Subject: Level of Effort/Cost Estimate Form Phase II Site Investigation Liftmasters, Inc.
12875 58th Street North Clearwater, Pinellas County, Florida
Dear Ms. Crain and Ms. Shirah:
In response to our recent discussions, please find attached Geosyntec’s Cost Estimate Form and proposed Level of Effort (CEF/LOE) to conduct limited Phase II site investigation activities for the above-referenced property (Site). Geosyntec was authorized to conduct a Phase I Environmental Site Assessment (ESA) at the Site in accordance with our 4 September 2020 proposal. Based on our observations during the Site visit on 20 October 2020, historical research, and personal interviews, Geosyntec documented three off-Site facilities considered to be suspected recognized environmental conditions (RECs). This Phase II Site Investigation proposal has been prepared to describe activities designed to evaluate the presence or absence of chemicals of concern in groundwater at the Site.
The scope of the proposed Phase II activities is focused on areas Geosyntec considers as having the greatest potential for environmental impacts based on our understanding of the Site and areas near the Site to date. The findings from this study are intended to provide supplemental information to the Florida Department of Environmental Protection (FDEP or Client) for consideration during due diligence. As such, this investigation will not satisfy all the site assessment requirements specified in Florida’s contaminated site clean-up regulations in Chapter 62-780.600 of the Florida Administrative Code (F.A.C)1. However, the sampling approach and
1 This sampling approach is not compliant with the State’s more conservative site assessment guidance (for example, in terms of horizontal and vertical density of samples) for evaluation of a contaminated site. The proposed analytical testing suite is based on our understanding of the Site’s
Ms. Crain and Ms. Shirah 13 January 2021
FE7551/Liftmaster Phase II SOW_13 Jan 2021 pp analytical parameters proposed herein have been developed with these regulations in mind. The associated scope of work consists of the following tasks.
Subtask 1 – Project Management
Subtask 2 – Prepare Quality Assurance Project Plan
Pursuant to USEPA’s applicable requirements, Geosyntec will prepare a QAPP specific to this Site and the proposed activities. The QAPP will include information on project planning elements, data generation and acquisition, quality assurance related to the drilling subcontractor, and data validation. The QAPP will be submitted to FDEP for review prior to initiating the fieldwork activities under Subtask 3.
Subtask 3 – Field Activities
This task includes the planning and preparation tasks required to initiate the field activities. These tasks include, but are not limited to, coordinating with an environmental driller and environmental laboratory,, coordinating with the landowner and the Client, procuring field equipment, and preparing a Site-specific health and safety plan. Geosyntec will initiate the proposed activities after receipt of the notice to proceed in order to meet the desired schedule for the Phase II. Prior to drilling activities, Florida One Call (811) will be contacted for a utility mark-out; additionally, three subcontractor costs proposals have been obtained for the completion of a private utility locate to clear the boring locations prior to drilling activities. The subcontractor performing the utility location will use a combination of geophysical methods including electromagnetics and groundwater penetrating radar. Based on the comparison of bids attached, GPR, Inc. has been selected to conduct this utility clearance work.
Three soil borings will be advanced at the Site to approximately 2 feet below the observed water table (anticipated to be approximately 5 feet below land surface [ft bls]) at locations depicted on Figure 1. Soil borings will be advanced using a hand auger and direct-push technology rig operated by a licensed environmental drilling company. Soil retrieved from the subsurface will be logged for lithological characteristics and evaluated for volatile organic compound (VOC) concentrations using a photoionization detector (PID). Following collection of soil samples, the soil borings will be converted to temporary groundwater monitoring wells to an anticipated depth history with agrichemicals, and the proposed locations are our best approximation of being representative of targeted area of concern. However, there remains a possibility that contamination could be missed due to the proposed boring density or undisclosed Site history.
FE7551/Liftmaster Phase II SOW_13 Jan 2021 pp
13 January 2021 of approximately 15 ft bls. The temporary groundwater monitoring wells will be installed using 10 feet of polyvinyl chloride (PVC) pre-packed well screen (0.006-inch slot size and 30/65 filter pack) followed by PVC casing to the surface. Each well will be developed to remove fine-grained sediment from the well. We have assumed that the samples can be collected in approximately 30 minutes or less from each well (on a subsequent day after well installation).
