The task assignment authorizes Geosyntec Consultants to conduct two quarters of groundwater sampling and preparation of monitoring reports with a total contract value of $8,355.09 under a fixed-price arrangement with zero retainage. The scope includes Subtask 2 (Quarter 1 Groundwater Monitoring Report and ADaPT/GIS Update) invoiced at $3,875.31 upon FDEP receipt and approval by December 9, 2021, and Subtask 3 (Quarter 2 Groundwater Monitoring Report and ADaPT/GIS Update) invoiced at $4,479.78 upon FDEP receipt and approval by March 31, 2022. The period of performance extends through June 30, 2022. Services include depth-to-water gauging at three monitoring wells, sample collection from two wells, volatile organic hydrocarbon analysis via EPA Method 8260, investigation-derived waste characterization and disposal through Clark Environmental (selected as the lowest-cost provider at $140 per drum), laboratory analysis, report preparation with professional certification, and GIS data updates. No duplicate samples are required due to the minimal number of samples collected.

Subject: Scope of Work and Level of Effort Groundwater Monitoring Report or Site Rehabilitation Completion Report King of Cleaners (ERIC_4984) 5043 Silver Star Rd, Orlando, Florida Task Assignment DC003N (pending)
Dear Ms. Posey:
Geosyntec Consultants, Inc. (Geosyntec) has prepared this proposed Scope of Work and Level of Effort for two quarterly groundwater monitoring events and preparation of two Groundwater Monitoring Reports (GWMRs) at King of Cleaners (Site) located in Orlando, Orange County, Florida. Groundwater samples were last collected at the Site in September 2020. A detailed explanation of the proposed scope of work is provided below.
SCOPE OF WORK
Subtask 1 – Project Management This subtask covers the general management of the project as specified in Contract No. HW550.
Subtask 2 – Quarter 1 Groundwater Monitoring Subtask 2 includes costs associated with quarterly groundwater sampling activities and preparation of the GWMR. Geosyntec personnel will gauge the depth to water in three shallow monitoring wells (MW006, MW009, and MW013) to evaluate groundwater flow direction and will collect groundwater samples from two monitoring wells (MW006 and MW013; Figure 1). Groundwater samples will be collected using conventional low flow purging methods in accordance with FDEP SOPs. No duplicate groundwater sample will be collected due to the minimal samples collected.
Groundwater samples will be submitted to a fixed-based laboratory for volatile organic hydrocarbon (VOH) analysis by Environmental Protection Agency (EPA) Method 8260.
Groundwater sampling activities are proposed to occur in October/November 2021.
Investigation Derived Waste (IDW) will consist of purge and decontamination water generated during sampling activities and will be placed in one 55-gallon drum for manifested off-site disposal. Necessary IDW characterization samples will be collected from the drum upon completion of the sampling event in order to develop a new waste profile and will include analysis of VOHs (8260B) and 8 RCRA Metals (6010C/7470A). Clark Environmental is the selected subcontractor for IDW transport and disposal. Subcontractor bids and evaluation form are included in Attachment A.
The GWMR will document the field activities and results from the first quarterly monitoring event. The groundwater analytical data will be evaluated and summarized graphically using iso-concentration maps, if applicable. The report will be signed/sealed by a licensed professional geologist/engineer in the State of Florida. An electronic PDF version of the report will also be uploaded to FDEP’s FTP server for loading to OCULUS. In addition, Subtask 2 includes costs for provision of ADaPT files and generation, population, and upload of GIS data tables.
Subtask 3 – Quarter 2 Groundwater Monitoring Subtask 3 includes costs associated with quarterly groundwater sampling activities. Geosyntec personnel will gauge the depth to water in three shallow monitoring wells (MW006, MW009, and MW013) to evaluate groundwater flow direction and will collect a groundwater sample from two monitoring well (MW006 and MW013; Figure 1). Groundwater samples will be collected using conventional low flow purging methods in accordance with FDEP SOPs. No duplicate groundwater sample will be collected due to the minimal samples collected. Groundwater samples will be submitted to a fixed-based laboratory for VOH analysis by EPA Method 8260.
Groundwater sampling activities are proposed to occur in January/February 2022.
IDW will consist of purge and decontamination water generated during sampling activities and will be placed in one 55-gallon drum for manifested off-site disposal.
The GWMR will document the field activities and results from the second quarterly monitoring event. In addition, Subtask 3 includes costs for provision of ADaPT files and generation, population, and upload of GIS data tables.
Invoice and Deliverable Schedule:
Geosyntec proposes the following deliverable and Period of Performance schedule below.

BID PROPOSAL FORM
Florida Department of Environmental Protection (FDEP) Drycleaner and Hazardous Waste Sites
Investigation Derived Waste (IDW) Transportation and Disposal
Contractor Company Name: HEPACO, LLC
Contact Name and Phone Number: Linda Basinger (813) 240-8020
Assumptions and Clarifications.
• Unless otherwise specified within this proposal all terms and conditions will be governed by the most recent Master Services Agreement between GEOSYNTEC and HEPACO, LLC. In the event that a current agreement is not in place all terms and conditions will be governed by HEPACO’s standard service agreement.
• All proposed disposal methods and prices are contingent upon profile approval and acceptance into HEPACO’s approved/preferred partner disposal facility.
• The above listed rates apply for less than truck load quantities (LTL). In the event that a higher quantity is needed at one specific location HEPACO can provide additional pricing that will reflect the volume.
• Above rates include a standard energy and insurance fee. If market conditions change dramatically, HEPACO reserves the right to increase or decrease these fees based on the appropriate energy index. In the event that this is required HEPACO will provide a 30 day notice of the anticipated change.
* Excludes Monroe County (Case by case basis determined by volume of IDW)
• Bid costs above should be inclusive of transportation fees, fuel fees, environmental recovery fees, etc.
• No standby time should be included in the above bid costs.
• Assume that the drums are accessible.
• It is assumed that VOH totals results will be provided for profiling. If other analytical results are required, please indicated here:
