About this file
This document is a Task Assignment Change Order Form (Change Order No. 1) issued by the Florida Department of Environmental Protection (FDEP) to Wood Environment & Infrastructure Solutions, Inc., a consulting firm based in Tampa, Florida. The change order modifies an existing indefinite delivery contract (FDEP Contract No. CN204, Task Assignment No. 33) for monitoring well services at Collier-Seminole State Park and Fakahatchee Strand Preserve State Park in Collier County, Florida. The contract is managed by FDEP's Division of Recreation and Parks, with Alyssa Skehan serving as the FDEP Contract Manager and Kristen Nowak as the Wood consultant representative.
The change order authorizes Wood to conduct professional elevation surveys for four additional water level monitoring wells and vegetation monitoring transects at Fakahatchee Strand Preserve State Park as required by Army Corps of Engineers permit. The scope of work includes ground surveying to obtain position and elevation data, including top-of-casing and ground surface elevations at wells, and ground surface elevations at rebar locations along transects. The net increase to the task amount is $3,048.34, consisting of $2,471.64 in fixed price labor costs and $576.70 in reimbursable expenses (mileage, lodging, and meals). The total task amount with all change orders is $28,197.81. The survey work is scheduled to commence March 18, 2021, and be completed by April 8, 2021, with the surveying to occur as an add-on to an already scheduled survey at Collier-Seminole State Park during the week of March 22, 2021. Payment is contingent upon FDEP approval of deliverables, which will include location and elevation data in Excel and PDF formats.

RE: DEP Agreement Number CN204 TA33 CO1 Project – Collier-Seminole Monitoring Well Installation Services – Change Order for the Addition of Fakahatchee Strand Monitoring Well and Transect Survey
Wood Project No.: 600639.2
Dear Mr. Kruse, Wood Environment & Infrastructure Solutions, Inc. (Wood) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this proposal and cost estimate to complete surveying services within Fakahatchee Strand State Preserve. These tasks will be performed in accordance with DEP Agreement Number CN204 and will be completed on a lump sum basis. The proposal includes our understanding of the SOW and price as outlined below.
SCOPE OF WORK
FDEP recently installed four shallow monitoring wells and four monitoring transects along Janes Scenic Drive within Fakahatchee Strand State Preserve in Collier County, Florida (Figure 1). FDEP requested Wood’s assistance with surveying the wells and transects.
Figure 1 - Fakahatchee Strand Janes Scenic Drive Monitoring Locations (approximate; not to scale)
Description: Wood will conduct a ground survey to obtain position and elevation data at each of the four installed wells and monitoring transects. Elevation data will include both top of the casing (TOC) and ground surface elevations at the wells, and ground surface elevations at each rebar location along the monitoring transects.
Each transect is comprised of four rebars, located 10 meters apart, for a total of 16 ground surface shots. Survey and mapping services and deliverables will be certified as meeting or exceeding, in quality and precision, the standards applicable for this work, as set forth in Chapter 472, F.S. Horizontal Datum will be referenced to the Florida State Plan Coordinate System, West Zone (0902), Units US Survey Feet, North American Datum of 1983 (NAD83/2011) Current Adjustment including the most recent NSRS adjustment. Vertical Datum will be referenced to the North American Vertical Datum of 1988 (NAVD 88), Units US Survey Feet, using the most recent geoid model to compute orthometric heights based on GPS-derived ellipsoid heights.
It is assumed that this task will be conducted during an already scheduled two-day survey that will be occurring the week of March 22, 2021, at Collier-Seminole State Park (CSSP). This change order for Fakahatchee Strand monitoring wells and transect survey includes one additional day of surveying and does not include mobilization costs outside of those that will already be occurring as part of the existing CSSP task assignment.
Deliverable: The deliverable will include a table summarizing the locations and elevations of each installed monitoring well and transect in excel and pdf format to be delivered to the Collier-Seminole SP park manager.
Performance Measure: Deliverables will be reviewed by the Department to confirm they are complete, accurate, and timely.

COVID-19
Notwithstanding anything mentioned in this proposal, if Wood’s work is delayed, disrupted, suspended, or otherwise impacted as a direct or indirect result of COVID-19 (coronavirus), including, but not limited to, by (1) disruptions to material and/or equipment supply; (2) illness of Wood’s or Subcontractors’ workforce and/or unavailability of labor; (3) government quarantines, closures, or other mandates, restrictions, and/or directives; (4) Wood's or Subcontractors’ restrictions and/or directives; and/or (5) fulfillment of Wood’s or Subcontractors’ contractual or legal health and safety obligations associated with COVID-19; then, Wood shall be entitled to a reasonable adjustment to the schedule and duration to account for such delays, disruptions, suspensions, and impacts.

Hi Kristen, Can you acknowledge that the subtask in the attached change order is actually subtask 6 (instead of 4 as shown). It is causing an issue with processing the invoice.
Thank you, Chris
