Amended Safety Plan

1.4 Administration

1.4.1 The Contractor will serve as the “safety authority” for activities performed at work sites under its control. As such, the Contractor will be responsible for implementing proactive preventive measures, providing adequate instruction to employees and sub-contractors, coordinating operations to prevent work by separate trades from interfering with or creating hazardous conditions for each other, and performing routine surveillance adequate to ensure compliance with safety requirements.

1.4.2 Project Manager (and Field Superintendent where applicable) will perform daily oversight and enforcement of the safety plan at each job site. Ensure employee, sub-contractor and visitor compliance. Provide additional surveillance when hazardous operations are on-going.

1.4.3 Provide additional safety instruction for workers prior to execution of hazardous activities.

1.4.4 Develop job specific plans if and when required by separate project requirements.

Describe preventative measures, training, equipment, worker responsibilities and method of monitoring.

1.4.4.1 Confined Space Entry Plan – Comply with applicable 29 CFR 1910, 1915, and

1926 standards. Coordinate plan and scheduled operations with Base emergency responders (Fire Department, Security and Medical).

1.4.4.2 Fall Protection and Prevention Plan – Describe means of rescue (self and assisted) and evacuation procedures.

1.4.4.3 Hazardous Material Abatement Plan – Describe removal and disposal methods:

Lead based paint (LBP)

Asbestos Containing Materials ACM)

Polychlorinated biphenyl (PCB).

1.4.4.4 Site Demolition Plan

1.4.4.5 Excavation Plan

1.4.5 The Contractor shall be responsible for providing first aid and medical assistance if their employees are injured or become ill. Provide a first aid kit at individual job sites with materials suitable for the type of injuries normally incurred by the type of work being performed. Locate where readily accessible during working hours.

1.4.6 Document each accident on same day as event and provide copy to CO and CR.

Indicate results of mishap (to person and property), cause and corrective action taken.

Maintain a record of accidents for duration of work at job site.

1.4.7 Ensure personnel are aware of nearest safe shelter-in-place location.

1.5 Government Oversight

1.5.1 The Government reserves the right to determine effectiveness of Contractor’s accident prevention procedures. Failure to implement and maintain an effective safety plan will constitute justification for Government issuance of a suspension of work. The suspension of work will remain in effect until corrective actions satisfactory to the

Government are implemented.

1.5.2 The Government reserves the right to require the Contractor make changes to safety procedures. To the extent deemed necessary, Contractor safety procedures will comply with EM 385-1-1 and NFPA 241 (Safeguarding Construction, Alteration and Demolition

Operations). Action required to obtain compliance will be performed by the Contractor at no additional cost to the Government.

1.5.3 Medical, Security and Fire Protection Assistance: The Base will provide emergency response services, when and if deemed warranted, for medical, security and fire protection. Responding Base emergency personnel will have the right to determine if support from other sources (federal, state, municipal or private commercial) is required.

1.5.4 Air Monitoring: The Government reserves the right to conduct separate air monitoring procedures in conjunction with work performed in confined spaces.

1.6 Contractor Request for Emergency Services

Dial 911. State location from where call is being made (Altus AFB). 911 Operator will redirect your call to the Base emergency operator. Provide information as requested. The Contractor will bear all costs required to respond to emergency service requests.

1.7 Closure and Obstruction of Operational Facilities

Obtain Government approval prior to commencement of work requiring closure or obstruction

(partial or complete) of occupied buildings and pedestrian and vehicular pathways.

1.8 Licensed Contractor

1.8.1 Perform work on gas lines and equipment, electrical lines and equipment, and mechanical equipment using only licensed contractors or with experienced craftsmen under the direct supervision of licensed contractors. Provide Government with a copy of license prior to start of work.

1.9 High Noise Level Protection

Wherein high noise levels generated by equipment are deemed to be a safety or health hazard by the Government, the Contractor will immediately cease the work in question and re-schedule execution during non-standard time periods. Non-standard time periods will be subject to

Government approval. Said work will be performed without additional cost to the Government.

1.10 Severe Storm Plan

Plan will indicate means and methods to quickly:

(a) Secure and protect equipment and materials at outside locations.

(b) Inspect work site (including roof) and remove or secure loose debris and other objects that could be scattered by blowing wind or flowing water. Verify adequacy of temporary erosion controls where applicable.

1.11 Fire Safety

1.11.1 Work sites and storage areas will be designated as “non-smoking” areas. Smoking will be restricted to confines approved by Base Fire Protection (Building 288). Smoking materials will be disposed in non-combustible receptacles approved by Base Fire

Protection.

1.11.2 Open fires and burning of trash on Government property is prohibited.

1.11.3 Welding, Cutting and Brazing Permit:

1.11.3.1 Obtain approved permit (AF Form 592), prior to start of work, from Base Fire

Chief Office (Building 288). Verify conditions at work site comply with applicable provisions of AFOSH Standard 91-5 (Welding, Cutting and Brazing); UFC 3-600-01

(Fire Protection Engineering for Facilities); EM 385-1-1 (Safety and Health

Requirements Manual) and NFPA 241 (Safeguarding Construction, Alteration and

Demolition Operations) prior to requesting pre-issuance site inspection by Base Fire

Protection. Maintain permit at job site until completion of operations.

1.11.3.2 Provide portable fire extinguisher(s) at “hot work” locations. Ensure workers at said locations are knowledgeable in equipment operation and application procedures. Inspect equipment as required to verify operability.

1.11.4 Portable Fire Prevention Equipment:

1.11.4.1 Government reserves the right to inspect and determine acceptability of

Contractor’s equipment. Equipment will be provided as acceptable to Base Fire

Department without additional cost to the Government.

Maintain unobstructed access and egress corridors within and adjacent to buildings.

1.12 Material Storage

1.12.1 Supporting Surface: Support materials on level, sound, and stable surface.

1.12.2 Stacking

1.12.2.1 Stack outside materials above ground. Use means that will prevent compromise of support in the event of severe rain fall.

1.12.2.2 Use means and methods to ensure stability without need for bracing. Means and method will be adequate to prevent sliding, spreading, tilting or collapse. Stack bagged material in crisscrossed alternating layers so ends and sides do not form continuous vertical breaks through the stack.

1.12.2.3 Limit stack height to 4 feet.

1.12.2 The storing of materials and equipment on roofs is prohibited without written

Government approval.
