Government approval is required for submittals with a "G" designation;

1.2 WORK COVERED BY CONTRACT DOCUMENTS
1.2.1 Project Description
The work includes the following and incidental related work:
In the existing bulk tank area, the three existing bulk tanks will be removed. Fuel piping will be demolished as needed to support the new layout. The fire protection piping will be demolished back to or close to the pump house. The majority of the existing berms will remain; however, portions of the berms will be opened to accommodate future drainage and maintenance. The existing walkways on the berms and elevated above the containment area will either be demolished or abandoned at sections not providing direct access to the new fuel tanks. Electrical conduits serving fire alarm pull stations around the bulk tank farm will be demolished back to Building 123. Electrical conduits serving alarms and sensors at each bulk tank and valve house will be demolished back to Buildings 123 and
124. The old site access road will be re-established with a replacement culvert to provide positive drainage flow and access to the site for future maintenance.
In the transient tank area, the four existing transient fuel tanks and their corresponding dog houses covering the utility pits will be demolished. Piping will be demolished as needed to support the new layout.
The existing concrete saddle foundations will be partially demolished and modified to support the new transient fuel tanks. The existing berm and drainage will remain. Electrical conduits serving transient tank systems and dog house lighting and fire alarm notification devices will be removed back to source panels within Building 103.
A total of four new 50,000 gal rated double-wall bulk tanks with leak detection will be installed in the existing bulk tank area. Both containment berms containing one existing 315k fuel tanks will be replaced with a pair of new 50,000 gal fuel tanks with fuel polishers. The bottom of the tank will be at an elevation of six feet above the top of existing ring foundations wall which will remain. Power and control wiring will be routed via conduit on existing pipe supports and new hot-dip galvanized
SECTION 01 11 00.00 20 Page 2 support beams to each bulk tank and fuel polishing system, originating on the basement and first floor of Building 124. Quantity (10) penetrations, all 2” or smaller, will be required exiting the south wall of Building 124 basement, at an above grade exterior elevation. A new concrete slab will be provided within the perimeter of the existing ring walls for access to the new fuel tanks. New concrete support walls and a mat foundation will also be constructed within the existing ring foundations to support the north end of the elevated fuel tanks in the bulk tank area. Since the length of the new fuel tanks extends beyond the existing perimeter ring foundations, the south end of the fuel tanks will be supported on new concrete support walls and mat foundation constructed outside of the existing ring foundations.
There will be four new double-wall transient tanks with leak detection, each rated at 20,000 gal. These tanks will be located in roughly the same location as the existing transient tanks. New concrete wall extensions will be placed on top of the remaining portion of the outer existing concrete support walls to accommodate the new fuel tank saddle plates. The opening at the top of each existing utility vault will be capped with a weather tight concrete wall and roof enclosure. Power and control wiring will be routed via conduit through Building 103 to the transient tank vault before penetrating the western vertical wall of the new concrete enclosure as required to support transient tank alarms, sensors, and polishing systems.
Quantity (10) penetrations 2” or smaller and Quantity (2) penetration 3” or smaller will be required from Building 103 into the transient tank vault.
Quantity (10) penetrations 2” or smaller will be required through the western wall of the new concrete enclosure adjacent to each transient tank. Replace two day tank fill pumps and motors such that the output is approximately 50 psi at 15 gpm. New pumps must be replaced in the same location as existed, with piping modified as necessary. Utilize electrical to existing electrical connections. Must be compatible with new controls sequence.
The system will operate in much the same way as before.
The project will require execution in phases.
The following items will be included in the bid package as options. Some or all of these options will be accepted should funding allow, but none are guaranteed.
Bid option 1: Demo Bulk Tank Berm Oil Water Separator (LS)
Remove the Oil Water Separator, four (4) Post Indicator Valves, and four
(4) bollards, associated piping, and (3) catch basins associated with the bulk tank berm area. The intent of this option is to eliminate maintenance needs for the oil water separator system once the double walled tanks are placed and the system is redundant.
Bid option 2: Concrete Slab on Abandoned 210K Bulk Tank Foundation (LS)
Remove approximately 6 inches of sand at surface inside existing ring wall foundation for the existing 210K bulk tank and place a new 6 inch reinforced concrete slab within the existing concrete ring walls. The top of the new concrete slab will match the elevation of the existing concrete ring wall.

