About this file

This is a Qualifying Project (QP) Form template (Attachment 0003) used to demonstrate relevant past performance experience for federal contract proposals. Offerors submit this form to establish their qualifications in specific technical domains by documenting previously completed contracts or agreements that align with the requirements outlined in a Performance Work Statement (PWS).

The form requires offerors to provide comprehensive project details including the contract/agreement number, proposed evaluation domain(s), period of performance, applicable NAICS code, and dollar value. For Large, Emerging Large, and Small Businesses, qualifying projects must have at least one year of performance but not exceed four years from the final solicitation date; Commercial-Sector Vendors must submit completed projects. Offerors must identify which technical capabilities from Section 3.0 of the PWS align with the project and provide a detailed work mapping description on page two demonstrating relevance to the proposed domain. The form requires calculation and documentation of specific performance metrics including vacancy rate, average time to fill rate, passthrough/subcontract rate, schedule rate (for Commercial-Sector Vendors), and completeness metrics with documented efficiencies. Projects are classified as either Level-of-Effort (LOE) or Outcome-Based QPs. All information must be certified by the contracting party point of contact and the offeror, with certifications attesting to accuracy and completeness. If the qualifying project was performed by a different legal entity than the offeror, the CAGE code must be provided along with a description of the relationship between entities. The government reserves the right to contact the provided point of contact to verify all submitted information.
