Solicitation P16PS00611 Amendment Three

In order to change the period of performance of the contract, correct specifications and drawing sheets, and answer questions received, the above numbered solicitation is hereby amended as follows:

1. In Section F, Deliveries or Performance, Clause 52.211-10, Commencement Prosecution and Completion of Work, the period of performance of the contract has been changed to read:

The Contractor shall be required to (a) commence work under this contract within 10 calendar days after the date the Contractor receives the notice to proceed, (b) prosecute the work diligently, and (c) (1) substantially complete the majority of the basement, first floor and east side of the second floor no later than June 19, 2017 with all punch-list work completed and final acceptance no later than September 1, 2017 and (2) substantially complete the stairways, elevator, west side of 2nd floor and a few other minor miscellaneous rooms by August 1, 2017 with all punch-list work completed and final acceptance no later than September 1, 2017. The time stated for completion shall include final cleanup of the premises.

2. The following general clarifications are made:

1) The contractor shall be aware of and properly remove and dispose of all hazardous materials included within the Work of the project.

a. Reference the attached report “LIMITED HAZARDOUS MATERIALS

INSPECTION - REHABILITATION PHASE I MAMMOTH HOTEL”

prepared by Tetra Tech, Inc. dated July 15, 2016 for identification of known hazardous materials.

b. Lead based paint shall not be required to be abated in their entirety from historic materials to remain. Contractor shall be aware that lead based paint is present within the work and shall take all appropriate precautions concerning its treatment throughout construction.

2) Notes within the documents indicating to “clean, restore, & refinish” or similar variations thereof as related to existing and historic wood components within the work are intended to undergo a stepped process consisting of distinct processes of cleaning, restoration (repair), and refinishing.

a. Cleaning, using methods tested and developed specific for the project based on requirements indicated in Interior Wood Cleaning & Restoration specification

b. Restoration, including repair of damaged or deteriorated wood components to match historic in-kind.

c. Refinishing consisting of staining/and or finish coatings to match in-kind historic.

3) Free standing casework within Room 160 is to be completed within the base-bid work of the project. Delete all references to such casework indicating “Not In Contract or N.I.C.” from all locations within the documents.

4) All casework within Room 171 is to be completed within the base-bid work of the project. Delete all references to such casework indicating “Not IN Contract or N.I.C” form all locations within the documents.

5) Replacement window units type B, C and D shall all be manufactured using wood species indicated in the contract documents for transparent finish application at interior and paint finish at exterior.

6) Existing windows 260.1, 260.2, 260.3, 260.4, and 260.4G shall be restored utilizing their historic material and finish in-kind.

7) Wood flooring shall be finished in accordance with the specifications in a semi-gloss sheen.

8) It is the intention of the work that wood flooring extends from hallway 173 through hallway 174 to the transition point at the stair and ramp (as graphically indicated on sheet AF2.1.

9) Contractors’ shall be aware that FSC certified wood is not the only certification agency that is allowed on the project. Reference technical specifications.

Woods must be provided that comply with only one of the multiple options of certification authorities indicated.

10) The project is not pursuing formal LEED certification at this time, however it is the intention of the Work that sustainable design practices will be followed that would allow a Certified level to be achieved if formal certification is desired.

3. The following changes have been made to the project specifications.

1) Section 010100; Add subsection 1.18 (and renumber all subsections following) with the following:

a. 1.18 OTHER REPORTS

b. A. LIMITED HAZARDOUS MATERIALS INSPECTION -

REHABILITATION PHASE I MAMMOTH HOTEL” prepared by Tetra Tech, Inc. dated July 15.

c. B. Electronic copies of all reports are available to plan holders upon request.

2) Section 013100 1.8.B; Delete subsections 1, 2 & 3 indicating permits and replace with the following:

a. “1. Stormwater Pollution Prevention Small Construction General Permit – Wyoming Department of Environmental Quality”

3) Section 260519 – 3.1 D; Change to read “Steel jacketed MC cable with an internal insulated ground wire. Aluminum jacketed MC cable is prohibited.”

4) Section 072100, Thermal Insulation, is attached.

4. The following changes have been made to the project drawings:

1) Sheet A0.4 is deleted and replaced with the attached sheet A0.4.