Bid option 3: Demo Cantilevered Concrete Pan at Transient Tank Dog House (EA)
Remove the existing cast-in-place concrete pan cantilevering from the existing utility pit walls at each transient tank. The existing concrete pan would retain water, snow, and ice since this portion of concrete will not be covered by the new concrete enclosure. The exposed ends of the existing rebar will be coated with a corrosion-inhibiting epoxy resin.
Bid option 4: Reinforced Membrane Waterproofing System (LS)
Apply cold liquid-applied membrane waterproofing to provide additional water protection to building. Color to be coordinated with cultural resources and chosen from manufacture’s base color options.
Bid option 5: Concrete Repairs Inside Transient Utility Tunnel - Crack Injection (LF)
Perform structural crack repairs on interior face of existing concrete utility pit walls at each transient tank. Cracks selected to be repaired will be routed, cap sealed, and filled solid with pressure-injected epoxy resin adhesive. Cap seal will be ground flush with wall surface and injection ports will be removed after crack injection material has fully cured.
Bid option 6: Concrete Repairs Inside Transient Utility Tunnel – Type B Spall
(SF)
Prepare surface, clean and paint the (2) existing steel pipe support frames located inside each existing bulk tank valve house.
Bid option 7: Paint Bulk Tank Pipe Supports (EA)
Transport 650 CY of excess soil offsite to an approved licensed facility in lieu of grading excess soils around the project site.
Bid option 8: Pavement - Mill & Overlay (SY)
Mill top layer of asphalt and resurface with new wearing course. Mill and overlay area is approximately 340 square yard, covering the roadways around the project area. The intent of this option is to provide fresh pavement at the conclusion of the project.
Bid option 9: Fuel Oil Delivery and Tank Filling (EA)
Provide ULSD fuel delivery to fill bulk fuel tanks when requested by the Contracting Officer. Each requested delivery (maximum 7) must provide a minimum of 9,300 gallons of fuel.
Bid option 10: On Site Temporary Fuel Storage for Fuel Oil Removed as Part of Phase 2 (eEA)
Contractor to furnish temporary storage container, mobile or fixed, up to 115K gallons maximum) in area to be coordinated with mission. The intent of this option is to avoid Gov from paying to remove fuel oil and then paying to deliver oil upon project completion. Temporary tanks to be removed after use and site where located returned to its previous condition. The duration of phase 2, expected to be approximately 3 months, is the duration these tanks would be on site.

Bid option 11: Bollards for Transient Tank Parking Lot (LS)
Seven (7) bollards will be placed along the northwest side of the existing parking lot behind B103. The intent of this option is to provide a safety net for the new transient tanks from vehicles.
1.2.2 Project Sequencing Requirements
Parts A, B, and C of this project are intended to be conducted concurrently, under a single phase.
Provide Work Sequencing, Phasing, and Preparation Plan indicating how the work is to be accomplished and incorporating Government directives. The intent of this Plan is to show the Government that a complete understanding of the project requirements are understood including subcontractors. The Plan must be carefully prepared and must include every aspect of the project including outages that will be required to support the construction and how it is to be accomplished with an absolute minimum of disruption to Government operations. No work may begin on the project until this plan is reviewed and approved by the Contracting Officer.
This project requires the phased demotion of three bulk and four transient fuel tanks. Four new horizontal double wall 50,000-gallon bulk and 20,000 gallon transient fuel tanks with containment and leak detection will be provided in their place. The stored fuel will be transported from the bulk tanks to the transient tanks utilizing the existing pumps and the existing pipeline systems. New pipe connections, fitting, valves and accessories will be provided at each new tank.
The proposed phasing is as follows:
Phase 0: The demolition and remove of the decommissioned 210,000 vertical fuel tank.
Phase 1: The demotion of one of the 330,000-gallon vertical fuel tanks and two of the 15,000-gallon transient fuel tanks. Two 50,000-gallon horizontal fuel tanks and two 20,000-gallon horizontal fuel tanks will be installed in place of the existing fuel oil tanks. This phase must be 100% operational before Phase 2 can begin.
Phase 2: The demotion of the remaining 330,000-gallon vertical fuel tanks and last two off the 15,000-gallon transient fuel tanks. Two 50,000-gallon horizontal fuel tanks and two 20,000-gallon horizontal fuel tanks will be installed in place of the existing.
1.2.3 Location
The work is located at the NCTAMS LANT DET CUTLER, Cutler, Maine, approximately as indicated. The exact location will be shown by the Contracting Officer.
1.3 PROJECT ENVIRONMENTAL GOALS
Distribute copies of the Environmental Goals to each subcontractor and the Contracting Officer. The overall goal for design, construction, and operation is to produce a project that meets the functional program needs and incorporates sustainability principles. Specifically:
a. Avoid site degradation and erosion. Minimize offsite environmental

