SUBJECT: Solicitation FA4620-20-R-0002, Multiple Award Construction Contract (MACC) Past Performance
Questionnaire
1. The 92d Contracting Squadron of the Air Mobility Command (AMC) is in the process of selecting a contractor for the
Multiple Award Construction Contract program for Fairchild AFB WA. The MACC is a multiple award, competitive design-build indefinite delivery/indefinite quantity (IDIQ) design/construction acquisition based upon a general statement of work. The work will consist of multiple disciplines in general construction and design categories to include maintenance, repair, and alteration of real property as well as new construction of base infrastructure, facilities, roads and airfields.
One of the considerations in proposal evaluation is the verification of the offeror’s past and present performance on contracts, which reflect the offeror’s ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had first-hand experience with an offeror, for the evaluation of the offeror’s performance on those contracts.
Our areas of interest in the offeror are summarized in the enclosed questionnaire. Please complete this questionnaire and return your written response no later the than proposal response deadline 27 April 2020 2:00 p.m. PT. This schedule will allow us sufficient time to analyze the data within the proposal evaluation period.
2. To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and emailed to heather.rogers.9@us.af.mil, or faxed to 509-247-8685 (Attention: Heather Rogers). Please call Heather Rogers at 509-247-4871 prior to transmission or if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.
3. When filled in this document is “Source Selection Sensitive Information” IAW FAR 3.104. Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
