FOVA Rehabilitation - Questions Amendment 4
1) Wood Flooring spec 96400 – part 2.1.A (1) notes that the new wood flooring is to ‘match species, grade, cut, thickness and face width of existing wood flooring’. I can’t find where the species is noted in the documents.
Answer: The hardwood flooring in Building 987 is maple and is 3 - 1/4" wide by 7/8" thick. In some locations the thickness is down to 3/4" because of wear and/or sanding.
2) Based on the existing survey asbestos is shown to be present in black mastic associated with floor tile within the 1st floor of Wing 3, are we to assume that mastic in all other areas is non-asbestos?
There are two types of 9”x9” floor tile present at this site, these are not listed on the survey. There is a clarification stating that sample W-3 S-4 is brown 9x9 tile, however, the sample results indicate the sample is white powder and brown fibers. It does not appear that mastic under this material was sampled.
Can White Shield provide further clarification on the floor tile samples collected and the corresponding results? It would be typical for an Asbestos Survey Report to contain a map showing sample locations and homogeneous areas.
Answer: Final_ACM_ HazMat Survey Summary_FOVA_041414KMA and FOVA 174962 ACM Quantity Estimate are being provided for information on asbestos containing materials.
3) There appears to be omissions in the survey, for example, boiler insulation and refractory materials, cove base mastic, as well as, areas that were inaccessible when this survey was performed. Will the Owner be providing an additional survey including all suspect building materials as required by SWCAA?
Answer: Final_ACM_ HazMat Survey Summary_FOVA_041414KMA and FOVA 174962 ACM Quantity Estimate are being provided for information on asbestos containing materials.
4) Bidding the handrails and I just thought that the rails could be painted with lead paint being that they were done so long ago.
Answer: I believe you are asking if the handrails are painted with lead based paint or not. The information the government has available was provided in the February 19 Solicitation 1 in the Hazmat Survey Compiled Documents and the FOVA Lead Base Paint Document. Include your assumptions with the proposal.
5) Amendment 2 Question 1 was in regards to window treatments. The response was that the requirements are in the plans and specs. We have reviewed the drawings and the specs and cannot find any details for window coverings such as blinds etc. Please advise.
Answer: All requirements are in the specification documents. The furnishing requirements are in Division 12.
6). Bid documents call for the tower to have a solid core, or angle iron leg. Can this be changed to allow for tubular legs? This will save on costs and still has to meet all engineering standards.
Answer: Specification Section 338113, 2.2, A. lists two options for the tower. Please also see question No.
8 below.
7) Antennas and cable. We need tower heights identified, and cable types for new and future installations to make sure the tower is properly engineered.
Answer: The answers to these questions and other load design issues are clearly identified in Specification 338113 – Radio Tower.
8) Are they accepting pre-bid substitution requests for this project? If so who shall they be directed to? Typically it’s the arch/eng. but some cases, like the parks, states, counties, etc., it goes to others.
Answer: Substitutions are accepted during the submittal process once the contract is awarded. Or equals are covered under FAR 52.236-5 Material and Workmanship.
9) Please provide the specific insurance requirements, coverage forms, and limits of liability for the project as they are cited but not given in the solicitation. Please also provide the specific parameters or minimum requirements for builders risk, limits of liability, and coverage forms.
Answer: Below is the FAR standard. NPS does not require a higher limit.
28.307-2 Liability
(a) Workers’ compensation and employer’s liability. Contractors are required to comply with applicable Federal and State workers’ compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer’s liability section of the insurance policy, except when contract operations are so commingled with a contractor’s commercial operations that it would not be practical to require this coverage. Employer’s liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers’ compensation to be written by private carriers. (See 28.305(c) for treatment of contracts subject to the Defense Base Act.)
(b) General liability.
(1) The contracting officer shall require bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence.
(2) Property damage liability insurance shall be required only in special circumstances as determined by the agency.
(c) Automobile liability. The contracting officer shall require automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims.

10) Page 107 of the original RFP for this procurement references a Past Performance Questionnaire, as does Question 2 of the Questions and Responses released with Amendment 2. However, the revised RFP provided with Amendment 1, appears to have removed this requirement and no questionnaire form has been provided in the Proposal Submission Package. Please clarify whether a Past Performance Questionnaire is required or if the Revised RFP included under Amendment 1 is correct.
Answer: Amendment 1 RFP is correct. However if the bidder wants to fill out the Past Performance Questionnaire, they may do so.
11) Please confirm that the offeror may use Subcontractor Experience to fulfill the Past Performance Requirement described under Factor 2 of Section L.
Answer: Yes they may
12) Page 107 of the original RFP also requests a narrative discussion of the offeror’s safety program, approach to safety for this project, and a resume for the Site Safety and Health Officer. The revised RFP document provided under Amendment 1 appears to remove this requirement. Please clarify whether this narrative is required.
Answer: This requirement is removed as per Amendment 1

Section L, Factor 4 lists the Project Manager, QC Manager, and Superintendent as Key Personnel for whom we are required to submit resumes for. Please clarify whether the offeror is required to submit resumes for all key personnel listed under Section H, or only those listed under Section L. Furthermore, will the offeror be evaluated more favorably if all personnel listed under Section H are representated under Factor 4? Finally, is the offeror allowed to submit additional resumes associated with other personnel that we feel will be key properly performing this project?
Answer: Please use that is listed in Section L, Factor 4. Section H will be updated when to the Awardee’s information at time of award.
15) Please confirm that, while there are limitations to the number of projects that can be submitted in response to Section L, Factor 2, the Volume I – Technical Proposal does not have any page limits. More specifically, please confirm that the narratives required under Section L, Factors 1, 3, and 4 do not have any page limits.
Answer: There is no page limit for these factors.
16) Please clarify what the evaluation panel would like to see regarding the qualifications of Key Subcontractors under Section L, Factor 4 – neither Section L or M describe the requirement as it pertains to Subcontractors, with the exception of mentioning it in the title of the Factor.