b. Use the minimum amount of energy, water, and materials feasible to meet the design intent. Select energy and water efficient equipment and strategies.
c. Use environmentally preferable products and decrease toxicity level of materials used.
d. Optimize operational performance (through commissioning efforts) in order to ensure energy efficient equipment operates as intended.
Consider the durability, maintainability, and flexibility of building systems.
e. Manage construction site and storage of materials to ensure no negative impact on the indoor environmental quality of the building.
f. Reduce construction waste through reuse, recycling, and supplier take-back.
1.4 SHPO STATEMENT OF WORK
1.4.1 INTRODUCTION
The deliverable for this contract is Maine Historic Building Record (MHBR) documentation of the Cutler Bulk and Transient Fuel Tanks and their related valve houses and infrastructure located adjacent to Building 103 at Naval Computer and Telecommunications Station (NCTS) Cutler in Cutler, Maine.
The tanks are contributing resources to the Cutler Very Low Frequency and High Frequency Communications Historic District (District), and as such is eligible for listing on the National Register of Historic Places.
The VLF Area includes 118 contributing resources and encompasses approximately three thousand acres. The district is significant under Criteria A and C as well as Criteria Consideration G for its exceptional historic and engineering importance in the role of naval communications with a period of significance from 1958 to the current post-Cold War period.
The tanks at Cutler, constructed in 1960, are still used for its original purpose: to provide fuel to generators which power essential operations at the installation. The three bulk tanks and four transient tanks sit on concrete foundations surrounded by earthen berms. Each tank has an associated metal-framed valve house clad in with corrugated metal panels.
The tanks connect to pumps and the generators through a network of piping that run along the ground outside and within tunnels. MHBR documents are complete for the adjacent power plant and for a fueling pier (no longer extant) that can be used to provide information on the larger context of the fuel and power system.
1.4.2 BACKGROUND
This work is required pursuant to the National Historic Preservation Act of 1966 as amended (NHPA) including Section 106 and will be a stipulation in consultation between the Department of the Navy and the Maine State Historic Preservation Officer for the stack replacement.

1.4.3 OBJECTIVE
The purpose of this task order is to develop a Maine Historic Building Record of the bulk and transient fuel tanks at Naval Computer and Telecommunications Station (NCTS) Cutler in Cutler, Maine in order to mitigate the adverse effect to the District caused by the demolition of all seven tanks, their valve house, and some of their related piping. The MHBR documentation will be archived at the Maine State Historic Preservation Office, Augusta, Maine and the Public Works Department, Portsmouth Naval Shipyard, Kittery, Maine.
MHBR documentation is equivalent to Historic American Building Survey documentation; however, the documentation is not submitted to the Library of Congress.
1.4.4 TECHNICAL APPROACH AND REPORTS
1. The contractor must meet The Secretary of the Interior's Professional Qualifications Standards (62 FR 33707 June 20, 1997) for history and/or architectural history.
2. The MHBR documentation shall be prepared and completed in accordance with the Secretary of the Interior's Standards and Guidelines for Architectural and Engineering Documentation for written, graphical and photographical documentation.
3. A thorough search of historic photographs and drawings shall be undertaken to document original conditions and change over time.
4. The documentation shall be completed to Level II MHBR documentation to include but not be limited to the following:
a. Drawings: select existing drawings, where available, shall be photographed with large-format negatives or photographically reproduced including, but not limited to the following:
1) Original plans, elevations and/or sections of the tanks, their valve houses, and related berms and piping.
2) Plans, elevations and/or sections of any significant changes.
3) Digital copies of historic plans printed on 8" x10" archival paper must be submitted if they exist. The Library of Congress' guidelines for scanning measured drawings (which can be found at http://www.loc.gov/pictures/collection/hh/digitizing.html) shall be followed, including the production of 400 dpi TIFF scans with 1 bit-per-pixel tonal resolution.
B. Photographs: black and white, 4"X5" photographs with large-format negatives including, but not limited to the following (Note: specific photographs required will be identified in the Schedule of Documentation issued by the ME SHPO):
1) Context and setting views of the tanks, The bulk tanks and transient tanks are located in two different locations.
2) Overall and detail views of each tank, the valve houses (interior and exterior), and significant landscape features and piping.

