Bidders must submit the state-provided Price Form with all unit and extended prices fully loaded to include labor, profit, overhead, and all costs necessary to perform the required work, with all prices entered in dollars and cents rounded to the nearest cent. Additionally, bidders must submit a completed Bid Proposal Affidavit certifying compliance with Maryland state procurement laws, nondiscrimination policies, minority business enterprise regulations, debarment status, tax payment compliance, and conflict-of-interest certifications, along with one or more customer references demonstrating comparable HVAC repair and maintenance experience.
