About this file
This is The Joint Commission's Life Safety & Environment of Care Document List and Review Tool, revised September 27, 2023, to incorporate Veterans Health Administration (VHA) requirements effective October 1, 2023. The document serves as a compliance assessment checklist for hospitals and critical access hospitals undergoing Joint Commission accreditation surveys.
The tool establishes documentation requirements and performance standards across multiple environment of care and life safety domains. Key areas include: buildings' compliance with NFPA 101 (2012/2024) standards; fire response plans with staff role assignments; fire protection and suppression system testing frequencies (quarterly to annually); emergency power systems inspection and testing (weekly to 36-month intervals); medical gas and vacuum system maintenance; medical equipment risk management and maintenance; utility system inspection and testing; water management programs; hazardous materials inventory and procedures; and interim life safety measures for impaired systems. Specific testing frequencies are prescribed for fire pumps (monthly/weekly for diesel), sprinkler systems (annually/quarterly), fire extinguishers (monthly inspection, annual maintenance), emergency lighting (monthly functional tests), and generator load testing (36-month intervals at nameplate capacity). The document includes VHA-specific modifications highlighted in yellow, notes alternative equipment maintenance (AEM) program provisions with text scheduled for deletion January 1, 2024, and references applicable NFPA standards (72, 25, 101, 99, 96, 80, 105, 110, 111) throughout. The tool is designed for organizational survey readiness efforts and continuous compliance monitoring, with additional resources available through The Joint Commission's Physical Environment Portal.

Life Safety & Environment of Care Document List and Review Tool (WITH VA MODIFICATIONS) Effective: 7/1/2023
The following pages present documentation required by the Hospital and Critical Access Hospital Accreditation Programs Life Safety (LS), and Environment of Care (EC) standards. The Life Safety surveyor will begin review of these documents soon after arrival for the onsite survey.
Surveyors may request other EC and LS documents, as needed, throughout the survey.
This list also includes some elements of performance that do not require documentation but appear as reminders to both organizations and surveyors of these expectations.
Organizations may want to consider using this tool in their continuous compliance and survey readiness efforts.
Revisions to this document are identified by underlined text.
To allow organizations time to prepare, this version of the tool identifies text associated with noted standards that will be deleted effective January 2024 related to the use of Alternate Equipment Maintenance (AEM) strategies. Changes appear on pages 62-64 and are identified by blue highlighted text. A new section to the Survey Activity Guide titled “Guidance on Use of Alternate Maintenance Activities and/or Schedules” immediately follows this tool and outlines the requirements that organizations will need to comply with if AEM strategies are used.
Additional resources, including a Fire Drill Matrix, are available on The Joint Commission website, Physical Environment Portal which is accessible using the following link: https://www.jointcommission.org/resources/patient-safety-topics/the-hysical-environment/.