3) Exterior oblique views of the bulk tank farm and the transient tank farm.
4) Interior Valve House details of any significant piping or equipment.
5) Detailed views of any manufacturers plates associated with the system if they exist.
C. Written data: short form narrative on the history and description of the tank system using in part the existing MHBR documents for NSA Cutler and primary sources when necessary including, but not limited to the following:
1) Summary of the tanks' construction history including a discussion of changing technologies used at the site.
2) Function and use of the tanks from the time of its construction to the present including the tanks relationship with the larger VLF and HF Communications Historic District and its role in Cutler's operations throughout its history.
3) A general description of the power generation system at the site and a more specific description of how the tanks are used.
4) A comparison of how power generation was handled at the Cutler site vs. other VLF sites.
In addition to the above, during consultation with the Maine State Historic Preservation Officer (ME SHPO) specifics related to the graphical, photographical and written documentation will be outlined in the Schedule of Documentation issued by the ME SHPO. For instance, the Schedule of Documentation may include specific views to be captured photographically and themes/topics/context that should be discussed in the written narrative. The contractor will be provided the Schedule of Documentation once received by the Navy.
Research shall be conducted at relevant repositories in order to complete the MHBR documentation including but not limited to the following: Public Works Department, NCTS Cutler; Public Works Department, Portsmouth Naval Shipyard Kittery, ME; National Archives offices in Washington D.C. and College Park; National Archives Northeast Regional Office in Waltham, Massachusetts and Naval History and Heritage Command, Washington, D.C.
5. The MHBR documentation shall include a 8 x 11 site plan showing the location of the Fuel Complex within its existing setting.
6. Color digital images of all MHBR black and white photographs shall be provided on a CD. The color digital images shall also be submitted on an 8 x 10 glossy photo paper labeled with a permanent marker and shall follow the National Park Service's most recent photo policy for National Register of Historic Places nominations. The color digital images shall be included in the final MHBR documentation to be submitted to the Maine State Historic Preservation Officer.
7. The contractor shall prepare a preliminary draft report that is at 99% for government review. Once the preliminary draft report has been reviewed by the government, the contractor shall be notified of any recommended changes in a timely manner by the NAVFAC MID ATLANTIC representative.

Production of the draft report should be within 30 days of receipt of the government comments.
8. The contractor shall prepare a final draft report for Maine State Historic Preservation Office review. Once the final draft report has been reviewed by the Maine State Historic Preservation Office, the contractor shall be notified of any recommended changes in a timely manner by the NAVFAC MID ATLANTIC representative. Production of the final report should be within 30 days of receipt of the Maine State Historic Preservation Office comments.

