Flight Lead, for the entire formation, after the debrief. The mission lead must validate all responses and Contractor Flight Lead Concur/Non-Concur with the Mission Lead’s validation. The Contrator Site Lead concurs/does not concur and provides the signed form to the GFR. This Evaluation form becomes UNCLASSIFIED//FOUO whenever data is inserted.
