Brevard County, Florida

About this file

This is Addendum 1 to a Request for Proposal issued by Brevard County, Florida, for Group Life, Disability, and Supplemental Health Plan coverage (RFP #1-26-12). The County seeks a single carrier/administrator to provide mandatory lines of coverage including Group Term Life, Accidental Death & Dismemberment (AD&D), Short-Term Disability (STD), Long-Term Disability (LTD), Accident, Hospital Indemnity, and Critical Illness/Cancer insurance for eligible employees, dependents, and retirees, with an intended effective date of January 1, 2027. The addendum addresses 33 questions and clarifications submitted by prospective carriers regarding the solicitation requirements. The proposal submission deadline is July 28, 2026, at 4:00 p.m., with no extension granted despite requests. OneDigital/Digital Insurance, LLC has been designated as the County's Broker of Record, and all quotations must be submitted at standard filed commission rates for the State of Florida for each applicable product line.

Key requirements specified in the addendum include that proposers must waive all minimum participation requirements for all lines of coverage, agree to administer coverage on a no loss, no gain basis for currently insured employees and retirees, and submit proposals that include all mandatory lines under a single carrier arrangement or risk being deemed non-responsive. The County is currently insured through Standard Insurance Company with coverage effective since January 1, 2019, and the new contract will consolidate supplemental health products that are currently offered individually through multiple representatives (Colonial Life, Aflac, and Allstate). Updated census data, claim runs, and incurred paid exhibits through March 31, 2026, have been provided. The County participates in the Florida Retirement System (FRS) and Social Security (FICA), and the evaluation committee may give preference to proposals offering implementation, technology, or discretionary credits in the range of $20,000 to $50,000 to offset technology, enrollment support, and communications costs. The Attachment C Premium Worksheet must be submitted in a separate sealed envelope, while completed proposal forms, including the addendum acknowledgment, should be submitted under Tab 7 of the main proposal package.

GROUP LIFE DISABILITY & SUPPLEMENTAL HEALTH PLAN

RFP#1-26-12

July 17, 2026 Addendum 1

TO ALL PROSPECTIVE CONTRACTORS

This is an Addendum to and shall be considered as part of the original Proposal package for the above-mentioned Proposal. Contractors are requested to acknowledge receipt of this Addendum with their Proposal.

CLARIFICATION TO QUESTIONS ASKED/RECEIVED:

1) Does the County already have a designated broker/agent? If so, whom?

The answer to this question may save dozens of hours of work from multiple vendors we are approaching regarding this RFP. The question stems from the highlighted section below from the RFP document.

Answer: OneDigital /Digital Insurance, LLC is to be designated as the County's Broker of Record.

2) Is this a carrier or broker bid?

Answer: This is a carrier solicitation.

3) Has Brevard County chosen a broker of record?

Answer: Please see response to Question #1

4) Will Brevard allow for an extension to allow carriers time to price, gather all required materials and assemble attachments?

Group Life Disability & Supplemental Health Plan

July 17, 2026

Answer: No extension to this RFP due date will be granted. Given the October open-enrollment timeline, the County is committed to maintaining the current schedule.

5) Why is the Addendum Acknowledgement on a page with the header “Attachment C-

Premium Worksheet”? Page 23 on the RFP. Is this an error?

Answer: The Premium Worksheet is Attachment “C” to the RFP documents and should be completed and submitted in a separate sealed envelope. Page 23 also includes the Addenda Acknowledgement that should be completed, signed and submitted with your Proposal under Tab 7: Required Proposal Forms.

6) Please confirm if the County participates in a state retirement plan.

Answer: Yes, the County participates in the Florida Retirement System (FRS).

7) Please confirm if the County participates in social security retirement plan.

Answer: Yes, the County’s payroll system deducts FICA taxes for applicable employees.

8) Please confirm if there is a set dollar amount on implementation credits requested.

Answer: There is no set dollar amount required. However, the Evaluation Committee may give preference to firms that propose implementation, technology or discretionary credits in the range of $20,000 to $50,000 to offset costs associated with technology, enrollment support and communications.

9) The experience is only through 11/30/2025 and with it being already in July:

a. Can we get an updated PDI (incurred) exhibit to include up through most recent?

Answer: Please see the updated Exhibit C – Incurred_Paid (Updated) included with this

Addendum 1.

b. Can we get an updated claim run to include up through most recent along with classes (Group has a 90 EP and 180 EP class option).

Answer: Please see the updated Exhibit C – Claimrun (Updated) included with this Addendum

1.

10) The census contains retirees that are “retiree waiver of premium”. Are these retirees currently on WOP with the current carrier? Are we being asked to cover these waiver liabilities?

Answer: The retirees listed in the census as " retiree waiver of premium " are currently on

Waiver of Premium with the current carrier. Proposers are being asked to assume these waiver liabilities.

11) Are we able to get better experience? Only given through 11/30/2025 and only 3 years’ worth but the group has been in force much longer.