2) Sheet A0.2 Detail 16; Delete single instance of note 3 located to the north of doors 168.1 between the projection of grids 3 & 4 within the forecourt area.

a. Reference C1.3 for the phasing plan related to asphalt work within this area. It is the intention of the work that paving work to the outside edge of the concrete curb is completed in a later phase beyond repairs associated with the execution of the work.

3) Sheet C1.0; Delete sheet in its entirety and replace with attached sheet C1.0 revised 7/25/2016

4) Sheet C1.1; Delete sheet in its entirety and replace with attached sheet C1.1 revised 7/25/2016.

5) Sheet C1.2; Delete sheet in its entirety and replace with attached sheet C1.2 revised 7/25/2016.

6) Sheet C1.3; Delete sheet in its entirety and replace with attached sheet C1.3 revised 7/25/2016.

7) Sheet C1.8 Detail 16; Add note at location between new curb and gutter and Construction Phase 2 area reading “Contractor shall saw-cut existing asphalt to a minimum width of 2 feet as measured from the face/lip of new concrete curb.

Contractor shall provide fill material and place between lip of new curb and asphalt. Fill shall consist of recycled asphalt millings with a 4” minimum compacted depth to 95% of the maximum proctor density. Asphalt millings shall meet the following gradation (Screen Size/Percent Finer After Processing or Milling):

a. 37.5 mm (1.5 inch)/100%

b. 25 mm (1.0 inch)/95-100%

c. 19mm (3/4 inch)/ 84 – 100%

d. 12.5 mm (1/2 inch)/70 – 100%

e. 9.5mm (3/8 inch)/58 – 95%

f. 7.5mm (No.4)/38 – 75%

g. 2.36mm (No. 8)/25 – 60%

h. 1.18mm (No.16)/17 – 40%

i. 0.60mm (No. 30)/10 – 30%

j. 0.30mm (No. 50)/5 – 20%

k. 0.15mm (No. 100)/3 – 15%

l. 0.075mm (No. 200)/2 – 10%

8) Sheet A2.0 Detail 16; Move note 3 from Hallway 001 to the north wall of Hallway 002.

9) Sheet A2.0 Detail 16; Change all instances of note 11 within the detail to note 10.

10) Sheet A2.0 Detail 16; Change all instances of note 12 within the detail to note 11.

11) Sheet A2.0 Detail 16; Add note 9 south of the crawlspace foundation all in the area of grid A6 – Field coordinate exact location to access storage room above at north east corner.

12) Sheet A2.1 Keynotes Legend; Change keynote 11 to read “(N) BELLHOP

CASEWORK, REFERENCE DETAILS ASK.001 FOR ADDITIONAL

INFORMATION.”

a. ASK.001 is attached to this addendum and incorporated by reference.

13) Sheet A2.7 Detail 11; At north roof between grid 3 and 5, move note 7 references to squares indicated to the south. Chang existing note reference to number 12. Add note 12 to the legend reading, New roof scupper and roof drain.

See details 11 & 14/ASK.004.

14) Sheet A2.7 Keynote legend; Change note 7 to read “(N) ROOF SCUPPER AND

DOWNSPOUT – PROVIDE HEAT TAPE FULL LENGTH.”

15) Sheet AF2.1 Detail 16; Change note 7 located above room tag 174 to note 4.

16) Sheet A3.2 Detail 10; Replace all casework indicated within the detail with that indicated in Detail 16/ASK.002. All other work shall remain as indicated within the documents.

a. ASK.002 is attached to this addendum and incorporated by reference.

b. ASK.003 is attached to this addendum and incorporated by reference.

17) Sheet A6.2 Details 11, 12, 13, and 14; Replace casework indicated within the noted details with casework indicated in ASK.002 & ASK.003. All other work shall remain as indicated within the documents.

a. ASK.002 is attached to this addendum and incorporated by reference.

b. ASK.003 is attached to this addendum and incorporated by reference.

18) Sheet A6.5/Detail 5: Delete detail and replace with detail 5/ASK.005.

19) Sheet A9.5 “(E) Windows to Remain” & “Window Schedule”; Replace with schedules and notes indicated on ASK.006 attached to this addendum.

20) Sheet M1.0 Air Separator Schedule; Change air separator to air/dirt separator model number to #4904AD-125.

21) Sheet M1.0 Air Separator Schedule; Change pipe size to 4”.

22) Sheet M1.0 Condensing Unit Schedule; Change unit CU-1 to Daikin model

#RXYQ216TTJU.