Final report - 4 hard copies, 3 electronic copies, 2 copies of archival b/w photos and negatives, and selected existing drawings and 8x11 color digital images shall be included in each hard copy and 1 electronic copy of all selected existing drawings and color digital images. Two copies (one bound and one unbound) shall be printed on archival paper.
All printing practices shall follow the Secretary of the Interior's Standards and Guidelines for Architectural and Engineering Documentation and the ME SHPO specific requirements.
1.4.5 DESCRIPTION OF MEETINGS

2. Field work Kickoff: Will be held at NCTS Cutler, Cutler, Maine
3. Project progress review meetings will be held monthly unless otherwise specified by the NAVFAC Mid-Atlantic Representative.
1.4.6 SPECIAL INSTRUCTIONS
1. The contractor is responsible for obtaining appropriate permissions and clearance from the appropriate MIDLANT and NCTS Cutler personnel to complete the required fieldwork and final documentation. Permissions that may need to be acquired include but not limited to the following: access to certain areas, use of cameras and/or laptops, and clearance of photographs for public release.
2. The contractor is responsible for recording minutes of all meetings and furnishing a copy of the minutes to the NAVFAC Mid-Atlantic representative, regardless of attendance, within 15 days, unless otherwise specified by the NAVFAC Mid-Atlantic representative.
3. The contractor shall forward all submissions (e.g. progress reports, draft and final reports, etc.) directly to the NAVFAC Mid-Atlantic representative.
4. The contractor shall forward all informal documents (field notes, etc.) directly to the NAVFAC Mid-Atlantic representative upon request.
5. The contractor shall submit to the NAVFAC Mid-Atlantic representative all documentation (original reports, drawings, etc.) upon request and/or completion of work.
1.4.7 GOVERNMENT FURNISHED INFORMATION
Copies of all cultural resources reports and other scientific reports relevant to the Task Order which are not available to the general public.

1.5 OCCUPANCY OF PREMISES
Adjacent facilities and buildings will be occupied during performance of work under this Contract.
Before work is started, arrange with the Contracting Officer a sequence of procedure, means of access, space for storage of materials and equipment, and use of approaches, parking, building entrances and exits, and emergency response vehicle access.
1.6 EXISTING WORK
In addition to "FAR 52.236-9, Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvements":
a. Remove or alter existing work in such a manner as to prevent injury or

b. Repair or replace portions of existing work which have been altered during construction operations to match existing or adjoining work, as approved by the Contracting Officer. At the completion of operations, existing work must be in a condition equal to or better than that which existed before new work started.
1.7 LOCATION OF UNDERGROUND UTILITIES
Verify location of underground utilities in accordance with Section
01 35 26.00 22 GOVERNMENTAL SAFETY REQUIREMENTS (PWD ME).
1.7.1 Notification Prior to Excavation
Ground disturbing activities and excavation work are not anticipated for this project and must occur without prior approval of the Contracting Officer. If required and approved, notify the Contracting Officer at least 48 hours prior to starting excavation work. Refer to Section 01 35 26.00 22 GOVERNMENTAL SAFETY REQUIREMENTS (PWD ME) for Dig Safe requirements.
1.8 SALVAGE MATERIAL AND EQUIPMENT
Items designated by the Contracting Officer to be salvaged remain the property of the Government. Segregate, itemize, deliver, and off-load the salvaged property at the Government designated storage area located within 2 miles of the construction site.
Provide a salvage plan, listing material and equipment to be salvaged, and their storage location. Maintain property control records for material or equipment designated as salvage. Use a system of property control that is approved by the Contracting Officer. Store and protect salvaged materials and equipment until disposition by the Contracting Officer.
1.9 BENEFICIAL OCCUPANCY
The Contractor must submit the Certificate of Beneficial Occupancy Acceptance Checklist with the required documentation no later than ten (10) working days prior to requesting the Government take Beneficial Occupancy.
This checklist will be provided to the Contractor by the Contracting Officer.
The Contractor is responsible for ensuring all work noted on this form is complete and accepted by the Contractor's QC Manager prior to submitting the form to the NAVFAC PWD ME Construction Manager for NAVFAC PWD ME signatures. Failure of the Contractor to verify all work and provide the required documentation will delay the processing of acceptance of Beneficial Occupancy.

