1. Purpose

a. The primary purpose of this report is to identify and document the existing

Office of Information and Technology (OI&T) Systems covering buildings 1, 2, 4, 7, 33, 8, 9, 12, 13, 16, 17, 18, 20, 45, 48, 100, 113, 128, 200, 205, 215, 217, 221, 228, 230 and the Joliet CBPC located at the Edward Hines Veterans Affairs

Hospital Center in Hines, Illinois. Included within this report is a summary of the existing conditions, identification of deficiencies and recommendations for remediation to comply with the Electronic Health Records Management (EHRM) project.

2. Project Site Description

a. The Edward Hines Veterans Affairs Hospital (Hines VA) Medical Center provides primary, extended and specialty care services to Veterans. This is a multi-building medical center consisting of a Main Hospital Building, several research/lab buildings, office/administrative buildings, and various other support buildings. The Medical Center utilizes a fiber optic and copper backbone cable network supporting data and voice as well as other systems. Technology systems used in buildings within the Medical Center include structured cabling networks supporting data and voice systems, Bio-Medical systems, and Security systems. The systems have been installed at different times over time and may not be fully integrated, may not adhere to VA standards or may be obsolete.

b. Site Plan

1) The Hines VA Medical Center is a multi-building 174-acre campus consisting of twenty-four buildings located 12 miles west of the city of

Chicago.

2) Refer to the Medical Center Site Plan in Appendix 1 for building locations.

3) Equipment for various technology systems including structured cabling

(voice and data distribution), wireless LAN, DAS, access control, video surveillance, nurse call, and other IT, security, and bio-medical systems are in Telecommunications Rooms and/or facility spaces throughout the site.

c. The goal for this assessment is to document existing Office of Information and

Technology (OI&T) systems within the EHRM scope of work and systems that are not within the EHRM scope of work but will be affected by the EHRM project.

1) Systems to be identified and documented include:

a) Provide a high-level review of Active Network Electronics (data switches) supporting voice, data, and video surveillance systems.

b) Structured Cabling (Data and Voice) Cabling and Distribution

(1) Site Telecommunications systems

(i) Incoming Telecommunications Services

(ii) Outside Plant (OSP) and indoor backbone cabling systems

E. Existing Systems Review

1. Active Network Electronics (Data Switches)

a. Data network systems for the Medical Center are manufactured by Cisco. Core network switches are located in Building 1. Layer 2/3 switches are in

Telecommunications Rooms and facility-shared spaces throughout the Medical

Center. The VA is in the process of refreshing the existing network switch infrastructure and replacing aging equipment which are approaching end-of-life.

b. Existing layer 2/3 switches are a mix of Cisco 3750 and 3850 models, as part of the VA’s planned refresh program existing switches will be replaced with Cisco

9300 switches. Core switches located in Building 1 are not planned to be upgraded at the time of this report.

2. Structured Cabling (Data and Voice) Cable and Distribution

a. Incoming Services

1) AT&T is the incumbent service provider for the medical facility. Services extend from the public right of way from two different directions into the site at a common entry point. Services consist of T1 and T3 digital transmission connections and Multiprotocol label switching (MPLS) to support Wide Area Network (WAN) connectivity. Primary services

(T1,T3,a dn MPLS) terminate in the Campus Support Room (G325) located in Building 1. Secondary T1 services terminate in room F107 located in Building 1..

2) The VA is evaluating current incoming services provided to verify that services are adequate to support the VA’s functions.

b. Technology Backbone Pathways and Cables

1) Outside Plant (OSP). Refer to the VA EHRM project documents for existing conditions, demolition, and proposed new conduit infrastructure.

