Accomplish the following requirements, in addition to the specific requirements of the Job Order, for work being performed on naval ships and craft located at NNSY.
When accomplishing the requirements of 009-09 of 2.1, include the following additions:
NNSY will provide crane and rigging services to support contractor work. Crane and rigging services will be provided during normal working hours for Contractor blasting operations (0720 hours through 2400 hours and for all other Contractor work 0720 hours through 1600 hours) Monday through
Friday, excluding Holidays. Requests for crane and rigging services outside of normal business hours will be evaluated on a case by case basis. Weekend lifts should not be planned as routine. Weekend lifts will only be supported if schedule requires. Lift notifications must be made 24 hours prior to need by the contractor representative. Weekend (if schedule requires) and Monday crane lifts shall be requested by Tuesday of the week preceding the weekend lift due to overtime considerations. Lifts will be prioritized and coordinated by the project team as needed for completion of the total project. NNSY will attempt to perform the lift within the requested shift (8 hour time frame) based on the logistics of the total number of pieces of equipment or material requested to be lifted within the shift. All contractor furnished equipment must have the weight empty and weight full clearly marked on it with sketches for lifting attachments and certifications of attachment points. Sketches must be provided to NNSY at least
30 days in advance of lifts. Submittal of sketches and lifting of equipment shall be done in accordance with 2.4. It is recommended the contractor use at least two fuel buffaloes to provide one in dock full of fuel and one on the pier to minimize fueling delays. Drip pans must be fabricated, installed and maintained beneath each buffalo to prevent spills.
Provide all Technical Work Documents (TWD) to NNSY Codes
265 and 246 via the SUPERVISOR for review and approval at least ten working days prior to start of work. Contractor will submit a Contractor TWD to Code 246 via the SUPERVISOR and obtain a Work Authorization Form prior to start of work. In an effort to expedite these reviews, the Contractor will provide these documents on CD ROM to allow for simultaneous reviews. Contractor shall provide 3 discs to the Work Integration Zone Manager (WIZM) via the SUPERVISOR.

CRANE SAFETY REQUIREMENTS. Cranes rented or owned by the contractor shall meet the requirements of 2.2, 2.3, all state and local regulations, and predetermined ground loading capacities. The contracting official will schedule an initial crane inspection via the SUPERVISOR with NNSY
Code 720C to meet in a parking lot off of Effingham and Portsmouth Boulevard adjacent to the NNSY Pass Office or Building 1502 for crane validation prior to entering NNSY. NNSY Code 720C is a crane inspector from the Lifting and Handling
Department of the Crane Division Code 700.
Submit one legible copy, in hard copy or approved transferrable media, of a report during the initial inspection listing documentation supporting certification requirements, the names of each crane operator including qualifications on specific crane being utilized, and crane ground loading requirements to the SUPERVISOR, Code 603.
Submit one legible copy, in hard copy or approved transferrable media, of a report listing crane ground loading data calculated using crane weight, load(s) weight to be lifted, boom extension and angle, lift radius, and sufficient strength and size of matting materials to determine maximum load on outriggers to the SUPERVISOR, Code 240, 72 hours prior to the initial inspection.
Obtain maximum ground loads for the crane working area in NNSY from the SUPERVISOR, Code 240.
Submit report required in 3.1.3.2 to the SUPERVISOR not later than 24 hours before the end of the last preceding day shift, when initial crane inspections are scheduled after normal day shift working hours, on a weekend, or on a federal holiday.
The lifting of personnel with a live hoist/friction crane is not allowed.
Wedged socket and connections shall not be used to handle ammunition, hazardous or explosive material or for lifting personnel.
Blockage of crane rails (within yellow travel zones) must be coordinated with NNSY, Code 742, via the SUPERVISOR.
No weight handling equipment will travel, set-up, or operate within 12 feet of the drydock walls at Numbers 3 and 4 dry docks. This includes mobile cranes, truck cranes, forklifts, JLGs, etc. No weight handling equipment will operate between the drydock wall and the yellow stripping for the inboard dock crane rail.
Contractors using subcontractors to perform any work involving weight handling equipment will ensure their subcontractors' work