NAS CUTLER ACCESS INSTRUCTIONS for CONTRACTORS If any changes to this process are needed, please notify William Harke, PWD-Maine at William.t.harke@us.navy.mil
The following instructions are for contractors wishing to gain access to NSA Cutler and NCTAMS det Cutler facilities. They are intended for those that: 1) do not have an active CAC, or 2) a non-government employee with a CAC AND a need to visit the Restricted Area. The Restricted Area is any land area or buildings South of Hudson Blvd and the New Entry Control Point (planned 2025).
Note that the NCTAMS badge is picked up upon arrival and dropped off at the conclusion of that visit, which may span multiple days.
1. Make contact with the appropriate NAVFAC Host or the Contract Specialist for the project being bid. Prime Contractors should fill in the “To Be Completed by Contractor” section of the Access Request List. This should include all their personnel and their subcontractors, if appropriate.
Any questions should be directed to the NAVFAC host. Generally, the hosts are:

b. A/E (including Pre-Award): D&C
c. Post Award Contractors: CM
2. After submitting the completed Access Request List, the NAVFAC Host will notify you when SECNAV 5512 forms are to be submitted. Please note:

c. SECNAV Form 5512 includes Personally Identifiable Information (PII) and is to be submitted ONLY to Cutler Security. PII should be treated appropriately.
d. Forms need to be submitted a minimum of 12 days prior to badge pickup
e. If the planned site visit is more than 30 days after 5512 submission, the contractor should bring this to the attention of the NAVFAC host.
f. Incomplete or missing Information will delay the vetting process. If information is incomplete or missing, Cutler Security will contact the individual.

1.2 SUBMITTALS

1.3 SPECIAL SCHEDULING REQUIREMENTS
a. During the implementation of the project, if differing site conditions are encountered that generate an RFI which requires technical review by the DOR and the Government for the electrical design, a minimum of 7 working days will be required to provide the vetted response. The Contractor must include this requirement in his/her project schedule.
b. The VLF Facility site will remain in operation during the entire construction period. The Contractor must conduct his/her operations so as to cause the least possible interference with normal operations of the Activity.
c. During the entire construction period for this project, other construction projects and maintenance activities will be occurring within and around the Helix Houses and on the Activity. The Contractor must coordinate the work of this contract with the other projects and maintenance activities.

d. Permission to interrupt any Activity roads and/or utility services must be submitted to the Contracting Officer in writing a minimum of 15 calendar days prior to the desired date of interruption. The Outage process (Instruction 11300.9) is used for this purpose, as discussed under UTILITY CUTOVERS AND INTERRUPTIONS. A minimum of 21 days (3 weeks) is required for outages that will directly affect the mission.
These outages must be on Monday maintenance days between 0800 and 1400. Any mission outages outside of these hours must be identified on the project schedule for long term coordination.
e. Coordinate the work with the sequencing/phasing requirements outlined in Section 01 11 00.00 20 SUMMARY OF WORK (PWD ME). The project must be sequenced to limit power and communication outages to the periods of time as indicated and as directed.
f. Have materials, equipment, and personnel required to perform the work at the site prior to the commencement of the work. Specific items of work to which this requirement applies include:
(1) Environmental protection components.
(2) Work within bunkers, manholes, and tunnels.
g. The work under this contract requires special attention to the scheduling and conduct of the work in connection with existing operations.
Identify on the construction schedule each factor which constitutes a potential interruption to operations.
The following conditions apply:
(1) VLF signal transmission operations.
(2) Work within bunkers, manholes, and tunnels will be subject to increased restriction. Work in these areas will be allowed for approximately one 6.5-hour period (8:00am to 2:30pm), once each week, during scheduled 'down days'. Down days may be cancelled or extended as directed by the Contracting Officer.
(3) Coordination with Activity operations.
(4) Coordination with other construction and maintenance projects on the Activity.
k. Removal operations must not commence until the specified work plans and environmental protection plan have been approved by the Contracting Officer.
1.4 CONTRACTOR ACCESS AND USE OF PREMISES
Ensure that Contractor personnel employed on the Activity become familiar with and obey Activity regulations. Keep within the limits of the work and avenues of ingress and egress. Do not enter restricted areas unless required to do so and until cleared for such entry.
All Contractors' equipment must be conspicuously marked with company signage for identification.