• The Basic life total premium appears to be wrong (see below). Is the total supposed to go back more than 3 years, or is this an error? The claims amount for basic life and basic AD&D is correct.

Answer: Please see the updated Exhibit C included with this Addendum 1.

• There are 6 LTD claims in the 3-year window that do not have a reserve, but also no term date. Have these claims termed or are they pending info?

Answer: Upon review, the current carrier identified 2 open LTD claims without a reserve (referenced as JY7739 and KL9022). The remaining claims referenced in the original inquiry do have reserves and termination dates on file. The 2 claims without reserves remain open because the claimants are receiving the minimum monthly benefit through the maximum benefit period, and an existing overpayment exceeds the total remaining minimum benefit, making a reserve unnecessary. These claims are not pending additional information.

Answer: Please see the updated Exhibit C experience reports included with this Addendum 1.

12) There is a request to match the employer’s share of FICA taxes for STD. Is that happening now? The experience shows $0 for FICA all 3 years so wanted to clarify.

Answer: The current carrier, The Standard, has a FICA services agreement in place. Under this arrangement, The Standard pays applicable FICA taxes on STD benefit payments and prepares the corresponding W-2s on behalf of the County. The $0 reflected in the experience data is consistent with this arrangement. Proposers should confirm whether they are able to provide equivalent FICA administration services and disclose any associated fees in their proposal.

13) Since this bid requires printing, mailing and notarizing we would need a response by

7/17 or it is very likely an extension of the due date would be needed. Could you let us know if you can provide the below and if an extension is likely?

Answer: No extension to this RFP due date will be granted. Given the October open-enrollment timeline, the County is committed to maintaining the current schedule.

14) Here is a full breakdown of what we need:

Census: Please see updated Exhibit A Census in MS Excel included in Addendum

• Salary and earnings for all employees, and specifically for those currently on disability Salary and earnings data, including for employees currently on disability, are included in the revised census.

• Clarification on the 59 retirees: which 47 are covered and which 12 are Retiree Life Waiver only? The revised census identifies which 47 retirees carry coverage and which 12 are Retiree Life Waiver only.

• VLTD elections by plan (90-day or 180-day) VLTD elections by plan (90-day and 180-day) are included in the revised census.

Rates

• Basic Life and AD&D rate breakdown: the bill currently shows a combined rate of

0.115 for both but we need them separated Basic Life and AD&D rate breakdown are provided in Attachment C.

• Supplemental AD&D rates broken out by EE, spouse, and child, and confirmation of whether the rates shown on the bill are inclusive of AD&D See below. Note: there is no Child AD&D coverage and the rates on Attachment C include both life & AD&D rates per applicable line, as noted in Attachment C.

Experience

• Updated experience through 3/31/2026 or later Please see the revised Exhibit C experience provided through this Addendum 1.

Basic Life, Supplemental Life, Voluntary STD, and Voluntary LTD

• Were there any rate changes from 1/1/2022 to current? No rate changes occurred from 1/1/2022 to present.

• Were there any plan changes in the last three years? The only plan change in the last three years was a modification to spouse age reduction rules, updated to mirror the employee age reduction schedule for administrative consistency. This change was effective 1/1/2026 and is reflected in Attachment B.

15) In reviewing the Mandatory requirements and would like to verify if we cannot meet any of the following requirements would we be automatically disqualified from the bid request process?

Answer: Proposers must submit a proposal that includes all required lines of coverage under a single carrier/administrator arrangement:

• Group Term Life

• AD&D

• Short-Term Disability (STD)

• Long-Term Disability (LTD)

• Accident

• Hospital Indemnity

• Critical Illness/Cancer

Optional Offerings (e.g., Life + Long-Term Care) may be included but will not be evaluated or scored.

16) Can we submit a permanent life product with group Accident, CI and Hospital Indemnity or are we required to have AD&D, STD and LTD as well?

Answer: A proposal that excludes any mandatory line (e.g., omitting AD&D, STD or LTD) may be deemed non-responsive by the Evaluation Committee and removed from further consideration.

17) Proposer must agree to waive all minimum participation requirements for all lines of coverage. Our Life and Group Health products require a minimum of 10 Employee applications to issue coverage, would this disqualify TMK?

Answer: All proposers must agree to waive minimum participation requirements for all lines of coverage. A minimum enrollment threshold of 10 employees as a condition of issuance does not satisfy this requirement. Failure to comply may result in the proposal being deemed non-responsive by the Evaluation Committee.

18) Proposer must agree to administer coverage on a no loss, no gain basis for currently insured employees and retirees. Trustmark does not offer coverage to retirees and we will waive pre-existing conditions on the health products. We do not offer no loss, no gain. Would we be disqualified?

