Maintaining Scaffolds Being Utilized by NNSY Employees

1. Agenda Item NNSY NN 12‐03 (Improvement Needed in Setting and Achieving Day to Day Standards in the Fabrication, Maintenance, Use, and Removal of Scaffolds) was closed out on 17 May 2012. One of the problem areas addressed in the document was the specifications of contractor erected scaffolds with one of the apparent causes listed as the scaffolding contract specifications do not give the level of detail required in meeting NNSY IPI 9911‐450 (Scaffolding;

Installation, Use, and Removal Of)

2. To ensure the proper level of details closely match the requirements of the NNSY IPI, Code 970 requests Enclosure (1) be included as part of the contract issued to scaffold erecting companies.

3. Address questions or comments to the Shop 64 Scaffold Manager (396‐3296) or the Code 970 Superintendent (396‐5107).

Requirements to be Included in All Contracts Issued to Contractors Erecting / Maintaining Scaffolds Being Utilized by NNSY Employees

Scaffolding should be installed in accordance with the requirements from OSHA 29CFR‐ 1915, Occupational Safety and Health Standards for Shipyard Employment, any applicable manufacturer’s technical manuals/documentation, as well as the following requirements listed below:

During fabrication, the following are required:

1. Prior to installation of scaffolding, an inspection of each area that requires scaffolding will be conducted with the Contractor, SUPERVISOR (Government activity responsible for the execution and contract administration of Navy maintenance and modernization), Norfolk Naval Shipyard (NNSY) Scaffolding Inspector and/or Project designated scaffolding representative, NNSY Work Integration Manager (WIM)/Work Integration Zone Manager (WIZM), and those parties deemed necessary by the WIM/WIZM to determine the best possible method and locations for installation of scaffolding to minimize modification of scaffolding.

2. The Contractor shall provide, install, and maintain tube and coupler type, or system type scaffolding with sufficient levels and projections to safely reach and have access to all surfaces at each area requiring scaffolding.

3. Prior to the installation/modification or removal of scaffolding, the Contractor shall notify NNSY Scaffolding Inspector and/or Project designated scaffolding representative of the scaffolding request via the SUPERVISOR, Contracting Officer Representative (COR), designated Government Oversight Representative; On‐Site Installation Coordinator (OSIC) or On‐Site Representative (OSR) or WIM/WIZM.

4. Prior to installation, modification, or removal, the Contractor shall establish a boundary in the area of the scaffold to prevent personnel from walking/working in the construction area.

5. Where conditions require ladder heights greater than 24 feet, a rest platform must be provided at no more than 24 feet that is assembled in such a way that personnel do not have to climb over or under a guardrail to enter the rest platform. In addition to having a rest platform at every 24 feet on the scaffold, the ladder section must end at that platform.

Another ladder section can start from an adjacent corner on the rest platform.

6. The side rails of ladders used for access to any level will extend not less than 36 inches above that level to serve as a grab rail to assist in safely accessing or exiting the ladder.

7. Utilize ladders with manufactured slip resistant rungs for all scaffolding locations.

8. Two ladder brackets shall be installed on all starter ladders and one on each remaining section above.

9. The first rung of a ladder shall be a maximum height of 16 ½ inches from the climbing surface.

10. Ladders shall be installed at 90 degrees to scaffold. If unable to achieve 90 degrees, a minimum of 7 inches of horizontal space from the back of the ladder shall be maintained.

11. Swing gates shall be installed and working properly at all ladder openings. Swings gates that are damaged or have become faulty shall immediately be replaced.

12. Toe‐boards shall be a minimum 1 x 6 inch or manufactured nominal equivalence while working on the topside of submarines.

13. Toe‐boards shall be 4 inches past the top of the scaffold platform. Toe‐boards must be installed on all open sides of scaffolds where personnel can work or pass through. Where toe‐boards cannot be installed, the area must be restricted by a rope boundary and a sign stating “Caution Missing Toe‐boards”.

14. Toe‐boards are not required for scaffolds less than 5 feet in height.

15. Scaffold platforms must be a minimum of 24 inches in width except in such cases as the geometry of the vessel or structure makes it impossible to provide such a width.

