About this file
This is a Disqualification Disclosure form required by the State of California under Public Contract Code Section 10162 for the City of Pasadena's Design-Build Services for Solar and Energy Storage Installations at Category 1 City-Owned Properties. The form requires prospective bidders to certify under penalty of perjury whether the bidder, any officer of the bidder, or any employee with a proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation. Bidders must check either "Yes" or "No" to indicate their compliance status, and any affirmative response requires the bidder to explain the circumstances and attach supporting documentation as an exhibit to the disclosure.
The disqualification disclosure is a mandatory compliance document designed to protect the integrity of the procurement process by identifying bidders with relevant disciplinary histories. This form serves as a gating mechanism for the Pasadena Water and Power's RFP process and must be completed as part of the proposal submission requirements. Non-compliance with this disclosure requirement or submission of false information constitutes grounds for bid rejection and potential legal consequences under California law.