Each well will be purged prior to sampling using a peristaltic pump equipped with dedicated high-density polyethylene tubing. The groundwater samples will be analyzed by the laboratory for the following parameters:
Volatile organic compounds (VOCs)- Environmental Protection Agency (EPA) Method 8260;
Polycyclic aromatic hydrocarbons (PAHs)- EPA Method 8270 SIM;
Total Recoverable Petroleum Hydrocarbons (TRPH)- FLPRO method;
Resource Conservation and Recovery Act (RCRA) 8 metals (arsenic, barium cadmium, chromium, lead, mercury, silver, selenium)- EPA Method 6010D and 7470A (mercury).
In addition, the groundwater samples collected from temporary monitoring wells TMW-1 and TMW-2 will also be analyzed for iron (EPA Method 6010), chloride (EPA Method SM300), and total ammonia as nitrogen (EPA Method SM4500), and the groundwater sample collected from TMW-2 will also be analyzed for polychlorinated biphenyls (PCBs) by EPA Method 8082 and organochlorine pesticides (OCPs) by EPA Method 8081. Groundwater sampling will be conducted in general accordance with FDEP standard operating procedures and per the QAPP.
Groundwater sampling equipment will be decontaminated (as warranted) between use at each monitoring well location. Purge water generated during the collection of groundwater samples will be discharged to an impervious surface at the Site, and temporary monitoring wells will be abandoned by Geosyntec immediately after collection of groundwater samples. Consequently, Geosyntec does not anticipate generating investigation-derived waste requiring off-Site disposal.
Geosyntec has obtained three bids from drilling subcontractors to assist with this subtask. Copies of the three bid forms have been attached along with a bid-comparison table. Cascade Drilling has been selected to conduct this work.
The Phase I ESA identified solid waste burning and runoff of unspecified firefighting liquids at the Woodchucks Recycling facility (located adjacent to the Site to the east). To evaluate the presence of dioxins in soil at the Site resulting from solid waste burning at the Woodchucks
FE7551/Liftmaster Phase II SOW_13 Jan 2021 pp
Recycling facility, soil samples could be collected from shallow soil in the vicinity of proposed temporary monitoring wells TMW-2 and TMW-3 (see Figure 1 for proposed locations) and analyzed for dixoins (at the discretion of the Client). To evaluate the potential presence of perfluorinated alkyl substances (PFAS) in groundwater at the Site resulting from runoff of firefighting liquids from the Woodchucks Recycling facility, TMW-2 and/or TMW-3 could be installed and sampled using PFAS protocol (at the discretion of the Client). Please note dioxin analysis of soil samples and PFAS analysis of groundwater samples would add notable extra cost to the project budget, and evaluation of these impacts may be conservative based on the presence of the Cross Bayou Canal between the Woodchuck Recycling facility and the Site. However, Geosyntec can provide a revised Cost Estimate Form to include this additional scope of work if requested by the Client.
Subtask 4 – Phase II Site Investigation Report
Geosyntec’s Phase II site investigation findings obtained through performance of the above tasks will be presented in a Phase II Site Investigation Report. The report will provide details of the work performed by Geosyntec and the analytical results for soil and groundwater samples collected at the Site. The report will include a figure of the Site which indicates sample locations and a comparison of detected compounds to FDEP cleanup target levels (CTLs; reference Chapter 62-780, FAC) for groundwater. A draft report will be prepared and submitted in electronic format to the Client for review and comment. Comments on the draft report will be incorporated into a final report that will be provided to the Client in electronic format.

RE: Utility Locating Services – 3 Bore Locations, Liftmasters, Clearwater, FL
Mr. Hollingshead, GeoTek Services is pleased to respond to your request for Utility Locating services at Liftmasters, Inc. in Clearwater, Florida. We will scan the 3 proposed bore locations with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings or vehicles. We will scan as close as reasonably possible to fixed objects such as curbs, landscaping, fences, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. Our quote for this project is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

GPRS appreciates the opportunity to provide this proposal. I encourage you to visit our website (www.gprsinc.com) and contact any of the numerous references listed. Our insurance certificate and W-9 can also be downloaded here. Please feel free to contact me if you have any questions, or if you need additional information.
