About this file
This is a Non-Debarment or Suspension Certification form required by the Kenton County Airport Board for the 2026 Fall Protection Systems Installation project at Cincinnati/Northern Kentucky International Airport. The certification document requires bidders to affirm under penalty of perjury that their principals are not currently debarred, suspended, or excluded from federal covered transactions; have not been convicted of fraud or criminal offenses related to public contracts within the preceding three years; are not presently indicted or charged with enumerated offenses; and have not had public transactions terminated for cause or default within the three-year period preceding the proposal. Bidders must execute the certification with notarization by a notary public, with the signature page providing spaces for the contractor name, authorized representative signature, date, and notarial authentication including the notary's signature and commission expiration date.
Any exceptions to the certification statements must be disclosed in writing, identifying the applicable principal, initiating agency, and dates of action. The form explicitly warns that providing false information may result in criminal prosecution or administrative sanctions, and bidders unable to certify to all statements must contact the Kenton County Airport Board sponsor prior to contract award to discuss exceptions. While exceptions do not automatically disqualify a bidder, they will be considered in evaluating contractor responsibility and eligibility for award.
