About this file

The document is a Daily Field Report from NOVA, a professional inspection and testing company, documenting structural steel observations and testing for the KSU Clendenin Building Repair project at Kennesaw State University on November 5, 2024. The report details a site inspection of partial structural repairs on the third floor, focusing on field welds between the third floor and roof level, specifically in the area between lines A-B.4. NOVA's representatives conducted visual inspections in accordance with IBC Chapter 17 Structural Tests and Special Inspections, examining welded connections per AWS D1.1 Table 6.1 criteria.

The report identifies several unresolved discrepancies, including anchor bolt size inconsistencies (1/2" versus specified 3/4"), intermittent fillet weld placement issues, and modifications to bent plates and existing building embeds. The general contractor (F.H. Paschen) was informed of these discrepancies, with Shear Structural providing additional review comments. The structural review notes that while most discrepancies were deemed structurally acceptable, there are ongoing concerns about vertical slotted holes in connection angles that need to be addressed to allow for potential differential movement between new structural steel columns and the existing concrete structure.

Activity Date: 11/05/2024

Weather: Cloudy Temperature: 64

NOVA Representative: Roy Dennis

General Contractor: F.H. Pashcen

The following summarizes our observations and activities this date:

Nova was on site to perform the following inspections / observations at the KSU Clendenin Building Repair.

Nova inspected field welds of the partial structural repairs at the third floor (32'0"). Area of inspection was in between third floor (32'0") to roof level (43'10") between line AB.4. (Reference contract drawings S200) Noted items of inspection appear to be in general compliance with the plans & specifications and in accordance with AWS D1.1 T8.1 visual inspection acceptance criteria * with the exception of discrepancies noted in the discrepancy section of this report.

The observations, which were performed in general accordance with the requirements of IBC Chapter 17 Structural Tests and Special Inspections, and IBCGA Amendments, employs visual observation techniques to verify general compliance with the requirements of the project plans and specifications. Welded connections are visually inspected per AWS D1.1 Table 6.1 criteria.

General Contractor stated engineer approved of splicing of bent plate and slotting bent plate to make weld onto existing embed, awaiting confirmation from SEOR.

Noted discrepancies were discussed with the GC prior to leaving the site.

The above recommendations should not be considered authorization for the contractor to do any work above and beyond work contracted with the owner. Any work performed by the contractor requiring a change order should be approved by the owner prior to work being performed.

11.06.2024 Shear Structural Review Comments:

1. Bethany Jones of Shear Structural has reviewed the Discrepancies listed on Page 1 of this Inspection Field Report.

2. Discrepancies 2 through 4 are Structurally Acceptable.

3. The following is in regard to Discrepancy 1 listed on Page 1 of this Inspection Field Report.

It is our understanding that the General Contractor is installing 3/4" diameter anchors per the Structural Detail 5/S-200.

An additional Discrepancy, noted in the Field Observation Report issued by Shear Structural on 11.28.2024, requires correction as follows.

Structural Detail 5/S-200 specifies that vertical slotted holes were to be provided in the connection angles at the top of the new columns to the bottom of the existing concrete structure. The purpose of these vertical slotted holes is to provide for differential movement of the new structural steel columns in respect to the existing concrete structure. Vertical slotted holes could not be field verified by Shear Structural because washers and tack welds had been installed. Washers and tack welds need to be removed from these connections to allow for differential vertical movement. In the photograph shown on Page 3 of this Inspection Field Report, the vertical slotted holes cannot be field verified because the washers have not been removed to allow for differential vertical movement. In the photograph shown on Page 3 of this report, it appears that the tack welds have been removed.
