About this file

This document is a Pre-Bid Site Visit Questions and Answers file for Project 636A8-24-001, a construction renovation of a dialysis facility at a VA hospital. The Q&A addresses 43 questions raised during the pre-bid site visit, covering technical clarifications, scope discrepancies, and administrative requirements.

Key technical clarifications include: 1-1/4" EMT conduit is correct for wall outlets with sheet revisions pending; linear diffuser air deflectors must be salvaged and handed over to the owner; a crane may use parking lots with VA CO/COR written approval; secondary grounding bus in TR-5W07 is existing to remain with riser revisions forthcoming; and missing specification sections 27 05 11 through 27 15 00 have been attached. The contractor must complete all abatement work, perform surface-mounted installation of temporary mixing valves and sinks that will be removed upon project completion, and provide concrete roof infill details for 10x10 and 14x14 openings. Acoustic ceiling substitution requests were denied. Scope clarifications specify that dashed lines on plans indicate demolition items, wireless access points and security systems are new work to be installed, and existing devices noted on drawings are to remain. Administrative requirements mandate: consolidation of construction phases II through V with 14-day buffer periods before phases I and II and again before phases V and VI for VA equipment removal; contractor responsibility for protecting and storing salvaged doors, plumbing fixtures, and other reusable items with photographic documentation; weekend/after-hours work restricted to noise-causing activities, staff work, and utility shutdowns affecting hospital operations; freight elevator A-5 designated for material transport with contractor-provided protective measures; and designation of separate personnel for Site Safety and Health Officer and Superintendent roles, though Superintendent may also serve as Quality Control Manager. The primary contractor must maintain direct site supervision with personnel physically present at all times.

PRE-BID SITE VISIT QUESTIONS

CONSTRUCTION – RENOVATE DIALYSIS

PROJECT #: 636A8-24-001

Question 1 – Sheet 1-T8 Detail 1 shows 1-1/4” EMT conduit between cable trays and outlets. Sheet 1-T12 Technology schedule equipment abbreviation SC-IO-W shows 1” routed above the nearest accessible ceiling. Please confirm that 1" EMT is acceptable for all wall outlets, or clarify direction.

Answer 1 – 1-1/4" conduit is correct, sheet 1-T12 will be revised in addendum.

Question 2 – Would like some clarification from design team per sheet notes: #1 on plan 1-H2.

"Linnear diffuser air deflectors are to be salvaged prior to demolition", with no other direction as what to do with them. And what exactly does prior to demo imply, they would come down in the demo phase. Please advise.

Answer 2 – "Hand over to owner" clarified in addendum.

Question 3 – Does the VA have a location for the crane if one was needed. The (2) exhaust fans on the roof are not that large so possibly can take up elevator, the stacks will be a bigger issue.

Answer 3 – A/E does not foresee the need for a crane, but if one is needed the parking lots can be used for it, but only if written approval is given by the VA CO/COR

Question 4 – Sheet 1-T11 detail #3 Technology bonding riser diagram. This detail appears to show the SC-GND-1 (SBB) in TR-5W07 as new. Sheet 1-T7 detail #1 existing telecom room detail shows that SBB is existing to remain. Which detail is correct?

Answer 4 – The secondary grounding bus in TR-5W07 is existing to remain. Grounding riser will be revised in addendum.

Question 5 – Sheet 1-T11 detail #3 Technology bonding riser diagram. Is the indicated 350 kcmil Telecommunications Bonding Backbone existing or new? If this is being provided under this project, is there an approximate distance from the SC-GND-1 in TR-5W07 to the existing main electrical ground bar (PBB) in the main electrical room and from the main electrical room PBB to the electrical entrance intersystem bonding termination point as shown in this detail 3?

Answer 5 – The secondary grounding bus in TR-5W07 is existing to remain. Grounding riser will be revised in addendum.

Question 6 – Specification sections 27 05 11 through 27 15 00 appears to be missing from the specifications manual.

Answer 6 – Specification sections 27 05 11 through 27 15 00 attached.

Question 7 – Sheet 1-T12 Technology equipment schedule seems to be missing rough in requirements for the nurse call equipment as shown on sheet 1-T1 Technology symbol list.

Answer 7 – Rough-in requirements for nurse call will be clarified in addendum.

Question 8 – Specification section 07 01 50.19 Preparation for Re-roofing. Please provide an existing roof plan and indicate what type of roofing is required.

Answer 8 – See Sheet H5 for general roof description. Roof plan detail to patch 14x14 opening included in Addendum.