16. There shall be no gaps greater than 1 inch between planking boards.

17. A minimum of ½ inch (nominal) thick plywood shall be used to fill gaps in planking. All plywood and wooden planks shall meet the requirements of MIL‐L‐19140E, TYPE II, Cat 2 and must be secured in place.

18. The planks of the scaffold deck shall be secured to bearer in such a manner to prevent shifting or lifting from updrafts (i.e. wired down or pipe and clamp).

19. Scaffolding intended for workers only (including the weight of tools) shall be capable of sustaining a load of 25 pounds per square foot at any point or level of working surfaces.

Scaffolding that will support workers and minor materials or equipment shall be capable of sustaining a minimum load rating of 50 pounds per square foot. Scaffolding designed with a load rating of 50 pounds per square foot shall not be used where the weight of the subject equipment results in any loading greater than 50 pounds per square foot.

20. Do not tie into or connect to NNSY scaffolding unless approved by the NNSY Scaffolding Inspector via the SUPERVISOR, COR, designated Government Oversight Representative;

OSIC or OSR.

21. No welded attachments shall be made to the ship’s structure. Alternate attachments points or methods for securing scaffolding to the ship’s structure shall be used.

22. Guard rails are required on all open sides of scaffolding when there is a fall hazard of 4 feet‐ or greater.

23. A third rail shall be installed on topside submarines (including the diving planes), approximately 10 inches above the deck surface.

24. Swivel clamps (including those on manufactured braces) shall not be used to bear any weight.

25. Wood, rubber, or steel plates shall be used at all locations where scaffolding comes in contact with the vessel or equipment.

26. When utilizing System screw jacks, the maximum allowable extension is 14 inches when measured from the deck to the top of the adjustment plate.

27. Scaffolds lifted by crane require special precautions. The Contractor competent person must check all couplers (with a wrench) for tightness. All boards must be secured to bearer and all toe‐boards are fastened securely to scaffold or platform prior to any crane lift. All trade litter and debris must be removed from the scaffold prior to any lift.

28. When using Systems vertical posts, pin and snap buttons must be mechanically fastened (i.e. 3/8 inch‐16NC (grade 5 or better) X 2‐3/4 inch long hex head bolt + 3/8 inch‐16NC hex head nut + 3/8 inch lock washer).

29. On “free standing scaffolds” horizontal diagonal bracing is required for each scaffold 6 feet or taller, every 21 feet in height and as close to the board level as possible.

30. Prior to any scaffolding removal, the Contractor shall notify the COR, designated Government Oversight Representative; OSIC or OSR, WIZM and NNSY Scaffolding Inspector and/or Project designated scaffolding representative via the SUPERVISOR.

During maintenance, the following are required:

1. Every scaffold, completed or under construction, shall have a "Ready for Use" or a "Not

Ready for Use/Out of Service" sign, installed at all accesses.

2. The Contractor shall notify the WIM/WIZM and the NNSY Scaffolding Inspector and/or

Project designated scaffolding representative upon completion of any scaffolding installation/modification to support inspections/certification via the SUPERVISOR, COR, designated Government Oversight Representative; and OSIC or OSR.

3. Prior to a scaffold being placed into service, it will require an initial inspection by NNSY Scaffolding Inspector and/or Project designated scaffolding representative to ensure it has been built/modified to contract specifications. A Contractor competent person shall be on‐ site for the inspection. During this inspection, if anything is not in compliance, the Contractor shall correct the deficiency before it is placed in service and the scaffold shall be re‐inspected.

4. After the inspection has been completed and it has been determined by the NNSY Scaffolding Inspector and/or Project designated scaffolding representative that the scaffold is in compliance with the contract specifications, a Contractor competent person and a NNSY Scaffolding Inspector and/or Project designated scaffolding representative will sign the NNSY “Installation/Inspection” tag, thus placing the scaffold into service.

5. The "Installation/Inspection" tag shall be hung on the main access (ladder) and all other accesses, at a height that a user can clearly read the information on the front and back.

6. Inspection and annotation on Contractor scaffold tag by a Contractor competent scaffold person shall be performed within the first 10 working days of every month and after each installation/modification to ensure that scaffolding meets all requirements of this enclosure and all references.
