About this file
This document is a Past Performance Evaluation Questionnaire for the Department of the Air Force's Multiple Award Construction Contract (MACC) solicitation FA462025R0001 at Fairchild Air Force Base. The questionnaire is designed to collect detailed performance feedback from previous project evaluators, focusing on a contractor's past performance across seven key evaluation areas: customer satisfaction, quality, timeliness, responsiveness, subcontracts and management, design, and change/cost control.
The solicitation seeks contractors for an indefinite delivery/indefinite quantity (IDIQ) design-build contract covering multiple construction disciplines, including maintenance, repair, alteration of real property, and new construction of base infrastructure, facilities, roads, and airfields. Evaluators are asked to rate the contractor's performance on a 1-5 scale for each evaluation area, with options to provide narrative explanations for low ratings. The completed questionnaire will become part of the official Source Selection records and will be critical in assessing a contractor's capability to perform on future projects at Fairchild AFB, Washington.