Question 9 – Plumbing schedule on sheet 1-P12 don't match what is shown on new plumbing plans specifically sheet 1-P8 & 1-P9. Please update.

Answer 9 – Open ended question. A/E to address discrepancies in the addendum.

Question 10 – Substitution requests for the Acoustical Ceilings (09 51 00):

Answer 11 – Ductwork insulation detail updated in the addendum.

Question 12 – SOW Paragraph 2 requires “moderate weekend/after-hours work”. Please clarify the items that need to be accomplished after hours.

Answer 12 – Weekend/afterhours work will include any work activities causing noise and disturbances that impact Hospital operations; to include staff work in addition to patient care. All 3rd and 4th floor work, and any utility shutdowns that affects Hospital operations, unless coordinated differently with COR. Examples include but are not limited to core drilling, power shutdowns, water shutoffs, HVAC shutdowns

Question 13 – Regarding phasing, particularly phases II, III, IV, V and VI. Is there a reason these are separate phases? Logically portions of each phase could be completed simultaneously.

Answer 13 – Consolidate phases II through V as indicated. Phase VI Restoration will require VA to relocate services back to Zone 1 and must be a separate phase. Provide 14 days between phases I and II and again between V and VI to allow for the VA to remove equipment prior to work.

Question 14 – Regarding ICRA barrier requirements, SOW paragraph 4 requires hard barriers. The drawings note soft and hard barriers. Please clarify if the SOW or drawings take precedence.

Answer 14 – Both documents apply. SOW states "work includes but is not limited to temporary hard barriers."

Question 15 – Will the Project Manager be required to be on-site for the weekly meetings or can these be attended via conference call?

Answer 15 – Meeting through Microsoft teams is preferred for weekly meetings

Question 16 – Reference specification 01 35 26 paragraph h. Mishap Investigation and Reporting. Can the standard OHSA requirements to include the form 300 and 301 suffice for these requirements? If not, please include the forms required for reference.

Answer 16 – OSHA 300 and 301 forms or equivalent forms are acceptable for use.

Question 17 – Reference specification 01 35 26 paragraph 1.9 requires a SSHO on site with no other duties, specification 01 45 00 paragraph 3.4.B states the CQC System Manager can also have duties as the Superintendent. This would require two on site personnel at all times. Please confirm the contractor requirement is to have two separate on-site personnel (a SSHO and QC/Superintendent).

Answer 17 – Contractor will need a SSHO and a separate person to be the superintendent

Question 18 – Please confirm if elevators can be used to get materials to each floor and which elevators the contractor(s) will be allowed to use.

Answer 18 – freight elevator A-5 can be used, but other elevators can be granted approval depending on the material and time of work. Contractor shall provide protection to the elevators to prevent damage and will clean up any messes from spills or tracked in dust/debris.

Question 19 – Reference Drawing HA 001, Abatement Keynote A1 reference (6 doors).

There are only 5 callouts on the print. Is the callout at the north end of corridor 5C002 referencing the two doors at the end of the corridor?

Answer 19 – Yes, note references two doors. Refer to A2. Keynote DE22 identifies the assumed asbestos containing doors.

Question 20 – Reference Sheet 1-S1, there are multiple infills shown but there is not a corresponding drawing for the work to be done on the sixth-floor roof level. To complete the infills the concrete will need to be installed on the roof side and the roof cut and patched. Can you please provide the roof locations of the infills, roof details and roof manufacture warranty information? This appears to affect roof metal walkways, an area within the penthouse and possibly lighting protection removal and replacement. None of these items are addressed on the drawings.

Answer 20 – Concrete roof infill locations are 10x10 and 14x14 openings indicated on sheet H5. Information included in Addendum.

Question 21 – Reference drawing A-A1 General Notes 2 and 6 are vague in whether this applies only to new construction or whether the contractor is required to repair existing deficiencies. It would be impossible to address existing deficiencies without a thorough above ceiling inspection prior to bidding. Can we assume the fireproofing is only for the work removed and installed under this contract and that existing deficiencies will be treated as a differing site condition?

Answer 21 – Fireproofing is only for the work removed and installed under this contract.

Existing deficiencies outside the limits of construction for this project are outside the scope of this project.

Question 22 – Reference Sheet 1-A2 and 1-A3 Demolition Planned Key Notes DE6. All equipment and furniture to be salvaged and returned to owner. Where will the equipment and furniture need to be delivered? And is there any packing or protection needed to be provided?