Answer: All proposers must agree to administer coverage on a no loss, no gain basis for currently insured employees and retirees. Trustmark has indicated it will not propose on Group Term Life and AD&D; therefore, retiree eligibility is not applicable to the lines Trustmark is proposing. However, please note that failure to meet the no loss, no gain requirement as it applies to proposed lines may result in the proposal being deemed non-responsive by the Evaluation Committee.

19) Are you able to obtain/provide Colonial Accident rates?

Answer: The information included in the solicitation package reflects all currently available and obtainable rate data.

20) Colonial offers 3 accident plans, which 2 plans should we be competitive with?

Answer: The information included in the solicitation package is all that is currently available.

Please refer to Attachment B, Plan Design Worksheet, for the desired provisions.

21) Colonial Hospital Indemnity Plans do not have admission amounts, can you provide?

Answer: The information included in the solicitation package is all that is currently available.

Please refer to Attachment B, Plan Design Worksheet, for the desired provisions.

22) Colonial Hospital Indemnity High Plan does not have surgery benefit amounts, can you provide?

Answer: The information included in the solicitation package is all that is currently available.

Please refer to Attachment B, Plan Design Worksheet, for the desired provisions.

23) Will the County waive the requirement for manual signatures and permit the use of electronic copy of the authorized signer’s signature on proposal documents?

Answer: Yes.

24) Page 12 of the Proposer Checklist references a "Signed Price Schedule - Attachment C" to be submitted in a separate sealed envelope. We note that Attachment C is provided as the "Attachment C - Premium Worksheet" Excel file, which does not contain a signature block. We also note that page 23 of the RFP is titled "Price Sheet" and contains a signature block and addendum acknowledgment section. Please clarify.

Answer: Please strike the word “signed”. Attachment “C” – Price Schedule should be completed and submitted in a separate sealed envelope. Do not include in your Proposal.

25) Is the "Signed Price Schedule - Attachment C" referenced on page 12 intended to be the signed Price Sheet on page 23, the Attachment C Premium Worksheet Excel file, or both?

Answer: Please see response to Question #24.

26) Should the separate sealed envelope contain only the completed Attachment C Premium

Worksheet, or should it contain both the signed Price Sheet (page 23) and the completed Attachment C Premium Worksheet?

Answer: Please see response to Question #24.

27) Which current carrier plans are the benchmarks for the group product plans to be matched?

Answer: Please see Exhibit B for the current plans offered.

28) Who is the broker on the new group offering? What is the commission expectation?

Answer: OneDigital /Digital Insurance, LLC is to be designated as the County's Broker of

Record. Quote at standard filed commission rates for the state of Florida for each applicable product line.

29) Will questions be answered along the way so that carriers can get info back to their necessary teams to get all elements of the RFP back in time to mail asap.

Answer: All questions received to date are incorporated into Addendum 1.

30) Please provide an STD Claim List.

Answer: This information is included in the revised Exhibit C – Claim Run (Updated) included with this Addendum 1.

31) Please provide an LTD Open & Closed Claims List that includes:

a. Date of Disability

b. Date of Birth

c. Gender

d. Gross Benefit

e. Net Benefit

f. Offsets

g. Total Paid

h. Reserves

Answer: Please see the revised Exhibit C – Offset Report included with this Addendum 1.

32) Have there been any rate or plan changes, at least in the experience period provided?

Answer: the only changes during the experience period are as follows. There have been no rate changes during the experience period.

o STD & LTD: effective 1/1/2023 Tax Collector is deleted from the list of Employer(s) shown in the General Policy Information portion of the Coverage Features.

o Spouse Life: effective 1/1/2026 Amended Spouse Life age reduction to be based on employee age (due to CBIZ limitations can only base premiums and age reduction on EE only)

33) Does the group currently have a ben admin carrier, and if so, who is it?

Answer: CBIZ is the current benefits administration system.

34) Both STD plans are 180 days in length, but there is also a 90-day EP LTD plan. It looks like the contract states that LTD takes over when it starts, but an employee is basically paying too much money if they have STD and a 90-day LTD. Looks like 65 people have the 14/14 plan and 90 day LTD, and then 13 people actually have the 90/90 STD and 90 day LTD, meaning the STD never starts and they are paying STD for no reason. Are we understanding that correctly?

Answer: The County's enrollment system (CBIZ) is configured to guide employees toward compatible STD and LTD plan combinations. The intended pairings are Plan 1 STD with Plan 2 LTD, and Plan 2 STD with Plan 1 LTD, for any employee electing both coverages. This ensures the STD maximum benefit period aligns appropriately with the LTD elimination period. The STD policy includes language providing that STD benefits are offset by any concurrent LTD benefits. The County acknowledges that some employees may not fully understand how these plan interactions work in practice, and this is an area the County is actively reviewing for potential administrative improvement.

Please note that the proposal opening date and time remains July 28, 2026 no later than 4:00 p.m.

All other terms and conditions remain unchanged.

Sincerely, Mary Bowers Purchasing Manager
