About this file
This is a Past Performance Questionnaire (PPQ) Form (Attachment 0006) used to evaluate contractor performance records for federal contract award decisions.
The form consists of four main parts: Part I collects general information about the contractor, agency point of contact, and agency details; Part II requires the offeror to complete contract identification information including contractor name, address, program title, contract number, NAICS code, prime contractor name, award date, completion date, and nature of the contractual effort. Part III contains the government/customer evaluation section, assessing contractor performance across six categories: Quality, Schedule, Cost Control, Management, Small Business Subcontracting, and Regulatory Compliance. Each category uses a five-point adjectival rating scale (Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory), with mandatory explanations required for Exceptional, Marginal, and Unsatisfactory ratings. Part IV provides return information fields for the respondent's signature, printed name, date, and contact information. The form explicitly states that information provided will be used in federal contract award decisions and must be as factual and accurate as possible, with examples and explanations provided on additional pages as necessary. The rating definitions distinguish between performance that exceeds requirements (Exceptional/Very Good), meets requirements (Satisfactory), partially meets requirements (Marginal), or fails to meet most requirements (Unsatisfactory).

Past Performance Questionnaire (PPQ) Form Attachment 0006
The following questions pertain to the Contractor's record of past (within the past four years) and current performance. The information that you provide will be used in the awarding of a federal contract.
Therefore, it is important that our information be as factual and accurate as possible. Please provide examples and/or explanations (use additional pages if necessary). The following adjectival ratings shall be used in your response.
Exceptional: Performance meets contractual requirements and exceeds many to the Government’s/Customer's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good: Performance meets contractual requirements and exceeds some to the Government’s/Customer's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory: Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal: Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractors proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory: Performance does not meet most contractual requirements, and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
