SOUTHEAST LOUISIANA VETERANS HEALTHCARE SYSTEM
NEW ORLEANS, LA
Project Number: 629-18-127 Project Title: Remodel Pharmacy for USP 800 Requirements
SCOPE OF WORK
1. GENERAL
1.1. The contractor shall provide all supervision, labor, material, tools and equipment to provide the following: remodel the current Pharmacy compounding area to meet USP 800 requirements; build out a new Segregated Compounding Area (SCA) as a backup to the primary compounding area.
1.2. This work will be done in phases. Each phase will be completed before starting the next phase.
1.3. Part of the work required will be completed after normal work hours and weekends.
1.4. Contractor shall possess previous experience building USP 797 and USP 800 compliant cleanrooms.
2. Summary of Design Goals and Objectives
2.1. The project is to renovate the existing Pharmacy Compounding Area to comply with the requirements of USP chapter 800 and updates to USP 797. The project will renovate approximately 800 square feet of space of Chemo and IV compounding in the pharmacy area including additional space consumed for storage. The existing locker room and toilet will be modified to provide additional working space. An alternate space (SCA) will be provided in an existing Service Bay across the corridor.
2.2. To meet criteria established by USP 797 and 800 for a Low/Medium Risk Class 5 environment for compounding and an ISO 7/8 Clean Room, the project renovation of existing areas will be planned in four phases to minimize the disruption of daily activities. The existing space is to remain in operation while the alternate compounding area is built in a currently largescale storage room across the corridor from the pharmacy.
3. Existing Conditions
3.1. The existing Pharmacy compounding space is accessed into an Ante Room, off the IV compounding area. Personnel must pass through the IV compounding to access Chemo buffering without an enclosed airlock. This sequence creates additional criteria for gowning/de-gowning. No hazardous storage area is currently available. The materials for these spaces include hard ceilings and walls with epoxy flooring with integral base. Wall surfaces are epoxy paint. Each area includes worktables, furniture, storage carts and storage shelves. Access is through a combination of swing and sliding doors.
3.2. The alternate compounding area will be constructed in a large storage room across the hall with its own access control. The current space is relatively open with excellent access to utilities and infrastructure.
3.3. Access to the Pharmacy Department must be limited at all times. Construction access must therefore be well defined and secured to the greatest extent possible. Access control will be an operational issue to be addressed by the local staff. The extent of the modifications will be extensive enough to preclude enclosure of all operations while maintaining Pharmacy operations. As a result, phasing and some night or weekend work is required.
4. Demolition Requirements
4.1. The alternate space will be constructed first to allow continuous operations during reconstruction of the existing space. The corridor entry door will require relocation. Other demolition work will be limited in this area. Existing space will be generally demolished including locker room and portions of the toilet spaces to provide additional workspace. Mechanical systems will be reworked in the interstitial space above.
4.2. Temporary enclosures will be required for security of the active pharmacy and new construction in general. However, new alternate construction space will be completely isolated from the balance of the active Pharmacy. Any required utility shutdown of the existing space is likely to occur over a weekend to minimize the impact to on-going Pharmacy operations.
5. Scope of New Construction
5.1. The new work is primarily that of tenant fit-up. HVAC systems will be modified to take the primary space off a high-volume, multiple-floor AHU and provide its own, more local and dedicated unit. The remaining portion of the locker room will be the new IV buffer space with an area that included the former refrigerators modified for chemo storage space. The toilet door will be relocated to the corridor.
5.2. Electromagnetic locks and card readers will be provided at compounding entry points, and power-assistance will be provided at buffer doors to ensure sterile passage between spaces. Mechanical, electrical, and plumbing (MEP) systems will be reconstructed as required for compliance.
5.3. As noted, the alternate space will be constructed first, with only the Ante Room opening into IV and Chemo separately.
5.4. New finishes will be in accordance with any required standards. New construction will consist of repair and replacement as required for new design a full height gypsum wall board on metal stud partitions and gypsum board constructed ceilings. Finishes will include epoxy flooring and base and High Build Glazed Coating (Epoxy Coating) on walls and ceilings. New hollow metal frames and hollow metal doors will be installed. A new pass-through cabinet will be installed as indicated on drawings. New systems work will primarily be mechanical improvements. A water repellant membrane similar to a roofing product will cover the Segregated Compounding Area.
5.5. Additional Pharmacy Perimeter Wall required to meet the design intent will be constructed with expanded metal security mesh at the alternate space to provide the necessary forced entry protection. The wire mesh will be installed on the face of the metal studs on the corridor side of the wall.
5.6. Work includes specialized construction labor & supervision for architectural, mechanical, structural, electrical, plumbing, fire protection and demolition work as required by provided construction documents. This Scope of Work is not considered to be a comprehensive description of work details for this project. All work shall be completed per the plans/drawings and specifications.

