About this file
This is a Non-Debarment or Suspension Certification form issued by the Kenton County Airport Board for the 2026 Fall Protection Systems Installation project at Cincinnati/Northern Kentucky International Airport. The certification requires bidders to attest under penalty of perjury that they and their principals are not presently debarred, suspended, proposed for debarment, or declared ineligible from covered transactions by any federal department or agency. Bidders must also certify that within the three-year period preceding the proposal, they have not been convicted of or had civil judgments rendered against them for fraud, criminal offenses in connection with public transactions, violations of federal or state antitrust statutes, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receipt of stolen property. Additionally, bidders must confirm they are not presently indicted for or otherwise criminally or civilly charged with commission of enumerated offenses and have not had one or more public transactions terminated for cause or default within the three-year period preceding the proposal.
Bidders unable to certify to any of the statements in the certification must list exceptions below, noting to whom the exception applies, the initiating agency, and the dates of the action. The form specifies that exceptions will not necessarily result in denial of award but will be considered in determining contractor responsibility, and any bidder noting exceptions must contact the Sponsor prior to award to discuss the exception. The certification must be subscribed and sworn before a notary public and signed by the bidding contractor with printed name and title, with false information potentially resulting in criminal prosecution or administrative sanction under the laws of the Commonwealth of Kentucky.
