About this file
Summary
This is Addendum 2 to an RFP issued by Brevard County, Florida's Purchasing Services Department for Group Life, Disability, and Supplemental Health Plan coverage (RFP #1-26-12). The County seeks to contract with a single carrier/administrator to provide group-sponsored Accident, Hospital Indemnity, and Critical Illness/Cancer coverage as expanded voluntary, employee-paid benefits effective January 1, 2027. Currently, the County maintains fully insured Group Term Life, AD&D, Short-Term Disability, and Long-Term Disability coverage through Standard Insurance Company under a contract effective January 1, 2019. The supplemental health products are presently offered individually through multiple representatives including Aflac, Allstate, and Colonial Life. The proposal opening date and deadline is July 28, 2026 at 4:00 p.m. The addendum clarifies that proposers must administer all benefits through a single platform (CBIZ), with no alternative enrollment platforms permitted, and that the awarded carrier will be the sole provider of group-sponsored supplemental health coverage.
The addendum provides critical clarifications addressing 19 questions from prospective contractors regarding plan designs, rates, claims data, experience reports, and administrative requirements. The County clarified that no credible group experience data exists for the supplemental health lines, as these products are currently offered on an individual basis rather than as group-administered plans. The County confirmed that supplemental health plans will be self-administered and self-billed by the participating employer, and that FMLA/leave administration is handled internally. Employees currently enrolled in Colonial Life, Aflac, or Allstate plans may continue coverage through direct billing, but going forward, the County will not permit on-site enrollment support, payroll deductions, or inclusion of these individual plans in employee benefits materials. The revised Exhibit C with the most current claims data from Standard Insurance and separate experience for active and retiree groups was included with the addendum.

GROUP LIFE DISABILITY & SUPPLEMENTAL HEALTH PLAN
RFP#1-26-12
July 23, 2026 Addendum 2
TO ALL PROSPECTIVE CONTRACTORS
This is an Addendum to and shall be considered as part of the original Proposal package for the above-mentioned Proposal. Contractors are requested to acknowledge receipt of this Addendum with their Proposal.
CLARIFICATION TO QUESTIONS ASKED/RECEIVED:
1) Do you have the following information available for the group’s Accident, Hospital &
Critical Illness plans:
• Plan designs
• Rates
• Participation Census
Answer: Through this solicitation, the County is seeking to expand its voluntary (employee-paid) benefit offerings to include group-sponsored Accident, Hospital Indemnity, and Critical Illness/Cancer coverage. Currently, these products are offered individually through multiple representatives, including Aflac, Allstate, and Colonial Life. Because these are not group-administered products, proposers should not assume comparable group experience is available for these supplemental health lines. The experience reports provided in this RFP include all available information; there is no credible group experience for the Supplemental Health lines.
2) If we are unable to residential build our life insurance products in CBIZ, could we still offer them to be enrolled on an alternative platform and provide election data return back to the County?
Answer: No. The intent of this RFP is to establish a single administrator to streamline benefits administration.
3) Would you be interested in more information about added benefits on our new hospital plan that includes reproductive health benefits as well as any other benefits such as family support benefits?
Answer: Yes. Proposers may submit this type of information as supplemental material for the County's consideration.
4) What is the current HRIS/Enrollment platform these benefits are enrolled in?
Group Life Disability & Supplemental Health Plan
July 23, 2026
Answer: CBIZ
5) Does Brevard County currently outsource FMLA/Leave administration as part of the short-term disability?
Answer: No. Leave administration is handled internally by the participating agency.
6) The voluntary life and voluntary AD&D rates are combined – can you share what the voluntary AD&D rate is?
Answer: See Addendum 1, Question 14.
7) Are you able to provide the AFLAC claims date over the most recent 3 years? (paid premium versus paid claims by product)
Answer: See Addendum 2, Question 1.
8) Are you able to provide a recent invoice of the AFLAC benefits?
Answer: See Addendum 2, Question 1.
9) Can you provide the Standard claims data with a more current valuation date? The claims data is from November of 2025 and we are looking for a claims run that is within the last few months.
Answer: Please refer to the revised Exhibit C included with Addendum 1, which reflects the most current claims run available.
10) Can you provide a full 5 years of Standard claims data? Currently it is only 12/1/2022 to
12/1/2025 (3 years)
Answer: Please refer to the revised Exhibit C included with Addendum 2.
11) Have there been and rate or plan changes in the most recent 5 years year with the
Standard?
Answer: See Addendum 1, Question 14.
12) Life and AD&D coverages only – Experience - In the experience report that was issued in Addendum 1, it appears that both active and retiree basic lift experience is combined.
Are you able to provide separate experience for actives and retirees?
Answer: Please refer to the revised Exhibit C included with Addendum 2.
13) Plan Design - Does the retiree group have its own certificate and/or policy? If so, please provide.
Answer: Yes. Please refer to Exhibit B – The Standard Life Plan Certificate for Retirees, included in the original bid solicitation package.
14) Plan Design - Is the retiree group open to future retirees? If not , when did the group close?
Answer: Yes, the group remains open to future retirees. Employees may elect Retiree Life insurance at the time of retirement. If a retiree does not elect coverage at retirement, they will not have the opportunity to enroll at a later date.
15) Supplemental Heath coverages only – Are in-force certs available for (Accident, Critical
Illness and Hospital Indemnity)? If not, please provide inforce benefit schedule amounts.
Answer: Please refer to Exhibit B (Allstate, Aflac, and Colonial Life) included in the original bid solicitation package.
16) Supplemental Health coverages only – Will the supplemental health plans be self-administered/self-billed by the employer, or will the insurance carrier be responsible for maintaining individual employee records and for generating monthly invoices?
Answer: The supplemental health plans will be self-administered and self-billed by the participating employer.
17) Supplemental Health coverages only – Are the current accident, critical illness, and hospital indemnity products through AFLAC and Colonial individual products or group products?
Answer: These are group products; however, they are not currently offered on a group administration basis. Employees currently enrolled in Colonial Life, Aflac, or Allstate plans will have the option to continue coverage through direct billing and convert their policies directly with the respective carrier.
Going forward, these carriers will not provide onsite enrollment support, payroll deduction, or inclusion in Brevard County employee benefits materials. The only group-sponsored supplemental health products will be those awarded through this solicitation.
18) Supplemental Health coverages only – Please confirm the awarded carrier will be replacing the existing accident, critical illness and hospital indemnity plans with this RFP (ie: there will be only one carrier offering supplemental health to Brevard County employees)?
Answer: Confirmed. The carrier awarded through this RFP will be the sole provider of group-sponsored supplemental health coverage for Brevard County employees.
19) If a responding organization is a subsidiary of a larger parent company and intends to include certain products or services provided by its parent company as part of its proposed solution, would the parent company be disqualified from independently submitting its own response to this RFP?
Answer: See Addendum #1, response to Question #15.
Please note that the proposal opening date and time remains July 28, 2026 no later than 4:00 p.m.
All other terms and conditions remain unchanged.
Sincerely, Mary Bowers Purchasing Manager