23) Sheet M1.0 Condensing Unit Schedule; Add not reading “Provide unit with

BACnet interface panel to control fan coil & radiant panel units from one thermostat per location.”

24) Sheet P3.0 Detail 1; Change 3” CW line running between grid lines 3 & 6.5 to be in-line with 3” CW line routed from Mechanical room 003. Extend connections serving rooms above between grid line 3&4 to continuous waterline to the south.

a. It is the intention that the 3” CW, 3” HW, and 1-1/2” HWRC are to be continuous from Mechanical room 003, routed in one group, to point of connection with water service feeding wing to the north between grid line 6 & 7.

25) Sheet P3.0 Detail 1; Add ball valves to 3” CW, 3” HW, and 1-1/2” HWRC at intermediate grids B.5/6.5 where the point of connection is indicated to occur with existing system to the north.

26) Sheet E1.1 Panel Schedule 1B; Change breaker 18/20 to 30A.

27) Sheet E2.1Detail 1; Add keynote 16 directed at receptacle behind bar fed from panel 1B-18/20 to read “Provide L6-30R receptacle. Connect to UPS in storage

175. See detail 8/E5.0”.

28) Sheet FA1.0; Delete sheet and replace in its entirety with sheet FA1.0 attached to this addendum.

29) Sheet FA2.1; Delete sheet and replace in its entirety with sheet FA2.1 attached to this addendum.

30) Sheet FA3.1; Delete sheet and replace in its entirety with sheet FA3.1 attached

31) Sheet FA3.2; Delete sheet and replace in its entirety with sheet FA3.2 attached to this addendum.

32) Sheet FA4.0; Delete sheet and replace in its entirety with sheet FA4.0 attached to this addendum.

33) Sheet FA7.0; Delete sheet and replace in its entirety with sheet FA7.0 attached

5. The above changes to the specifications and drawings answer many of the technical questions received to date. A complete list of technical questions and their individual answers will be provided in the next amendment.

6. The following questions regarding the solicitation have been received and answers provided:

Q1. On page 140, Factor 3 of Section M it indicates that Factor 3 the Project Plan has a maximum of 6 pages allowed. But then it breaks Factor 3 down into 3 elements, each with their own page limitation that together exceed the total allowed for Factor 3. Which is correct maximum allowed for Factor 3? Is it 6 or is the 14 pages that you get from adding elements 3.1, 3.2 and 3.3?

A1: The 6 page maximum limitation noted for factor 3 is hereby eliminated.

The page limitations noted for each of Factor 3’s elements shall govern.

Thus the following page limitations govern:

Element 3.1: 6 page maximum Element 3.2: 2 page maximum

Element 3.3: 6 page maximum

Q2. In follow up to the above question regarding Factor 3 Project Plans (page 140). On this page it appears that Factor 3 is being considered part of the Technical Factors in Volume 1. However, in Section L (pages 126-128) it indicates that the two volumes will be broken out as (edited for summarization)Technical and Business. Item 9, Key Personnel, in the Business proposal appears to be part of the Factor 3 Project Plans. Clarify which Volume Elements 3.1 – 3.3 (pages 140-141) should be included.

A2: Item 9, Key Personnel as shown in solicitation section “L”, Instructions for the Submission of Offers and Other Information, Part “C” Proposal Format, Volume II Business and Price Proposal is hereby moved to solicitation section “M”, Part “C”, subpart 1, Factor 3, Element 3.3, first bullet item.

Q3. On the Experience Questionnaire, it indicates you can provide a maximum of Ten projects. However in Section M, Factor 1 (pg 138), it indicates a maximum of 5 projects completed in the past 5 years. Can we assume that section M overrides the Experience Questionnaire?

A3: Yes, the requirements of Section M shall be followed.

Q4. The Experience Questionnaire reads Provide a Maximum of 10 Projects. Section M, subsection C, Factor 1: Prime Contractor’s Prior Experience reads Offerors shall present a minimum of 3 and a maximum of 5 projects completed within the past 5 years. Only the first 5 projects presented shall be evaluated; additional projects will be disregarded. Which statement is accurate? Are you seeking a maximum of 5 or 10 projects for past experience?

A4: As answered above, the limitation in Section M apply.

Q5. The Past Performance Questionnaire – I would like to confirm that you want the owner’s reps to return these to us for inclusion into the proposal, rather than forwarding their responses directly to you.