AERIAL PLATFORMS.
practices follow and adhere to these guidelines.
Contractor must provide a 24 hour notice for NNSY supplied crane lifts.
No contractor rented or owned cranes shall be operated on or around submarines.
Accomplish a verification inspection of each aerial platform rented or owned by the contractor prior to utilization on production work. Inspection can be performed at the job site. Record inspection results for each aerial platform on NNSY form (Pre-entry Aerial Inspection Form). Submit a completed and signed copy of each form to NNSY Code 970.2, via the SUPERVISOR, Code 603, by fax or delivery of hard copy. Upon submission, production work may proceed. The form will serve as notification to NNSY of the presence and location of the equipment and afford NNSY an opportunity to visually inspect and confirm the information received to ensure and maintain the safety and protection for its employees and Naval facilities. The contractor/vendor remains responsible for the safety, reliability, certifications and operation of the equipment, all operators, and other personnel affected.
Copies of a form for providing the above information will be provided upon request by the SUPERVISOR, Code 603. An asterisk on the form denotes items operator(s) shall inspect at job location after set-up of unit. Maintain a copy of the document with the unit. For aerial platforms delivered and/or requiring replacing after 1600, perform the verification inspection and submit the completed and signed form no later than 0800 the following day. Any aerial platforms requiring lifting by NNSY at the job site will require prior approval by NNSY Code 700. Aerial platforms equipped with lifting pads and an approved manufacturer's procedure/sketch for lifting in accordance with the requirements of 2.4 will be approved for lifting without any production delay. Aerial platforms not equipped with lifting pads with an approved manufacturer's production/sketch for lifting will require a minimum of one working day for NNSY Code 700 review/approval.
No aerial platforms will travel, set-up, or operate within 12 feet of the drydock walls at Numbers 3 and 4 dry docks. Do not operate between the drydock wall and the yellow stripping for the inboard dock crane rail.
Operators shall have available at the job site a copy of their certification of training for the specific unit being utilized. Provide for review upon request.

DIVING OPERATIONS.
COORDINATION OF INCOMPATIBLE OPERATIONS.
boom will not be allowed into NNSY.
Perform daily pre-use inspections by the operator(s) prior to assuming operating responsibility.
Report verbally each accident immediately (i.e., personnel injury, material or equipment damage, collision or unplanned contact with objects) to NNSY via the SUPERVISOR as soon as management becomes aware;
provide a formal written report of the event to the NNSY via the SUPERVISOR within 24 hours of each accident, followed with a summary of circumstances, explanation of cause, and corrective actions taken. Secure the accident site, protect evidence, and withhold further aerial platform operations until the cause is determined and corrective actions are implemented and approved by NNSY via the
SUPERVISOR.
Ensure subcontractors follow and adhere to these guidelines.
NNSY inspection team will be performing random monitoring of aerial platforms while in use. Any unsafe operations/conditions observed/found will be questioned and may result in stoppage of work operations.
Notify Port Operations within thirty minutes of divers entering the water. Submit one legible copy of the Port Operation notification to NNSY via the SUPERVISOR.
Ensure that the ship's "Code ALPHA" flag is up each day prior to and during all diving operations.
Prior to start of work each day, coordinate all potentially incompatible work operations with the NNSY's WIZM and Ships Force via the SUPERVISOR. DO NOT PROCEED WITH ANY WORK UNTIL INCOMPATIBILITIES ARE
RESOLVED. Operations that may be incompatible and may require coordination of scheduling prior to start up are: ventilation of toxic or flammable/combustible;
spray painting and paint application; use of solvents; hot work; tank work;
fueling/defueling operations, C02 Fire Extinguisher System Work; asbestos work;
mercury work; energizing circuits and starting machine; working aloft; removing floor plates or gratings; diving operations; abrasive blasting; compressed air
(blow down); and, CHT (Sewage) work. Note: The above list is presented as a guide and is not all inclusive. The contractor is expected to be alert to the possibility of potentially incompatible operations, and resolve incompatibilities to ensure hazards are adequately controlled.

