20 February 2019
Mr. Killian Talley Florida Department of Environmental Protection Waste Cleanup Program 2600 Blair Stone Road, MS 4520 Tallahassee, FL 32399-2400
Subject: TACO #2: Vick’s Cleaners Inc.
302 Gulf Breeze Parkway, Gulf Breeze, Florida FDEP Eric ID ERIC_5421 FDEP Task Assignment No. DC030C
Dear Mr. Talley:
Please find attached Task Assignment Change Order (TACO) #2 for the level of effort (LOE) previously prepared by Geosyntec Consultants, Inc. (Geosyntec) for supplemental site assessment activities at Vick’s Cleaners, Inc., (the Site) located in Gulf Breeze, Santa Rosa County, Florida.
This proposed CEF/LOE is based on the comments provided by the Florida Department of Environmental Protection (FDEP) and dated 25 April 2018 regarding the Draft Site Assessment Report (SAR) prepared by Geosyntec (January 2018) and based on November 2018 DPT field data and subsequent site visits to re-evaluate previously proposed offsite sampling locations. A detailed explanation of the proposed scope of work and report preparation is provided below. This TACO #2 represents updated costs related to obtaining access agreements, MAGS installation and testing, soil sampling, DPT groundwater sampling, and additional reporting requirements related to increased sampling effort.
SCOPE OF WORK
Subtask 1 – Project Management This subtask covers the general management of the project as specified in Contract No. HW550.
Subtask 2 – LOE Preparation & Fieldwork Logistics Subtask 2 includes costs for LOE preparation, obtaining site access, obtaining right-of-way (ROW) permits from Santa Rosa County to conduct off-site DPT groundwater sampling, and to oversee an underground utility survey of all proposed onsite and off-site drilling and sampling locations.
Time for additional communication with property owners for off-site property access and for two additional sites visits. One site visit was needed to take photos for the City of Gulf Breeze ROW permit application following responses to Sunshine 811 utility locates. One site visit was needed to meet with Mr. Britton Stamps, Duncan Avenue property owner. Costs are included in TACO #2 for an additional Geosyntec staff member to meet with Duncan Avenue property owner, as property owner requested the meeting to take place at his residence due to his limited mobility.
Costs associated with obtaining off-site property access agreements for 60 Northcliffe Drive (Community Bank), 301 and 333 Gulf Breeze Parkway (Bank of America and Flowerama), and right of way permits from the City of Gulf Breeze for Live Oak Street and Hoffman Drive, including figure updates, are included in TACO #2.
Subtask 3 – DPT Groundwater Sampling DPT groundwater sampling will be conducted at up to eight locations (DPT013 through DPT020) as shown on Figure 1 (attached). Groundwater samples will be collected at 4-ft intervals beginning at the top of the groundwater table (16 ft BLS) to 60-ft BLS at DPT013 through DPT015 (11 samples per locations.) DPT013 will be sampled until refusal which maybe deeper than 60 -ft BLS.
Groundwater samples will be collected at 10-ft intervals beginning at the top of the groundwater table (16 ft BLS) to 60-ft BLS at DPT016 through DPT020 (5 samples per locations. Groundwater stabilization parameters will be collected during sampling and samples will be submitted to a fixed-base laboratory for analysis of volatile organic compounds (VOCs) by EPA Method 8260.
DPT groundwater sampling activities are estimated to take four field days to complete.
Singley Construction Company is the suggested subcontractor for DPT groundwater sampling as they provided the low bid for 4 days of DPT sampling. Due to weather delays caused by severe rain and purging difficulties due to lithology (excessive flowing sand drawn into sampling screens and DPT rod fittings), costs are included in TACO #1 for 2 additional days of DPT groundwater sampling needed to complete locations DPT013, DPT014, and DPT015.
Costs are included in TACO #2 for sampling of four supplemental DPT groundwater locations DPT021 through DPT024 (Figure 1). Groundwater samples will be collected at 10-ft intervals beginning at the top of the groundwater table (approximately 16-ft BLS) to 60-ft BLS at DPT021 through DPT024 (5 samples per location). DPT016 will be sampled continuously at 4-ft intervals beginning at the top of the groundwater table (approximately 16-ft BLS) to 60-ft BLS to delineate PCE concentrations observed at DPT015 during the October 2018 sampling event. In TACO #2, costs for six supplemental sample intervals at DPT016 are requested to cover the difference between the new 4-ft proposed intervals rather than the original 10-ft sampling intervals.
Supplemental DPT groundwater sampling activities are estimated to take 4 field days to complete due to mobilization between locations.
Subtask 4 – Monitoring Well Installation and Sampling Four monitoring wells will be installed following completion and evaluation of DPT groundwater sampling activities. Continuous soil borings will be advanced at all proposed monitoring well locations to record site lithology. Proposed monitoring wells MW009 through MW012 will be installed at the locations shown on Figure 1. Monitoring well MW009 will be installed near MW002 into the main producing zone of the Sand and Gravel aquifer (approximately 100-ft BLS) and will be screened from 90 to 100-ft BLS. Monitoring wells MW010 through MW012 will be installed at various offsite locations in the right-of-way of Northcliff Drive and Duncan Avenue.
These wells will be installed to a depth of 50-ft BLS with a screened interval from 40 to 50-ft BLS;
however, the actual depth and screen interval will be based on DPT groundwater sampling data and site lithology. The newly installed monitoring wells will be installed using the sonic drilling method and constructed of 2-in. diameter Schedule-40 polyvinyl chloride (PVC) pipe with 10-ft of 0.010-in. pre-packed standard factory-slotted screen. They will be completed at the surface with a flush-mount manhole, a 2-ft by 2-ft by 4-in. thick concrete pad, and a locking monitoring well cap. The monitoring wells will be developed until the discharge is relatively sediment-free. A groundwater quality meter and turbidity meter will be used for water quality measurements during development. The top-of-casing and horizontal coordinates of the new monitoring well will be surveyed by a licensed Florida surveyor. Monitoring well installation activities are estimated to take five field days to complete.
Huss Drilling is the selected subcontractor for monitoring well installation activities and Fabre Engineering is the selected surveyor.
Geosyntec personnel will gauge the depth to water and collect groundwater samples from monitoring wells MW001 through MW012. Groundwater samples will be collected using conventional low flow purging methods and submitted to a fixed-based laboratory for VOC analysis by EPA Method 8260. Groundwater sampling activities are estimated to take three field days to complete.
Subtask 5 – IDW Management Purge water, drill fluids and cuttings and decontamination water generated during assessment activities will be placed in 55-gallon drums for manifested off-site disposal. Investigation-derived waste (IDW) samples will be collected and submitted to a fixed-base laboratory for VOC and semi-volatile organic compound (SVOC) analysis by EPA Methods 8260 and 8270, and Resource Conservation and Recovery Act (RCRA) 4 metals, as required by the disposal contractor. The analytical results will be used to profile the IDW prior to disposal.
PermaFix is the selected subcontractor for waste disposal. Please note the 10% energy recovery (transportation) fee charged by PermaFix, resulting in a total per drum disposal cost of $159.50.
Vicks SSAR LOE DC030C_TACO#2.docx
Costs are included in TACO #2 for disposal of two additional drums for purge water, drill fluids, and decontamination water that will be generated during supplemental DPT groundwater sampling activities, MAGS sampling, and soil borings proposed in TACO #2 Subtasks 3 and 7.
Subtask 6 – SSAR Preparation Subtask 6 includes costs associated with the preparation of a Supplemental Site Assessment Report (SSAR) which documents the results of the field assessment activities. The DPT and MW groundwater analytical data will be evaluated and summarized using Geographic Information System (GIS) techniques, Iso-concentration maps and cross-sections will be prepared to summarize lithology and analytical results. The SSAR will be signed/sealed by a licensed professional geologist/engineer in the State of Florida. An electronic PDF version of the report will also be uploaded to FDEP’s FTP server for loading to OCULUS. One hard copy of the SAR will be provided to FDEP upon approval of the SAR. In addition, Subtask 6 includes costs for provision of ADaPT files and generation, population, and upload of GIS data tables. Costs are included in TACO #2 for additional reporting efforts related to the supplemental sampling efforts proposed in TACO #2 Subtasks 3 and 7.
Subtask 7 – MAGS Installation with Vapor and Soil Sampling Geosyntec will oversee the installation of three MAGS wells at the locations shown on Figure 1 and Figure 2. The MAGS wells will be installed by Singley and constructed of 2-in. diameter Schedule 40 PVC installed to a depth of 10-ft to 12-ft BLS (or approximately 0.5-ft above the water table) and equipped with 10-ft of 0.010-in. factory-slotted well screen. A 20/40 grade environmental sand filter pack will be placed from the bottom of the well screen to approximately 0.5-ft above the top of the well screen. Proposed MAGS well MAGS012 will be installed inside the current Vick’s Cleaners, and a 6-in. core drill will be used to core through the flooring. Soil samples will be collected during MAGS well installation at MAGS012 and MAGS013 and five additional soil borings will be advanced for sample collected from 0 to 12-ft BLS inside the current Vick’s Cleaners (Figure 2). Singley will core the concrete and hand auger to the appropriate depth and Geosyntec will collected the soil samples for screening and for laboratory analysis. Soil samples from each soil boring will be collected at depth intervals of 0 to 0.5-ft BLS then every 2-ft BLS until the water table for field screening with a PID and laboratory analysis.