1.4.1 Subcontractors and Personnel Contacts
Furnish a list of contact personnel of the Contractor and subcontractors including addresses and telephone numbers for use in the event of an emergency. As changes occur and additional information becomes available, correct and change the information contained in previous lists.
1.4.2 Vehicle List
Submit an original list of vehicles to be utilized at the work site with the following information for each vehicle:

f. Current Base pass expiration date (if applicable).
1.4.3 Identification Badges and Installation Access (NCTS Cutler)

Submit to John Watson and Seth Maker via email (email addresses below) a list of names of those Contractor personnel who require access to the Facility, along with the company each person directly works for, the buildings/locations on the Facility where each person will be working, and the project title. The Contractor is required to identify the start and end date of their request. Each request can cover up to a year.

When Contractor employees arrive at the main gate and Pass Office, they must have appropriate ID and check in for a base access badge at the main gate and also a mission access badge at Building 100. The list of ID requirements is on page 3 of the access form. If a drivers license indicates “not for Federal use” or something similar on it, it cannot be used as a form of ID.
f. See below for additional information.
Contractor must submit the company name, names of employee's, date of birth, place of birth and contracted period of performance using SECNAV Form 5512-1 (May 2021) (Attachment A) a minimum of 7 working days in advance for vetting and approval as described above. The employees will be placed on an access list for the duration of the contract as requested for up to 365 days. At the end of that time period the Contractor must resubmit access request information. The Contractor must follow this procedure each year for the duration of the contract. A picture ID (i.e., driver's license) will be required to enter the facility. Driver's licenses must be "Real ID" compliant. Refer to OPNAVINST 5530.14E Chapter 12, Table 12-2, for a list of acceptable forms of ID for Base access (refer to Attachment A). All vehicles are required to have current registration, inspection and insurance. For concerns regarding access contact the Base Police at 207-259-8267.
1.4.4 Contractor Daily Check-In
Contractors must report to the NCTAMS LANT DET CUTLER Administration Office for check-in and issue of a building/facility access badge before proceeding to their work site. The contractor must return the access badge to the NCTAMS LANT DET Cutler Administration Office once their work is completed. For contractors that will be working onboard NCTAMS LANT DET Cutler for periods longer than one week, an inventory of the company's employee's access badges must be verified with the NCTAMS LANT DET Cutler Administration Office by COB every Monday.

All contracts involving radiation generating devices must conform to the requirements listed in Section 01 35 26.00 22 GOVERNMENTAL SAFETY REQUIREMENTS (PWD ME) and U.S. Army Corps of Engineers Safety Manual EM 385-1-1. All requirements are to be submitted to the Contracting Officer at least 14 days prior to commencement of operations involving radiation generating devices. 1.5.2 Laser Control
Contractor must comply with laser safety requirements under 21 CFR 1040 and ANSI 2136.1-1986 for any work under this contract utilizing lasers.
1.5.3 Energy Conservation
In cooperation with Government representatives, the Contractor must participate in an active program directed toward the efficient use of energy. Government furnished utilities will not be provided for air conditioning of Contractor trailers or office areas.
1.5.4 Fire Prevention
Contractor must familiarize and require all their employees to become familiar with fire prevention regulations within the Station to include the proper method of turning in a fire alarm, storage of flammable and combustible materials and control of combustible waste and trash. Any HOT WORK (welding, burning, grinding, cutting, etc.) requires a HOT WORK PERMIT prior to commencing such work. This permit is obtained from the Activity's Fire Department via the Contracting Officer.
1.5.5 Pesticide and Herbicide Control
Contractor must not apply pesticides nor herbicides unless specifically required by this contract. Where application of pesticides or herbicides is required, provide the submittals required by the specification and obtain written approval prior to any application. Contracting Officer will require review and approval of pesticides or herbicides submittals.
1.5.6 Smoking Policy
In accordance with NAVFAC policy, smoking is prohibited inside all buildings and other facilities except those areas specifically identified as smoking areas (e.g., smoking shelters). Smoking is not permitted within 20 feet of air intakes, doorways or windows.
1.6 WORKING HOURS
Regular working hours must consist of a period established by the Contracting Officer between 7 AM and 3:30 PM, Monday through Friday, excluding Government holidays. The regular working hours must be confirmed with the Contracting Officer.