2) The Inter and Intra building backbone cable system consists of fiber optic cable, multi-pair copper cable, and coaxial cable. Refer to the VA

EHRM project documents for existing conditions, demolition, and proposed new backbone infrastructure.

a) Existing Fiber optic cable

(1) All fiber optic cabling that supports the data network extends from the Campus Support Center located in

Building 1. Fiber optic cabling consists primarily of 62.5-

31 May 2023 micron, multimode fiber optic cable terminated with ST-style connectors. The existing fiber type and connector do not comply with current VA OIT requirements.

b) New fiber optic cabling

(1) New fiber optic infrastructure will be installed as part of the VA EHRM upgrade project. Fibers will be installed in diverse pathways (primary and secondary).

c) Existing high-pair count copper backbone cables

(1) Existing copper cabling is distributed from the Primary

Branch Exchange (PBX) room located in building 1, underground, and/or within the building to each

Telecommunications Room. Copper cabling supports the existing PBX system. Cables terminate on wall-mounted 66-series termination blocks on both ends.

3) Refer to Appendix 2, Existing Medical Center Equipment Room

Inventory for the current fiber and copper backbone cable inventory.

c. Horizontal Structured Cabling System

1) Refer to the EHRM existing conditions and demolition package drawings for documentation of existing voice and data workstation locations.

2) Existing horizontal cable consists of a mix of category 5, category 5e, and category 6 cables and terminations that are not all compliant with current VA EHRM and VA OIT requirements.

3) New work area outlets (WAO) consist of two category 6A cables terminated on two category 6A modular jacks (RJ45), additional outlets will be provided as required for specific work areas.

d. Medical Center Wireless LAN (WLAN) Systems

1) Refer to the EHRM existing conditions and demolition drawings for the location of existing WAP (wireless access point) locations. Additionally, refer to Appendix 4 for Bio-Med equipment rooms with Veterans Guest

Internet Access (VGIA) terminations and network equipment.

2) The Medical Center currently distributes data over two separate wireless

LAN (WLAN) systems; one designated for VA official systems (employee

WLAN) and the VGIA for guest and patient internet access. The two

WLAN networks are isolated from each other and with separate access points (antennas), horizontal cables, horizontal terminations, and network switches.

3) The employee WLAN network is maintained by the Medical Center IT staff, the VGIA WLAN network is maintained by the BioMed Staff.

4) The existing Employee WLAN will be replaced as part of the EHRM upgrade project. Refer to the EHRM new build telecommunications floor plans for locations to new WAP data outlets for the new Employee

WLAN system.

5) The VA intends for the Wi-Fi networks, Employee WLAN and VGIA, to be consolidated onto one data cable infrastructure system using the IT data network. The combined WLAN network system will share antennas, cables, terminations (workstations and patch panels), and network switches but will be separated virtually in OIT network switches and will be maintained by the Medical Center IT Staff.

e. Telecommunications Spaces and Support

1) As part of the EHRM upgrade project all Telecommunications Spaces shall conform to the VA most current Infrastructure Standard for

Telecommunications Spaces (OIT) requirements for room quantities, room dimensions, power, and cooling. Currently, many spaces currently used for telecommunications equipment in the Medical Center’s building do not meet the minimum standards set forth by OIT standards. As part of the work to be performed under the EHRM upgrades existing telecom spaces will be renovated and new telecom spaces will be created with proper interior dimensions, power, cooling, and other VA requirements.

Refer to the EHRM existing conditions and demolition package drawings for locations of existing TR spaces.

2) Refer to the EHRM Infrastructure Upgrades Phase II BOD document attachment 6, Existing Room Data Sheets, for survey information on existing IT spaces in the Medical Center Buildings. Information includes room dimensions, lighting, power, HVAC support, grounding, and IT equipment. Existing room locations are shown in the project EHRM project demolition package drawings.

3) EHRM new build floor plans show locations of new telecommunications spaces including TR spaces that will be added to adhere to VA OIT requirements. All spaces will have dimensions of power, HVAC and grounding in accordance with the VA’s most current requirements.

f. PABX Telephone System

1) The Medical Center currently uses a centralized analog voice system distributed from a central NEC SV9500 Primary Branch Exchange (PBX) located in building 1.

