SECTION 1: GENERAL INSTRUCTIONS

This is a Total Small Business Set Aside competition requirement under NAICS code 611699 - All Other Miscellaneous Schools and Instruction, size standard $16.5M.

The Government intends to award one contract resulting from this Request for Quote (RFQ) to the responsible offeror whose quote represents the Highest Technically Rated Offeror (HTRO) with a fair and reasonable price in accordance with Federal Acquisition Regulation (FAR) Subpart 12.6, FAR 13.106, and FAR Subpart 13.5.

Offerors shall not submit more than one quote as the prime in response to this RFQ. Offerors submitting a prime quote are permitted to submit a separate quote as a subcontractor, or team member to another prime.

Only quotes submitted in accordance with these instructions will be accepted. The Government will retain one copy of each Offeror’s quote (FAR Subpart 4.8).

Offerors that do not have an active registration in System for Award Management (SAM.gov) at https:// sam.gov at the time quotes are due will not be considered for award.

Quotes shall be clear and concise and must include sufficient detail to permit the Government to adequately validate quote statements.

Quotes shall be validated one at a time followed by finding price fair and reasonable and not evaluating all offerors.

The Government reserves the right to cancel this RFQ at any point before contract award. In the event this RFQ is canceled, the Government has no obligation to reimburse Offerors for any costs incurred.

SECTION 2: QUOTE FORMAT AND SUBMISSION

Quotes in response to this RFQ shall be valid for 180 calendar days (FAR 52.215-1(d)).

2.1 Operating System and Applications

Quotes will be accessed with the Microsoft Windows 10 Operating System. Quote files must be submitted using only the applications listed below:

Documents submitted in any extension other than the three listed above will not be evaluated. This may result in a quote being considered unresponsive. Additionally, if self-scoring cannot be validated due to invalid file types, the offerors rating will be lowered.

2.2 Virus Free Electronic Submission

Offerors must ensure that quotes are virus free. The Offeror must certify, in the quote cover letter, that all electronic quote information delivered throughout the evaluation is virus free. Offerors should specify the software, version, and virus definition used to check the data.

2.3 Multimedia and Graphics

Sound and video (e.g., MPEG) files must not be imbedded within the quote. Offerors should:

a. Avoid submitting large files

b. Simplify color palettes and minimize the use of color images or graphics

c. Minimize size of graphics files

d. Avoid scanned images

2.5 Quote Formatting

Quote formatting does not apply to the Work Samples addressed in Section 3.2.1

2.5.1 Fonts: Quote text fonts shall not be smaller than Times New Roman, 11 point font, single column, with normal character spacing and position. The text for charts, tables, graphs, and figures shall be no smaller than Times New Roman, 10 point font. Line spacing shall be at least single- spaced.

2.5.2 Pages: All pages must be formatted as 8.5” x 11” letter size with one-inch margins all around. 11” x 17” is allowed for charts, tables, and graphs only. Any quote text included on these pages, other than the information specific to the graphics, will not be considered in the evaluation. One 11” x 17” page will be counted as two pages. All material must be contained within the page limits identified below for each section, and all appendices, charts, graphs, diagrams, tables, photographs, drawings, etc., are included in the page count. Not included in the page count are title pages, tables of content, glossaries of abbreviations, acronyms, or definitions, or copies of certificates (i.e. ISO9000). Evaluators will only evaluate up to the maximum number of pages specified in Section 3.

2.5.3 Headers and Footers: Information contained in headers and footers must only provide company name, solicitation number, disclosure markings (FAR 52.215-1(e)), dates, or page numbers. Text sizes must be no smaller than Microsoft Word, Times New Roman, 9 point font.

2.6 Quote Submission

2.6.1 Questions Submission: Questions in response to this RFQ are due no later than the date and time specified in the RFQ on the Government Point of Entry. All questions must be submitted via email and only to the Points of Contact below. It is the Offeror’s responsibility to confirm receipt.

3.1 Business Section

This section will include all non-evaluated requirements listed below. Failure to address all required documentation (negative responses included) may render the quote non-responsive.

3.1.1 Cover Letter. Includes:

(a) Name

(b) Address

(c) CAGE Code

(d) Unique Entity Identifier

(e) System for Award Management (SAM.gov) registration expiration date

(f) Quote validity statement

(g) Virus free electronic submission statement

(h) POC authorized to obligate the company (name, email, and phone)

3.1.2 Financial Information. To expedite the responsibility determination required by FAR 9.103(b), provide documentation of the following:

(a) Offeror’s adequate financial resources, or the ability to obtain them (FAR 9.104-3(a)).

i. Evidence may include official letter(s) from financial institutions demonstrating that the Offeror has the financial resources required to cover all financial commitments.

