About this file

This document is a Quarterly Programmatic Report template for Expulsion Prevention Services involving the Arizona Department of Economic Security (ADES) and related child care providers. The report must be submitted electronically to dcccontracts@azdes.gov within 15 calendar days of each quarter's end, requiring comprehensive documentation of collaboration efforts and program outcomes across multiple domains including child care providers, community organizations, early care and education stakeholders, and the Department of Child Services (DCS).

The report template requires detailed programmatic insights, including quantitative and qualitative data on positive behavior supports, technical assistance sessions, training participation, provider assessments, and program impacts. Specific reporting elements include the number of technical assistance sessions and participants across Tier 1, Tier 2, and Tier 3 levels, training participation metrics, provider feedback, and narratives about program effectiveness. The document also solicits information about subcontractor performance monitoring to ensure consistent statewide service delivery and invites additional comments about the Expulsion Prevention Services program.
