About this file
The document is a Performance Questionnaire (Attachment 0003) for Solicitation W519TC-25-R-0023, which provides detailed instructions for contractors to report past performance information. The questionnaire requires prime contractors, teammates performing 20% or more of contract effort, and major subcontractors to disclose any performance problems or contract terminations within a specified timeframe. Specifically, contractors must provide comprehensive details about performance issues, including contract numbers, descriptions of problems, corrective actions taken, mitigation plans, and points of contact who can verify the information. The government emphasizes that this is the only opportunity for contractors to address negative past performance, and they will not provide additional chances to explain or mitigate such performance concerns during the evaluation process.
