WHEN FILLED IN THIS DOCUMENT IS SOURCE SELECTION SENSITIVE INFORMATION IAW FAR 3.104
The Mission and Installation Contracting Command at Joint Base Lewis-McChord are in the process of selecting a contractor for Installation Support Services, Directorate of Public Works, Yuma Proving Ground, Yuma, AZ.
The contractor shall be required to provide DPW services to include: preventive maintenance and repair of facilities, real property, and test sites; support of Unaccompanied Personnel Housing; grounds keeping;
engineering design; wastewater collection; sanitary landfill operation; heating and air conditioning maintenance and repair; water treatment and distribution; pest control; and test site support.
One of the considerations in proposal evaluation is the verification of the offeror’s past and present performance on contracts, which reflect the offeror’s ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had firsthand experience with an offeror, for the evaluation of the offeror’s performance on those contracts.
The past performance questionnaire will assess the offeror’s probability of meeting the Solicitation’s requirements, on an acceptable/unacceptable basis, as indicated by that offeror’s record of past performance.

This factor will be evaluated by identifying the awarded price (at the time of the award, and/or by the end date or current expenditure if an active contract). The Government expects that a magnitude of effort in dollars will provide insight and understanding of the offerors’ past performance with differing sizes of contracts. Management
Management is considered the General Overall approach that demonstrates the ability of the vendor to integrate with the Government team, build working relationships, manage to meet the mission, and deliver services when and where required (within the terms and conditions of the PWS and Contract Terms). This factor is subjective and based completely upon the submitter’s overall perceived, supportable, position regarding the experience they have had and the opinions they have formed. The Government seeks to evaluate this information to gain a broad determination of how the proposing vendor conducts operations within the broad categories listed below.

* Employs adequate workforce (Prime and Subcontracted labor), with workflow process and clear lines of communication for all functional areas.
* requires little oversight for simple tasks and can work through complex tasks as a team with the customer.
* uses acceptable methods of quality control and achieves the customer's level of required service frequently.
* implements and manages price control practices that are consistent with fair and reasonable pricing, as well as good stewardship of costs.
* responds in a timely manner in regards to the customer's need, delivery of projects within an acceptable time frame, meeting the needs of the customer per scheduled task, call out, task order or other contractual requirement.
