About this file

This document contains a past performance questionnaire for a contractor bidding on a federal contract opportunity. The past performance questionnaire requests information about a contractor's previous projects, including descriptions of work, contract periods of performance, dollar values, and ratings for project management abilities. Reference contacts are asked to rate the contractor's performance across various metrics on a scale of 1 to 6, and provide narrative explanations for any ratings of 1 or 2. The federal contract opportunity involves renovating inpatient wards at a Veterans Affairs medical center to remodel the pharmacy compounding area and build a segregated compounding area backup. The project value is between $1 million to $5 million. The solicitation is set aside for service-disabled veteran-owned small businesses. Responses are due by February 22, 2021 to the Veterans Affairs Network Contracting Office located in Shreveport, Louisiana.