(b) Offeror’s organization, experience, accounting, and operational controls, and technical skills, or the ability to obtain them (FAR 9.104-1(e)).

i. Evidence may include a list of organizational policies or Standard Operating Procedures (SOPs) which demonstrates the organizational controls and measures of the Offeror.

ii. If providing a list, include the title of each policy or SOP, the date of each document, a brief description of each, and state to which element of FAR 9.104(e) it applies.

3.1.3. Pre-Award Documentation. The contractor will include the following in the pre-award documentation:

(a) Acknowledgement of the terms and conditions of the RFQ and amendments

(b) Any exception to the RFQ terms and conditions

Completion confirmation of the required Representations and Certifications in SAM.gov. The Offeror shall fill in all provisions. If any are not applicable, offeror shall annotate N/A so that none of the provision fill-ins are left blank. Offeror shall confirm that all Representations, and Certifications remain current throughout the evaluation process.

3.1.4 Partnering Documentation. Provide documentation, including signed agreements and SBA letters (13 CFR 125.8) as applicable, identifying the arrangement, and disclosing company relationships (FAR 9.603) for any Joint Venture, Teaming Arrangement, Mentor-Protégé, etc. established for the purposes of this requirement

3.1.5 Pricing Documentation. Provide the following:

(a) Completed Appendix D Total Evaluated Price Matrix. Please ensure that the document is completed in its entirety. An incomplete document will cause the proposal to be considered nonresponsive and the quote will be considered un-awardable. The government will not clarify incomplete documents.

3.2 Prior Technical Experience (PTE)

3.2.1 Work Samples. The Offeror will submit a maximum of 5 unique work samples which demonstrate their prior technical experience against the required Parachute Rigger & Instructor Training Services scoring elements, and which comply with the work sample qualifications established in 3.2.3 below.

3.2.1.1 When referring to work samples, the terms “contract” and “task order” are not interchangeable. The term “contract” will refer to a stand-alone contract action and the term “task order” shall refer to an order issued under an Indefinite Delivery Indefinite Quantity (IDIQ) contract under FAR 16.505 ordering procedures. IDIQ contracts and Blanket Purchase Agreements (BPAs) will not be counted as a valid work sample. Orders against IDIQs/BPAs do count as a valid work sample. Multiple task orders issued against IDIQs or BPAs cannot be combined to count as one sample.

3.2.1.2 Work samples will consist of official contract documentation (e.g. Performance Work Statements, Contract Data Requirement Lists [CDRLs], Contract Section A, etc.) or official corporate documentation (e.g. payroll data, company SOPs, HR manuals, etc.). When contracts are submitted as work samples, the Offeror will include the signed first page of the contract. The Government will accept a Statement of Fact from the cognizant government contracting officer or COR to confirm missing information, should contractual documents not carry all the information necessary to substantiate the Offeror's scores. The Offeror will not create any new documentation, such as project narratives, or project descriptions. Work samples shall include sufficient information to substantiate each claim. If a work sample substantiates multiple elements each element must be identified within the work sample. Work samples must be submitted by the Prime Offeror. Subcontractor proprietary information may be redacted. The redacted work sample must stand on its own to be verified against the CRM.

In lieu of official contract documentation the government will also accept a statement of fact signed by the cognizant government contracting officer or COR. At a minimum a statement of fact shall include the Contract Number, Period of Performance, Contracting Officer or Contracting Officer Representatives Information (Name, Telephone Number, and Email), and a brief statement outlining how the contractor met the criteria being evaluated. For example, a contracting officer or COR may write, “On contract XYZ company ABC provided personnel to conduct medical training for the LMN Squadron. Over the life of the contract XYZ company hired 8 contractors that qualify as those with advanced medical qualifications, 10 contractors with current NREMT-P, and 6 contractors with DoD Special Operations Medical Instructor Experience.” A sample statement of fact is attached to this solicitation.

3.2.1.3 Offerors will give each work sample an identifier (i.e., WS1-WS5). All pages within the work samples will be numbered. Work samples will be provided in their original format; page numbers may be added. Font, text size, and margin requirements established in Section 2.6 do not apply to work sample submissions. Work samples must be in English.

3.2.2 Work Sample Cover Sheet. See Appendix A for the Work Sample Cover Sheet template that must be submitted with each work sample. Information on this page will not be used to validate contractor claims.

3.2.2 Work Sample Qualifications. Contracting documentation work samples that do not meet the minimum qualifications below will not be considered by the Government in validating their associated self-rated score.

(a) Shall have at least six months of active performance within the last five years, as of the date of RFQ issuance.

