About this file
The document is a Past Performance Questionnaire (PPQ) form from the VA Office of Construction & Facilities Management for evaluating contractor performance for a Facility Condition Assessment (FCA) contract. The form is designed to capture detailed information about a contractor's previous project performance, including contract specifics, project scope, and comprehensive performance ratings across seven key evaluation areas: quality, schedule/timeliness, communication, management/personnel, cost/financial management, subcontract management, and overall performance. The evaluation uses an adjectival rating system ranging from Outstanding to Unsatisfactory, with evaluators rating the contractor's performance and indicating whether they would work with the firm again.
The associated federal contract opportunity is a Request for Architect-Engineer (A-E) Qualifications (SF 330s) for a Facility Condition Assessment of Veterans Integrated Service Network 16 (VISN 16). The project involves conducting independent facility condition assessments at nine VA Medical Centers and Veterans Benefits Administration Regional Offices located in Louisiana, Mississippi, Texas, and Arkansas. The current FCA database encompasses approximately 10,638,554 gross square feet across 285 buildings. The assessment will involve tours, interviews, inspections, and research to prepare a detailed report of identified corrective actions. This is a qualifications-based solicitation, and only selected firms will receive the full solicitation package after initial SF 330 evaluation.