TRAILERS OR CONEX BOXES USED AS A TEMPORARY JOB-SITE OFFICE OR
MANNED OFFICE SPACE.
Requirements for Contractors using CONEX boxes as job-site work and/or office spaces shall be in accordance with paragraph 4.e of reference 2.7 and the following:
The Contractor job-site CONEX boxes will be inspected by a Fire Department representative and an Electrical Safety Over-Site Committee designated member prior to authorizing electrical power connection by Shop 99.
Electrical power will be provided in either 440 VAC or 115 VAC only. The
Contractor is responsible to provide a breaker panel with GFCI breakers or GFCI circuitry, a disconnect switch on the exterior of the container and a step-down transformer for any other voltage requirements/connections. Inspections and connections will be requested by the SUPERVISOR via the WIZM.
The Contractor job-site CONEX boxes will have a Primary and Alternate Point-of-Contact and telephone number listed on the exterior of the container in 2 inch lettering.
The Contractor job-site CONEX boxes will be landed and positioned in accordance with the approved project pier/waterfront drawing.
The Contractor job-site CONEX boxes containing classified or NOFORN material will have provisions to safe-guard these material and be locked when unmanned.
Contractor trailer temporary services (potable water, sewage disposal, and electricity) shall be requested through the SUPERVISOR.
NNSY will provide temporary services via pier-side risers only. The contractor will provide, install, connect and maintain all temporary services between the
NNSY pier-side riser/location source to its trailer and/or CONEX box. Connect and remove services when directed by the SUPERVISOR.
Trailers shall meet Federal, State, and Local building and environmental requirements, (i.e., lighting, ventilation, heating/cooling, safety, Fire Safety, etc.).
All manned (full occupancy)office trailers or CONEX boxes located on the ship, in the dry-dock and on the pier will be equipped with an automatic or manual internal sprinkler system designed to provide 0.1 GPM per square foot of floor area and an audible alarm that will sound when the sprinkler system is activated. In addition, the fire department recommends installing two fully charged multi-purpose, dry-chemical fire extinguishers rated as 10A, 60-BC, or 20-ABC in a convenient location in each trailer or container.

Accomplish the requirements of 009-60 of 2.1 with the following addition:
Prior to loading materials, tools and equipment aboard ship, the
Contractor shall install identification tags consisting of Contractors name, description and destination. Tags must endure repair process.
For temporary services requested by the Contractor, the Contractor is responsible for possessing the necessary materials to connect hookups at locations pierside, provided by NNSY. The Contractor is responsible to run services from NNSY provided location to their work site, shipboard or pierside.
The connections used by the Contractor must be capable of adapting to the following shipyard temporary service connections:
SMOKING POLICY
TEMPORARY COVERS.
Prepare the schedule and associated reports required by 009-60 of
2.1 in an electronic spreadsheet format that will transfer into Microsoft Project software. Submit the electronic files in conjunction with the "required by" dates in 009-60 of 2.1.
shall be installed in enclosures and shall be audible outside the container.
The Contractor will ensure any trailer or CONEX box located on the pier or ship is secured or weighted for heavy weather at the time of installation in accordance with 2.6.
Smoking requirements within NNSY are detailed in 2.9.
Smoking is only authorized in areas identified as Designated Smoking Areas (DSA).
All DSAs will be identified by a "Designated Smoking Area" sign as shown in
Enclosure One of 2.9. A map of DSAs within NNSY is shown on Enclosure 3 of 2.9.
Contractor personnel violating the requirements of 2.9 will be subject to removal from NNSY premises and other appropriate remedies under the contractual agreement.
Template, fabricate and install temporary top hats on each internal tank manhole.
Fabricate top hats from 1/4 inch thick aluminum.
Top hats shall not exceed 12 inches in height.
Install each top hat using new 3/16 inch thick gaskets conforming to MIL-C-6183, Class I, grade firm, synthetic rubber and cork composition. Install top hats using temporary fasteners.