A5: Correct; the owner’s reps should return past performance questionnaires to the Offeror who should include the Questionnaires in their proposal.

Q6. Amendment – is there any chance the amendment you previously mentioned will include a deadline extension?

A6: This has been done through Amendment Two.

Q7. Section L – Paragraph C – The requirements for Volume II include paragraph 7 Subcontracting Plan, which notes that a subcontracting plan is not required for businesses that are considered small under NAICS 236220.

However it also states that “if not required for that Offeror, the Offeror will receive a neutral rating.” Does this mean that a large business could receive a better evaluation under this section, purely by nature of submitting a Subcontracting Plan?

A7: Subcontracting plans are not a part of the trade-off process’s evaluation criteria as noted in solicitation section “M”. However, as noted in solicitation section L, Volume 2, item 7 “In order to be considered for award, all large businesses will be required to submit an acceptable Small, HUBZone Small, Small Disadvantaged, Women-owned, Veteran-Owned Small, Business Subcontracting Plan”.

Q8. Section L – Paragraph C – The requirements for Volume II include paragraph 8 Employment Reports on Veterans, which indicates a “certificate of compliance” shall be submitted with the proposal. Please clarify if an email noting that a VETS-4212 submission was completed by the offeror is acceptable or this requirement.

A8: Yes, an email from the Federal Government noting that a VETS-4212 submission was completed by the offeror is acceptable

Q9. Section L – Paragraph C – The requirements for Volume II include paragraph 9 Key Personnel, which also seems to be a requirement associated with Factor 3 in Volume I. Please clarify where this information is to be featured.

A9: See response above.

Q10. Section M – Paragraph C.1. - Technical Factors – Are offerors able to provide projects under Factor 1: Prime Contractor’s Prior Experience that are not 100% complete? If so, please provide a percentage of completion that is acceptable.

A10: Projects shall be substantially complete. Substantial completion shall be defined as issuance of a Letter of Substantial Completion by the project owner.

Q11. Section M – Paragraph C.1. - Technical Factors, Factor 1: Prime Contractor’s Prior Experience – The experience questionnaire provided in the Proposal Submission Package is two pages in length, but the page limitation associated with this section is 6 pages. This section notes that offeror’s may submit 3-5 projects in response to the evaluation factor. If the offeror submits 5 projects, that submission would encompass 10 pages. The way the evaluation factor is currently written offerors are forced to either provide minimal information for each project submitted or be limited to the minimum number of projects.

Would the contracting office consider expanding the page limitation to allow for more discussion of the projects?

A11: The 6 page limitation noted in solicitation Section M – Paragraph C.1. - Technical Factors, Factor 1: Prime Contractor’s Prior Experience is hereby increased to 10 pages.

Q12. Section M – Paragraph C.1. - Technical Factors, Factor 1: Prime Contractor’s Prior Experience – Previous solicitations promulgated by this office did not impose a page limitation on this section, which allowed the inclusion of a short narrative discussing previous experience. Would the contracting office consider expanding the page limitation to allow for a short narrative outlining experience?

A12: Refer to Government’s response to question 11 above.

Q13. The solicitation indicates that electronic submission of the proposal is acceptable. Does that include both the technical and pricing proposals? Do you accept an electronic copy of the bid security (Bid Bond)?

A13: Offerors shall e-mail all portions of their proposal including a pdf of their bid bond. Offerors shall mail the original bid bond so as to insure it is received by the Contracting Officer no later than 1 week after the date proposals are due. No portion of an Offeror’s proposal, other than the original bid bond, shall be mailed.

Q14. I've attached a substitution request for ……..

A14: Substitution requests are not pre-approved but instead prime offeror's may include "or equal" requests with their proposal. Please refer to clause 52.211-6 – Brand Name or Equal shown in section “L” of the solicitation and the Proposed Equals form included in the RFP Propposal Submission Package.

7. Solicitation Section “L” – Instructions for the Submission of Offers and Other Information, Paragraph “B” – Submission Instructions: Change to read:

“Offerors shall e-mail all portions of their proposal including a pdf of their bid bond so that it arrives no later than 2:00 PM Mountain Time August 16, 2016.

Proposals shall not be mailed or couriered.

Offerors shall mail or courier the original bid bond so as to insure it is received by the Contracting Officer no later than 1 week after the date proposals are due.
