MEMORANDUM FOR (Fill in name and address of POC questionnaire is being sent to)
FROM: Nathan Lee
AFLCMC/WWMK
6072 Fir Ave, Bldg. 1233 Hill AFB, UT 84056
SUBJECT: Request for Source Selection Information
1. The F-16 System Program Office (SPO) is in the process of conducting a formal Source Selection for the acquisition of services supporting the F-16 PACAF Depot Maintenance Program. These services include support for aircraft scheduled paint and corrosion control maintenance, depot-level modifications, drop-in maintenance, and Contractor Field Team (CFT) support.
2. One of the considerations in proposal evaluation is the verification of an Offeror’s present and past performance on contracts of similar scope. We depend on information, particularly relevant commentary received from agencies such as yours, for the evaluation of the Offeror’s performance on those contracts.
3. Our areas of interest in the Offeror are summarized in the enclosed questionnaire. We request your written response no later than 5 calendar days from the date of receipt.
4. Although this questionnaire was distributed to you by an Offeror/Contractor, it is important that you return the completed questionnaire directly to the Government Past Performance Evaluation Team (PPET) at the following e-mail address: nathan.lee.3@us.af.mil.
NOTE: DO NOT RETURN QUESTIONNAIRES TO OFFEROR/CONTRACTOR!
Please note that once the questionnaire is filled out, it becomes source selection sensitive and needs to be handled accordingly. If you have any concerns, please email Nathan Lee at nathan.lee.3@us.af.mil.
5. Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this source selection.