Temporary services will be provided from pierside riser connections;
The Contractor will not be allowed to use electric air compressors in NNSY. Only diesel air compressors are allowed. Drip pans must be fabricated, installed, and maintained beneath each diesel air compressor to prevent spills.
All temporary services and support systems shall be installed and maintained to meet the requirements of 2.5, including a weekly inspection. All temporary services shall be tagged at the source, destination and bulkhead penetrations in accordance with the requirements of 2.5 using NNSY Temporary
Service Tags and serialization from Project Temporary Service Manager (PTSM) Log
Book. Tags will state use, source, destination and Contractor's name. Quick disconnects shall be used when necessary within 3 feet of a safety bulkhead if the services penetrate the bulkhead in accordance with the requirements of 2.5.
All tags or waivers shall be requested through the WIZM and/or the PTSM via the
SUPERVISOR.
NNSY will only supply steam to the 2 inch diameter line/connection (pierside connections at the riser).
NNSY will only supply Electric 440V 3 Phase (pierside connections at the turtle back or substation).
NNSY will only supply potable water 2 1/2 inch diameter
(pierside connections at the riser).
NNSY will only supply L.P. Air 2 inch diameter (pierside connections at the riser).
Contractor SHALL NOT use canvas hose within the boundaries of the ship's hull.
Surveillances of temporary services will be performed by the WIZM, and/or Code 132 and/or the PTSM at least once per week. When services are found that are either not routed properly or not tagged in accordance with the requirements of 2.5, they will be removed after a 24 hour notice has been provided to the owner of that service.
In addition to the requirements of 2.5 for installation and securing of temporary services, Contractor shall meet the requirements of
2.12 for locations inside ship's hull and/or critical areas of ship. Technical requirements for coated wire referenced in 2.12 are as follows and wire is available from known sources provided in 4.2:

Contractor will remove all trade litter when Contractor breaks for lunch and at the end of each shift. The Contractor's assigned work areas will remain in a clean and orderly condition. Contractor will remove all industrial material at completion of work operation. Industrial material and other Contractor equipment will remain in designated pierside and drydock areas.
Unless otherwise specified in Work Items, scaffolding shall be provided/installed/maintained/removed by the Contractor for Contractor performed
Work Items in this Work Package.
Contractor provided scaffolding which will be utilized by NNSY employees shall be installed, maintained and removed in accordance with the requirements of
2.10 and 2.11.
Jobs designated by the WIZM as "Critical In Nature" will be required to have a Pre-Job Brief. The Brief will be hosted by the WIZM and he/she will determine the required attendees (NNSY/Contractor, etc.).
DESTRUCTIVE WEATHER PLAN. In the event of destructive weather at NNSY, the Contractor will receive directions from the Project via the SUPERVISOR in accordance with the Shipyard and Project Destructive Weather Plan and 2.6.
ELECTRICAL SAFETY REQUIREMENTS: If unexpected voltage is discovered during the Initial Voltage Verification (IVV) of electrical equipment, the
Contractor will utilize 2.8 to determine if the voltage is induced or fixed. If the voltage is determined to be induced, the Contractor will document in the
Block 9 of the Work Authorization Form (WAF) a statement of the locations and values (i.e., Terminal TB 1-6, 4 volts induced, Terminal TB 1-7, 7 volts induced) of the induced voltages found. If the voltage is determined to be fixed or above the threshold noted in Paragraph 6.8 of 2.8, the Contractor will stop conducting the IVV, secure the site and notify the SUPERVISOR. SUPERVISOR will notify the
WIZM and Code 246 for NNSY or Ship's Force evaluation, response and corrective action requirements.
SURVEILLANCES: Contract specification conformance surveillances of
Work Items/documents within the Specification Package will be performed by the
Naval Supervisory Authority (NSA) during the period of performance in accordance with 2.8. Both conformance and non-conformance items will be reported to the executing contracting authority for information or resolution. When a non-
Contractor cannot expect any overtime on the part of NNSY for temporary support services requested after NNSY normal working hours.