(b) Shall fall into one of the following categories:

i. Prime Offeror work samples. If a company is proposing as a Joint Venture, work samples will be considered from either member or from the JV. The Prime Offeror may have been a subcontractor on the work sample.

ii. Subcontractor/Teaming Partner work samples. The Subcontractor/Teaming Partner shall:

1) be identified as a Subcontractor or Teaming Partner as required by section 3.1.4;

2) have acted as the Prime or Subcontractor on the submitted work sample.

3.2.4 Self-Rated Scoring Matrix. The Offeror shall complete the Self-Rated Scoring Matrix in Appendix B. Self-rated scores shall be verifiable using the work samples provided. The burden of proof to substantiate the Offeror’s self-rated score rests with the Offeror. The work samples shall include all the evidence required to validate the proposed self-score.

3.2.4.1 The Offeror shall complete the “Offeror Response” column (in green) on the Self-Rated Scoring Matrix. The Offeror shall populate the matrix with the requisite Full Time Equivalent (FTE), Contract quantity, or other requested information and not the corresponding score. The Offeror’s Weighted Score will be calculated automatically based on these inputs. Unless stated otherwise, the Offeror shall not change, adjust, or manipulate any other cells within the matrix. The Offeror shall accurately rate itself using the scoring instructions for each element described in 3.2.5.

3.2.4.2 Based on the offerors input the corresponding point total shown in row 3 will be multiplied

by the weight factor shown in column G. These weighted totals will then be added for each
category to get the offerors score for each category. The three category scores will then be
added to get the offerors total weighted score. For this scoring matrix the maximum total
points available is 6,500 points (4,100 for category I; 900 for category II; and 1,500 for category III)

3.2.4.3 At least 50% of the Offeror’s total self-rated score must come from the Prime Offeror’s work samples. If the offeror fails to prove this in their work the samples, they will not be considered for award.

3.2.5 Categories and Definitions of Scoring Matrix.

3.2.5.1 Category I – Parachute Rigger & Instructor Labor Experience

DEFINITION: Based on the submitted work samples the Offeror shall provide the number of Parachute Rigger & Instructor Training specific personnel placed in each of the positions listed.

Number of contractors placed in instructor positions - AFSPECWAR/DoD Special Operations equivalent. Instructors with experience in Air Force Special Warfare (AFSPECWAR)/Department of Defense (DoD) Special Operations encompasses all instructor positions of Air Force Special Operations (Pararescue (PJ), Combat Control Technician (CCT), Joint Terminal Attack Controller (JTAC), Tactical Air Control Party (TACP), Special Reconnaissance) and other Special Operations such as Army Green Beret (SF), Army Ranger, Navy Sea Air & Land (SEAL), Navy Special Warfare Combatant Craft (SWCC) Marine Recon, Marine Raider, Delta Force Operator, Development Group (DEVGRU).

Number of WEPTAC instructors performing duties in AFSPECWAR or SOF contracts. Weapons Tactics (WEPTAC) instructors involved in the training of proficient weapons handling and employment for Air Force Special Operations (Pararescue, Combat Control Technician, Joint Terminal Attack Controller, Tactical Air Control Party, Special Reconnaissance) and other Special Operations such as Army Green Beret (SF), Army Ranger, Navy Sea Air & Land (SEAL), Navy Special Warfare Combatant Craft (SWCC) Marine Recon, Marine Raider, Delta Force Operator, Development Group (DEVGRU).

Experience level of personnel placed in this category - WEPTAC. Experience level is broken down into Evaluator, Instructor, or number of years of instruction for personnel involved in the instruction of Special Operations medical positions such as Pararescue, SEAL Medic, Green Beret Medic. Comment by DEDMON, JEREMY M CIV USAF ACC 355 CONS/PKB: Definition of Evaluator, Instructor, number of years. Comment by IBARRA, DANNY F III CIV USAF ACC 68 RQS/AFE: Evaluator - EJ means a person who has conducted 7-level upgrade training and has been selected by a commander to conduct evaluation duties performed by that specific function or skill (i.e. flight examiner (FE)). EJ is a specifically a coding for Pararescue AFSC.

Instructor - IJ means a person who has conducted 5-level or above upgrade training and has been selected by flight leadership or above to conduct instructor duties for UGT for lesser qualified personnel. IJ is a specific coding for each AFSC.

Number of years - desired a minimum of SOF Medic - 4 years serving is such a position.

JTAC - 4 years serving as an operational JTAC with at least 1 deployment.

WEPTAC - 6 years serving in an operational organization with at least 1 combat deployment.

Experience level of personnel placed in this category - JTAC. Experience level is broken down into Evaluator, Instructor, or number of years of instruction for personnel involved in the instruction of Special Operations medical positions such as Pararescue, SEAL Medic, Green Beret Medic.

