About this file

This is a Qualifying Project Form (Attachment 0004) used to document and validate a contractor's relevant past performance on a specific project that demonstrates capability for a federal contract opportunity.

The form requires offerors to provide comprehensive project documentation including the qualifying project number, specific contract/agreement number, proposed domain(s), dollar value, and applicable NAICS code with alignment explanation. Offerors must confirm they served as the prime contractor and provide the period of performance, government and/or private official points of contact with full contact information, and a description of how the project aligns with technical capabilities outlined in Section 3.0 of the Performance Work Statement. The form mandates calculation of two key performance metrics: vacancy rate for the last full year of performance (calculated as the number of vacant positions at year-end divided by total proposed positions, multiplied by 100, with allowances for official descopes) and average time to fill rate (derived from the mean average of individual vacancy fill times, calculated as the date of offer acceptance minus the date the vacancy opened). Additional required information includes the CAGE code of any affiliate or subsidiary performing work, confirmation of legal entity status, description of any relationship between the offeror and the entity providing the qualifying project, and a brief description of work demonstrating relevance to the proposed domain. The form concludes with an offeror certification statement attesting to the completeness and accuracy of all information provided, with notice that the government reserves the right to contact the provided point of contact for verification purposes.
