About this file

This file contains two affidavits required by the State of Florida for vendors contracting with the School Board of Polk County for Environmental, Commissioning, or 3rd Party Inspection services. The first affidavit, effective July 1, 2024, requires vendors to certify under penalty of perjury that they do not use coercion for labor or services, including prohibitions against using physical force, restraint, isolation, confiscation of identification documents, causing financial harm, fraud, or providing controlled substances for exploitation purposes. The second affidavit, effective January 1, 2024, requires vendors to certify that they are not owned by or do not have controlling interests from designated foreign countries of concern, including the People's Republic of China, the Russian Federation, the Islamic Republic of Iran, the Democratic People's Republic of Korea, the Republic of Cuba, the Venezuelan regime of Nicolas Maduro, or the Syrian Arab Republic, and that the vendor is not organized under the laws of or principally located in any of these countries.

Both affidavits must be completed with vendor information, authorized representative details, and notarization before submission. The affidavits serve as mandatory compliance documentation for any nongovernmental entity seeking to execute, renew, or extend a contract with a Florida governmental entity. Vendors must provide these signed affidavits under penalty of perjury, with notarization required to verify the signatory's identity and authority to execute the documents on behalf of the vendor organization.