Experience level of personnel placed in this category - AFSPECWAR/SOF Medic. Experience level is broken down into Evaluator, Instructor, or number of years of instruction for personnel involved in the instruction of Special Operations medical positions such as Pararescue, SEAL Medic, Green Beret Medic.

Master Riggers with 3-yrs experience placed in AFSPECWAR or DoD Special Operations positions. Master Parachute Riggers with 3-yrs experience in Military Free Fall and Static Line jump operations for Air Force Special Operations (Pararescue, Combat Control Technician, Joint Terminal Attack Controller, Tactical Air Control Party, Special Reconnaissance) and other Special Operations such as Army Green Beret (SF), Army Ranger, Navy Sea Air & Land (SEAL), Navy Special Warfare Combatant Craft (SWCC) Marine Recon, Marine Raider, Delta Force Operator, Development Group (DEVGRU).

Senior Riggers with 2-yrs experience placed in AFSPECWAR or DoD Special Operations positions. Master Parachute Riggers with 2-yrs experience in Military Free Fall and Static Line jump operations for Air Force Special Operations (Pararescue, Combat Control Technician, Joint Terminal Attack Controller, Tactical Air Control Party, Special Reconnaissance) and other Special Operations such as Army Green Beret (SF), Army Ranger, Navy Sea Air & Land (SEAL), Navy Special Warfare Combatant Craft (SWCC) Marine Recon, Marine Raider, Delta Force Operator, Development Group (DEVGRU).

INDOPACOM / Near Peer Threat Planning / Cobra Gold or similar JCS Experience placed personnel with experience in this level. Indo-Pacific Command (INDOPACOM), Modern Adversary, Gold Cobra: multi-national Indo-Pacific military exercise level planning for realistic training scenarios.

Experienced JTAC contractors with over 60 Combat Strikes placed on DoD Contracts – Number of JTAC personnel that have worked on DoD contracts that have over 60 combat strike.

Number of Contractors with Military Airborne Operations Qualifications placed. Ability to show qualifications of the movement of armed forces into an objective area for ground combat operations.

Number of contractors placed with advanced medical qualification (Flight Paramedic, Advanced Tactical Practioner, American Heart Association Instructor, Advanced Cardiac Life Support Instructor, or Critical Care Paramedic)

Number of personnel placed with NREMT-P currency. National Registry of Emergency Medical Technicians – Paramedic.

DoD Special Operations Medical Instructor Experience

3.2.5.2 Category II –Parachute Rigger and Instructor Corporate Experience DEFINITION: Based on the submitted work samples the Offeror shall provide the response indicated for each element.

For average time filling vacancies, work sample(s) must show average over one contract or task order performance year. Work sample may not be a position that was filled by an incumbent contractor/employee. Work sample must show time from notification of vacancy to time contracted employee started on site.

Experience with Guardian Angel/AFSPECWAR training requirements. Length of time involved in Guardian Angel (Pararescue) and Air Force Special Warfare training curriculums, iterations, lifecycles, planning, execution. Personnel must have performed functions for at least 2 years.

For the contract with a multi-discipline Guardian Angel/AFSPECWAR contractors, the work sample must show a team of three or more.

3.2.5.3 Category III – Organizational Metrics

DEFINITION: Based on the submitted work samples the Offeror shall answer yes or no for each element, except retention.

For the retention rate of Parachute Rigger & Instructor element, the response should tie back to the personnel listed in Category I. Based on submitted work samples, the Offeror shall calculate its retention rate as follows: first, identify the total number of positions across all work samples used to respond to Category I (i.e. the number of positions required to complete work defined in the PWS), known as “starting headcount”. Then, identify those positions that once filled did NOT turnover for at least two calendar years, known as “ending headcount”; the Offeror will then divide the ending headcount by the starting headcount and multiply by 100. Only enter in percentages whole numbers rounded down to the next whole number. For example, 84.7% must be entered at 84%.

Offeror must ensure PTO is mandated due ensuring companies having the ability to retain employes Offerors shall include with the retention rate work sample(s) an Excel spreadsheet showing their calculation with visible formulas. We need to make sure that if an employee is missing for more than 5 days, they can provide a suitable substitute. This is usually a requirement due to course schedule and working with reservists that will leave in the middle of a course for 2 weeks and we need them during that time.

For the Secret Facility Clearance element, a Secret or higher must be held by all members of a Joint Venture. Submit security clearance documentation as a work sample.

3.2.6 Cross Reference Matrix. The Offeror shall provide a Cross Reference Matrix (CRM) which clearly identifies the exact location of the substantiating data within the work samples. The CRM shall be completed using the Appendix C Cross Reference Matrix. Multiple references may be included in a single row only if necessary to substantiate the Offeror’s claim.
