About this file

This is a Past Performance Questionnaire (PPQ) template designed for evaluating contractor performance on federal contracts awarded by the Veterans Health Administration (VHA) Network Contracting Office 20 (NCO 20) in Vancouver, Washington. The form is specifically created for solicitation CON 687-23-101, Repair Exterior Features located in Walla Walla, Washington, and is to be completed by previous clients or project evaluators who have worked with the offeror being considered for award.

The PPQ template requires contractors to provide comprehensive contract information including company details, point of contact information, contract specifics (title, number, type, location, award date, and original/final pricing), and a detailed project description explaining scope and relevance to the current solicitation. Section 2 requires an evaluator (client representative) to assess the contractor's past performance across seven critical categories: Quality, Schedule/Timeliness of Performance, Communication, Management/Personnel/Labor, Cost/Financial Management, Subcontract Management, and an overall summary assessment. Evaluators select from five adjectival ratings (Outstanding, Above Average, Satisfactory, Unsatisfactory, or Not Applicable) with specific definitions provided, and must include narrative comments for each category. The form also includes a yes/no question regarding whether the evaluator would hire or work with the firm again. Completed questionnaires should be returned to the offeror for inclusion in the proposal submission rather than submitted directly to NCO 20.
