Engineers in Training (EIT) are not acceptable and failure to provide personnel meeting the licensure requirement may result in a deficiency and make the SF 330 submission unsatisfactory for this criterion. It is encouraged for the SF 330 response to include an organizational chart showing the proposed team. If an organizational chart is provided, each discipline must have a lead or primary individual clearly identified for that discipline when multiple individuals or positions are proposed under a discipline. Additionally, ensure the SF 330 response includes the following data about each proposed team member: member’s name, firm name, office location, proposed team assignment, % of time to be spent on this team, education level/discipline (example: Bachelor of Science – Mechanical Engineering), State(s) of professional registration, number of years of professional experience, and # of years with the firm.

b. List all contracts and task orders awarded by the VA to the firm in the past 12 months

Past performance will be evaluated on contracts with Government agencies and private industry in terms of management, quality of work, performance schedules, and regulatory compliance. Provide CPARS assessments for each Government project submitted in Section F. For any project that does NOT have a CPARS assessment, a Past Performance Questionnaire (PPQ) is required to be submitted with the SF 330. The attached PPQ form may be used or previously completed PPQs may be submitted if the Point of Contact information is included and current. If the firm (Prime or Subcontractor) was not the Prime contractor on any project submitted in the SF 330, a PPQ must be submitted listing their specific role (key disciplines) as a subcontractor. The PPQ should be rated and signed by the evaluator. In the event that a firm does not receive a requested complete PPQ from an evaluator prior to the time for submission, the firm shall submit the partially completed PPQ with Section 1 and the Evaluator Information in Section 2 completed. Ensure a POC name, phone number, and email address are listed with the project title and contract number (and task order number, if applicable). The Government reserves the right to also evaluate past performance on any previously awarded federal contracts/task orders for A/E services via CPARS or with any other information available to determine general trends in the contractor’s performance.

PPQs and CPARS assessments will not be counted towards the page limitations for this submission. However, any narratives provided for past performance in Section H will be counted toward the page limit.

5. Selection Criterion 5 – Location in Geographical Area of the Project and

Knowledge of Locality

Clearly indicate the location(s) where the majority of the off-site work will take place.

Additionally, describe the team’s work in and knowledge of the project’s geographical area. The evaluation board will evaluate the location of the proposed team against the geographical area of the project and knowledge of geological features, climatic conditions, or local construction methods that are unusual or unique that may be applicable to this project.

PLEASE NOTE: The A/E Evaluation Board must be provided with complete and accurate information for ALL selection criteria above in order to rank interested and eligible firms.

All firms not invited to participate in discussions will be notified at this time.