Answer 22 – VA will remove equipment like chairs and dialysis machines, but contractor will remove medgas and items to be reinstalled and store those off site. Items are changing in the addendum

Answer 23 – No storage will be provided by the VA for the storage of all the doors in DE7.

Contractor will take pictures of all sides of the door right when removed. All doors will be protected and labeled with door number and location and stored by the contractor and returned in the same condition as when removed. Contractor will provide the pictures to the VA COR/CO prior to reinstallation. Any damage caused during the project will be repaired or replaced by the contractor at no cost to the VA.

Question 24 – Reference Sheet 1-A2 and 1-A3 Demolition Planned Key Notes DE9. Where will the plumbing fixture need to be delivered? And is there any packing or protection needed to be provided?

Answer 24 – Contractor is responsible to provide materials to properly protect and store the fixtures within the VA either in the basement or in one of the bathrooms of 5W.

Coordinate with the COR prior to removal. Contractor will take pictures of all sides of the fixture and mark the package materials with description and location and provide to the VA COR/CO prior to reinstallation.

Question 25 – Reference Sheet 1-A2 and 1-A3 Demolition Planned Key Notes DE13, DE14, DE16, and DE18. Will the VA provide an area for storage of materials to be reinstalled? If so, where will the storage area be?

Answer 25 – VA will move DE14 items. Contractor remove DE18 and coordinate with COR to give to EVS. DE13, DE16, Contractor will take pictures of all sides of the items right when removed. All items will be protected and labeled to include location and stored by the contractor and returned in the same condition as when removed. Contractor will provide the pictures to the VA COR/CO prior to reinstallation. Any damage caused during the project will be repaired or replaced by the contractor at no cost to the VA.

Question 26 – Reference Sheet 1-A2 and 1-A3, Demolition General Notes 9 states “Salvaged items scheduled for reuse must be cleaned, restored and/or refinished to like new condition, unless otherwise noted”. “Restored and/or refinished to like new condition” will be impossible to do. See the door photo to be salvaged for reuse below which is damaged through the existing door veneer:

Please clarify what should be done with each item to be salvaged for reuse.

Answer 26 – Restore to the state as to which it was removed. Any items damaged by the contractor will be repaired to similar state of the item prior to removal, or to be replaced at no cost to the VA. Contractor will take pictures of all sides of the items and mark the package materials with description and location and provide to the VA COR/CO prior to reinstallation.

Question 27 – Reference Sheet 1-A4, Floor Plan Keyed Notes FL10. Grinding the floor per this note would result in approximately 1-1/4” of structural concrete removed at floor drain.

It appears from the drawings the concrete is 4” thick which would remove 25% of the floor structure. This is not a question but a statement for record that this could expose reinforcement and may compromise the structure as the work is not reflected on the structural drawings.

Answer 27 – The structural drawings call for a 2 1/2" slab. The top 1 1/2" is a non-structural topping. The grinding and floor sloping cannot and should not disturb the structural floor.

Question 28 – Reference Sheet 1-A20, Equipment Schedule. There are acquisition codes of W and one blank that are not reflected on the VA Acquisition code legend. Please identify “W” code and provide a code for P8705 Dialysis Box, recessed, Single Valve.

Question 29 – Reference sheets 1-P1, 1-MG1 and 1-H1 View Key line types. The line type and tag key shows three line types; solid, dashed and long dashed. One is listed as “WIDE LINE” and one as “NARROW LINE” however they both appear the same. One states, “New Work by Contractor” and one says, “Existing to remain or work by others”. Please differentiate the line types.

Answer 29 – Line types clarified in addendum.

Question 30 – Referencing Sheets 1-P2, 1-P3, 1-P4, 1-P5, 1MG2, 1-H2 and 1-H3. Please confirm all plumbing, MEDGAS and HVAC represented by dashed lines are to be demolished by the contractor.

Answer 30 – Dashed lines indicated demolition as part of the scope of work as noted on the drawings.

Question 31 – Reference Sheet 1-P9. Keynotes 1 and 2 require installation of mixing valve (MV-2) and sink (SK-5). There are not any corresponding architectural requirements for these installations or information on how to install these. Is the contractor to surface mount and repair, install piping in the wall, remove/replace ceramic tile? Please clarify the requirements. Also, as these are temporary installations will these items be removed upon completion of the project or will they remain permanently in place?

Answer 31 – Intent is to be surface mounted install. Removed upon completion. AE will add notes to the Addendum