2) The Medical Center plans on retiring the existing analog PBX and transitioning to Voice over IP (VoIP) system for the site that will utilize the data network fiber optic backbone system, network switches and horizontal cable.

g. Distributed Antenna System (DAS) System

1) There are currently two DAS systems in use in the Medical Center. A

Verizon DAS is used in buildings 1, 200 and part of 228. Sprint DAS system is used in part of building 228.

2) The VA is removing the Sprint DAS and installing a new T-Mobile DAS with an antenna located on building 200. The Verizon DAS will remain and will continue to be used.

3) The Sprint DAS will use the VA data fiber network for interbuilding distribution.

h. Emergency Responder Radio System (EMS)

1) The emergency responder radio system used by first responders is a sub-system of the Distributed Antenna System. As DAS systems are upgraded the EMS system will also be included.

3. Security

a. Electronic Access Control

1) Access control for the Medical Center is a mix of systems from different manufacturers including Softwarehouse, Keyscan, and Lenel.

Softwarehouse has the most equipment and distribution within the facility.

2) All new IT spaces shall be equipped with access control systems in compliance with the VA Physical Security and Resiliency Design Manual.

3) Refer to Appendix 2 Existing Medical Center Equipment Room Inventory for existing spaces that contain access control equipment. Access control equipment will be relocated into new Telecommunications

Rooms as part of the EHRM project. New cabling will be installed to each card reader.

b. Video Surveillance

1) The existing video surveillance system is an analog solution. There are no plans to upgrade the existing cameras throughout the facility at the time of this report. However, all new Telecommunications Rooms will receive the latest IP-based megapixel cameras and video management system in accordance with the VA Physical Security and Resiliency

Design Manual. Once the new video management system is installed camera upgrades around the facility can be supported under separately funded projects.

4. Bio-Medical

a. Nurse Call System

1) The Biomedical department is responsible for maintaining the nurse call system.

2) The existing Nurse Call system consists of multiple manufacturers and many different software versions. Many of these systems are at end of life or are no longer supported. Nurse stations utilize a centralized approach where patient stations, staff assist buttons, call buttons, etc.

3) Nurse call code blue notification is distributed via the nurse call systems and through the wireless network. The building overhead paging system does not support code calls. As wireless systems are upgraded the current plan is to distribute code calls via wireless.

4) There are plans to consolidate and standardize all nurse call systems to

Rauland Responder 5. The upgrade is scheduled to begin in 2022. No completion date has been provided.

5) Space for new nurse call components will be reserved in each new

Telecommunications Room. Nurse Call equipment to be installed by the

VA under a separately funded project.

b. Cable Television (CATV) Distribution and Patient Entertainment

1) The Biomedical department is responsible for maintaining the CATV and

Patient Entertainment system.

2) Direct TV is the Service Provider distributing content throughout the facility. The headend system is located in the Campus Support Room located in Building 1.

3) Coaxial cable is used to distribute signal. Taps and splitters are in existing Telecommunications spaces and will be relocated as part of the

EHRM upgrade project.

4) There are no plans to upgrade the coaxial-based system with an IPTV system utilizing category cabling.

a) The existing CATV systems in buildings 128 and 221 are integrated with nurse call systems in each building. The existing combined systems will remain active but space will be reserved in new Telecommunications Rooms for the VA to relocate the systems at a later date.

5) Patient Entertainment

a) Buildings 128 and 217 utilize computer tablets that connect to the VGIA WiFi system. Tablets are provided to the patients for educational materials and entertainment.

c. Patient Wandering System

1) The Biomedical department is responsible for maintaining the Patient wandering system.

2) Accutech is the manufacturer, and it is integrated with several existing nurse call systems.

3) The system utilizes a combination of wired and wireless devices, exciters, and keypads that track patients and access in designated areas of buildings 200 and 217. Alarm notification is provided at the nurse station.

4) Refer to “Bio-Medical Systems Equipment Rooms” matrix in attachment

4.

5) There are no plans to add, upgrade or replace the existing Wandering system at the time of this report.

6) Space will be reserved for Wandering systems in each new

Telecommunications Room.
