About this file
This is an exhibit document related to a Group Life & Disability and Supplemental Health Benefits solicitation for Brevard County, Florida. The County seeks to expand its voluntary employee-paid benefit offerings to include group-sponsored Accident, Hospital Indemnity, and Critical Illness/Cancer coverage, consolidating these products under a single carrier/administrator effective January 1, 2027. Currently, these supplemental health products are offered individually through multiple representatives including Aflac, Allstate, and Colonial Life. The County intends to award the contract to one carrier/administrator for all requested lines, with group-administered benefits eligible for payroll deduction and group administration beginning January 1, 2027. This represents an expansion of the County's existing fully insured Group Term Life, Accidental Death & Dismemberment, Short-Term Disability, and Long-Term Disability coverage currently provided through Standard Insurance Company since January 1, 2019.
Employees currently enrolled in individual Colonial Life, Aflac, and Allstate plans may elect to continue those coverages on a direct-bill basis; however, the County will not permit on-site enrollment for individual plans or allow payroll deductions for them following the contract award date. Proposers should not assume comparable group experience is available for the supplemental health lines being solicited, as these products have been offered on an individual basis rather than as group-administered benefits. The transition to group-sponsored administration represents a significant operational change in how the County manages and administers these voluntary benefit offerings to its employees and retirees.

Welcome
It’s time to enroll in your employee benefits.* This booklet has been provided to inform you of all the benefit options available to you. Please take the time to review the various plan designs and coverages and decide which options best fit your needs.
WHAT DO YOU NEED TO ENROLL?
When it’s time for you to enroll, you will need to have the following items:

to enroll in one or more of the plans

TAKING ADVANTAGE OF PRE-TAX BENEFITS
Brevard County Board of Commissioners has put into place a Section 125 pre-tax plan. Certain coverages you contribute to are deducted from your paycheck on a pre-tax basis. The IRS stipulates that when you elect to have your deductions taken out with pre-tax dollars, you also agree to remain in the benefit plan of your selection for one full year, unless you experience a change in family status. Examples of changes in family status include the following:
| • | Marriage |
| • | Divorce |
| • | Birth/adoption of a child |
| • | Death of a spouse/child |
| • | Spouse loses his/her job |
You must notify human resources in writing within 30 days of the family status change and provide appropriate documentation.
The information contained in this booklet is only a summary of coverage and is not a binding contract. A certificate of coverage will be made available to you that describes the benefits in greater detail. Should there be differences between the information in the booklet and the contract, the contract will govern.