A.3 PROPOSAL PREPARATION AND EVALUATION INFORMATION
This part describes the precise mechanism for evaluating and selecting offers for this procurement for:
PROJECT NO. 629-18-127 Remodel Pharmacy for USP 800 Requirements A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Proposals must be complete, self-sufficient, and respond directly to the requirements of this solicitation. Offerors are required to meet all solicitation requirements. The Government reserves the right to make a final determination on a proposal’s acceptability/ unacceptability solely on the basis of the proposal submitted and to proceed without requesting additional information. Failure to meet a requirement may result in an offer being removed from consideration for award.
B. Offerors must provide all requested information for each factor. Proposals must address each element of the factors as applicable to be considered responsive to this solicitation. Factor headings should be highlighted in yellow or typed in bold text for identification. It is the offeror’s responsibility to ensure the submitted proposal is in the proper format and the provided information is clear and free of ambiguities.
C. Offeror’s proposal should contain the offeror’s best terms from a price standpoint. Offerors are cautioned to submit sufficient information and in the format specified in the proposal instructions. Communication conducted to resolve minor or clerical errors would not constitute discussions and the Contracting Officer reserves the right to award a contract without the opportunity for proposal revisions or discussions.
D. Evaluations: - Factor 1 (Technical Factor) is more important than Factor 2 (Past Performance). All Non-Priced Factors (Technical and Past Performance), when combined, are approximately equal in importance to Price (Factor 3).
E. THE GOVERNMENT RESERVES THE RIGHT TO AWARD TO OTHER THAN THE OFFEROR WITH THE LOWEST PRICED PROPOSAL OR HIGHEST RATED TECHNICAL PROPOSAL. The source selection process will be a negotiated source selection process utilizing a tradeoff strategy for all non-price evaluation factors in accordance with FAR 15.101 and FAR 15.101-1 respectively. Offers will be evaluated using the evaluation factors listed. It is the offeror's responsibility to ensure the completeness of the proposal. The SSEB may evaluate solely on the information provided in the initial proposal and will not assume that an offeror possesses any capability that is not specifically identified in the proposal.
a. The objective of the source selection is to select the offer that represents the best value to the Government. In order to select the offeror representing the best value, the Government will rate the offers by conducting a comparative assessment of all acceptable offers against source selection criteria in the solicitation. Benefits associated with additional costs will be identified and considered by trading off difference in capability and price. If one offeror has better capability and lower price, then that will be the better value. If one offeror has the better capability and the higher price, then the source selection authority (Contracting Officer) will decide when the difference in capability is worth the difference in price. NOTE: Price will be evaluated but not ranked by CO.
b. If the source selection authority considers the better capability to be worth the higher price, then the more capable higher-priced offeror will be the better value. If not, then the less capable, lower-priced offer will be the better value. The source selection authority will continue to make capability and price trade-offs until he has decided which offeror represents the best value to the Government. Therefore, the source selection may result in the contract being awarded to other than the lowest priced offeror or other than the highest technically rated offer.
Proposal size will be limited as shown and font/size shall be no smaller than Times New Roman 11. If these page limits are exceeded, the pages in excess of the limit will not be read or considered.

It is the Government’s intent to award without discussions (See FAR 52.215-1). In accordance with FAR 15.306(b)(1)(i), communications may be held with offerors before establishment of the competitive range if the offerors past performance information is the determining factor preventing them from being placed within the competitive range. If discussions are required, the CO will establish a competitive range of the most highly rated proposals and will then enter into negotiations and will allow for Final Proposal Revisions. Final proposal revisions shall be submitted as specified except as noted below and will be due at a time and place to be determined.
F. Basis for Contract Award:
1. Source Selection: The Government intends to award a single Firm-Fixed-Price contract for construction to the best overall proposal which is determined to be the best value to the Government.
2. Awarding Without Discussions: In accordance with FAR 52.215-1(f) (4), the Government intends to evaluate proposals and award the contract without discussions. Communication between the Government and offerors will be limited to exchanges for clarification as described in FAR 15.306(a). In accordance with FAR 15.306(b)(1)(i) communications may be held with offerors before establishment of the competitive range if the offerors past performance information is the determining factor preventing them from being placed with the competitive range. Such communications shall address adverse past performance information to which the offeror has not had a prior opportunity to respond. Any proposal that does not have a reasonable chance of receiving award may be accordingly eliminated from competition and further evaluation. A competitive range may be established if discussions are determined to be necessary by the Contracting Officer. Should the Contracting Officer determine that discussions are necessary, they will be held with only those offerors determined to be in the competitive range, after which final proposal revisions will be requested. Final proposal revisions shall be submitted as specified and will be due at a time and place to be determined. Firms submitting final proposal revisions will not be requested to re-submit any documents which are unchanged from their initial proposals. Offerors shall submit necessary changes as individual paragraphs or otherwise as briefly as possible, together with a table of contents, which clarifies where within the initial proposal additional or changed documents would be placed.
3. Relative Weight of Evaluation Factors: Evaluations: - Factor 1 (Technical Factor) is more important than Factor 2 (Past Performance). All Non-Priced Factors (Technical and Past Performance), when combined, are approximately equal in importance to Price (Factor 3).
