The information that you provide will be used in the award of an Army contract. Therefore, we ask that the information you provide to be complete and candid. If you do not have knowledge of or experience with the contractor that is referenced, then please forward this Questionnaire to the person who does. RATING DESCRIPTION
Exceptional The Offeror's recent or past performance significantly meets and exceeds all contract requirements to the Governments benefit and occasionally provides some additional and worthwhile services and/or benefits. (Note: This rating requires a narrative.)
Very Good The Offeror’s recent or past performance meets and exceeds most of the contract requirements. Some minor problems were effectively resolved by the Contractor’s corrective actions.
Satisfactory The Offeror's recent or past performance met the customer's expectations and contract requirements.
Marginal The Offeror's recent or past performance does not meet some of the contractual requirements. The Offeror’s corrective actions were minimal and ineffective in resolving serious problems. The Offeror required major Government resources to ensure achievement of the contract objectives. (Note: This rating requires a narrative.)
Unsatisfactory The Offeror's recent or past performance did not meet customer expectations and/or contract requirements. Corrective actions were ineffective and recovery from problems were not accomplished in a timely and cost effective manner. (Note:
