Document Summary

This document is a Task Assignment Notification/Change Order Form for contract HW550 issued by the Florida Department of Environmental Protection (FDEP). The primary contractor is Geosyntec Consultants, represented by Teresa Fischer, with Kelly Crain serving as the DEP Contract Manager. The work is being performed at Paynes Prairie Preserve State Park (Site ID 60SL-1) in Alachua County, Florida, specifically for Area of Concern 1 (AOC 1) excavation activities. The change order extends the original task assignment SL-004W, adjusting project timelines and deliverables due to a schedule extension requested by the Park Biologist.

The contract encompasses a limited source removal project involving excavation of organochlorine pesticide (OCP) and arsenic-impacted soils from a cattle dip vat site. The total fixed price for the task assignment is $295,534.86, with no cost or fee adjustments in this change order. The project scope includes five subtasks: project management, level of effort preparation, contractor submittal review, limited source removal excavation (estimated at 1,585 tons of soil, with approximately 300 tons potentially characterized as hazardous waste), and preparation of a Limited Source Removal Report. The original completion date of May 27, 2017 was extended by four weeks to June 23, 2017. Invoice payments are structured in two phases: $287,040.25 upon completion of planning, submittals, and field work, and $8,494.61 upon project management completion and report submittal.

Hi Kelly:

Attached please find the Task Assignment Change Order to extend deliverable and the project completion due date. I set everything out by one month in the event we need to start later than

March 27th. I should have confirmation of that by the end of the day today and will send you an email as soon as I know.

Thanks, Teresa

From: Teresa Fischer Sent: Wednesday, November 30, 2016 3:01 PM To: Crain, Kelly (Kelly.Crain@dep.state.fl.us) <Kelly.Crain@dep.state.fl.us> Cc: 'Phillips, Dave M.(Haz Waste)' <Dave.M.Phillips@dep.state.fl.us> Subject: Paynes Prairie Preserve State Park Area of Concern 1 Limited Source Removal Work Plan and LOE, Revised 11/30/2016

Hi Kelly:

Please find attached Geosyntec’s revised proposed level of effort (LOE) to conduct a limited source removal of organochlorine pesticide (OCP) and arsenic-impacted soils at Paynes Prairie Preserve State Park (PPPSP) Area of Concern (AOC) 1 (Site ID No. 60SL-1) in Alachua County, Florida in preparation for Site closure purposes under risk management option II (RMOII). Geosyntec’s proposed work plan is described below and will be completed in accordance with the Technical Specifications issued by Geosyntec on October 2016 and within the limits discussed during the Site Walk held on October 13, 2016.

I am also including a deliverable schedule for grouped subtasks in order to invoice incrementally.

The required tasks for limited source removal work are described below:

Subtask 1: Project Management mailto:TFischer@Geosyntec.com mailto:Kelly.Crain@dep.state.fl.us

Subtask 2: LOE Preparation – The LOE was developed based on guidance contained in Chapter 62- 780, Florida Administrative Code (FAC) (June 2013) and Chapter 62-777, FAC for conducting closure activities, including source removal of areas where soil had detections of OCPs and arsenic above respective soil cleanup target levels (SCTLs).

Subtask 3: Contractor Submittal Review –Geosyntec received and evaluated bids for the source removal activities under a separate Task Assignment. FECC, Inc. (FECC) provided the lowest priced, complete bid package in response to Geosyntec’s Request for Bid dated October 2106. Geosyntec will provide technical review of FECC’s submittals including a Health and Safety Plan (HASP), permits, private utility locate report, Clearing and Grubbing Plan, Decontamination Plan, backfill source laboratory analytical results, and Waste Disposal Characterization and the disposal facility background information.

Subtask 4: Limited Source Removal – Excavation of OCP and arsenic-impacted soils will be performed as described in Geosyntec’s Technical Specifications for PAYNES PRAIRIE PRESERVE STATE PARK CATTLE DIP VAT – AOC 1 dated October 2016 under Geosyntec’s oversight. FECC attended the Site Walk and is aware of PPPSP Park Ranger concerns and will work with Geosyntec and the Park Ranger to insure specific requirements set for the during the Site Walk are completed.

The total volume of soil to be excavated is estimated at 1,585 tons. Of the 1,585 tons, Geosyntec estimates that approximately 300 tons may be characterized hazardous waste because the toxaphene concentrations may fail the Toxicity Characteristic Leaching Procedure (TCLP) once FECC collects composite soil samples for analyses.

FECC and Geosyntec have assumed that excavation activities and restoration will be completed within 10, 10-hour days. Prior to conducting excavation activities, FECC will procure composite soil samples for waste profiling purposes from AOC 1. These samples will be used for generating a hazardous (if necessary) and non-hazardous waste disposal profile. Note that pricing included in the LOE assumes that 1,285 tons of soil will be characterized as non-hazardous waste and 300 tons may be characterized as hazardous waste. Although we do not anticipate more than 300 tons of soil characterized as hazardous waste, Geosyntec will contact FDEP immediately if it appears that more soil will need to be characterized as hazardous waste. Should the waste characterization samples indicate that the soil can be disposed as nonhazardous waste, Geosyntec will complete a Task Assignment Change Order to reduce costs accordingly.

The excavation area will be cleared with the exception of the trees identified by the Park Ranger prior to the removal of the cattle dip vat (CDV) and impacted soils. After impacted soil and the CDV concrete have been excavated, clean backfill will used to fill the excavated areas. A backfill sample will be obtained from the borrow pit source prior to excavation/backfill activities. The results of the backfill laboratory analysis will be reviewed by Geosyntec to ensure that the backfill meets FDEP’s 1 October 2010 “Sampling of Backfill for Sites Undergoing Excavation” Memorandum. The backfill will be machine compacted. It is assumed that a density test will not be required.

Geosyntec will collect up to eight confirmatory soil samples from the walls of the excavation area to confirm the removal of OCP and arsenic-impacted soil. Soil samples will be submitted to Advanced Environmental Laboratories, Inc. (AEL) in Jacksonville, Florida for analysis of OCPs and arsenic using EPA Methods 8081A and 6010B, respectively. Geosyntec will coordinate with AEL for delivery of sample kits and submittal of the samples.

Subtask 4: Preparation of an Limited Source Removal Report (LSR Report) – An LSR report will be generated to summarize field activities for the excavation of soil, field methodologies, and laboratory analytical results of the confirmatory sampling. The report will include appropriate tables and figures.

Geosyntec appreciates the opportunity to assist you and the FDEP with this project. Please review and contact me if you have any questions.

Warmest Regards, Teresa