SCOPE OF WORK
We understand the scope to be to search for underground utilities within the vicinity of (3) monitoring well locations. The (3) locations are shown on the map below. We will locate roughly a 10’ radius around each of the (3) locations. We will attempt to trace any utilities for which there are structures visible from the work area. Utilities will be marked on the surface using paint, flags, or other appropriate means. The client will be responsible to provide drawings or notify GPRS of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival in order to avoid additional charges. Please visit www.simspec.org for an overview of our industry-leading best practices that will be applied to this project.
EQUIPMENT
• Underground Scanning GPR Antenna. The antenna frequencies range from 250 MHz-450 MHz is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the types of materials being scanned through. Some soil types such as clay may limit maximum depths to 3’ or less. As depth increases, targets must be larger in order to be detected and non-metallic targets can be especially difficult to locate. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link
• Electromagnetic Pipe Locator. The EM locator can passively detect the signals from live AC power or radio signals travelling along some conductive utilities. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting signal can then be detected by the receiver. A utility’s ability to be located depends on a variety of factors including access to the utility, conductivity, grounding, interference from other utilities, and many others. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link
• Traceable Rodder. The rodder has a copper wire encased in fiberglass. The line is pushed through accessible pipes before placing a current on the wire and the signal is then traced from the surface. The maximum traceable depth is 10’ depending on the soil conditions and the maximum distance is 200’.
The line can be pushed through a pipe with direct access such as a sewer line at a cleanout or a storm drain catch basin. It may not be able to be pushed through deeper pipes within manholes. Electrical conduits will not be accessed by GPRS. The signal cannot be located through metallic pipes. For more information, please visit: Link
OPTIONAL EQUIPMENT
• GPS. This handheld GPS unit offers accuracy down to 4 inches, however, the accuracy achieved will depend on the satellite environment at the time of collection and should not be considered to be survey-grade. Features can be collected as points, lines, or areas and then exported as a KML/KMZ or overlaid on a CAD drawing. For more information, please visit: Link
• Video Pipe Camera (2”-8” pipes). This push camera is designed for video pipe inspection and pipe locating. It has a 1”-2” diameter, self-leveling camera head that provides high quality images in pipes 2”-8” in diameter. There is also a 512 Hz sonde behind the camera that can be located from above ground and provide the approximate dpeth to the pipe for pipes up to approximately 10’ deep. The locatable signal from the sonde will not pass through metal pipes or solid metal barriers except for cast-iron. Access through a structure such as a cleanout, drain, etc. or an open pipe within arm’s reach is required for inspection service and locating. When inserted, the camera can be pushed through the pipe to a maximum of 325’ depending on the model being used. If the project requires entry into confined spaces to gain access to the subject piping, this would need to be coordinated by the client and a permit obtained if required. Video and photos of the interior of pipes can be povided along with inspection reports. Inspection reports are available upon request prio to execution of field work. For more information, please visit: Link
• Video Pipe Camera (1”-4” pipes). This push camera is designed for video pipe inspection and pipe locating. It has a 1" diameter, self-leveling camera head that is used in pipes 1”-4" in diameter. There is a 512 Hz sonde behind the camera that can be located from above ground and provide the approximate depth to the pipe for pipes up to 10' deep. The locatable signal from the sonde does not pass through metal pipes or solid metal barriers except for cast-iron. Access through a structure such as a cleanout, drain, etc. or an open pipe within arm's reach is required for inspection service and locating.

When inserted, the camera can be pushed through the pipe to a maximum of 100' and past 90° bends in most 2" lines. If the project requires entry into confined spaces to gain access to the subject piping, this would need to be coordinated by the client and a permit obtained if required. Video and photos of the interior of pipes can be provided along with inspection reports. Inspection reports are available upon request prior to execution of field work. For more information, please visit: Link
• Robotic Crawler Camera. This modular, 6-wheel robotic video pipe inspection camera (crawler) is capable of 0° turns and equipped with varying wheel treads and sizes to adapt to most pipe applications. The crawler is typically used to inspect and locate pipes that are 8" in diameter and greater. The crawler is controlled by an operator at the surface using a remote control that allows for video to be viewed live during collection so observations can be made and noted as the crawler travels through the pipe. This camera is equipped with a 512 Hz sonde behind the camera head that can be located from above ground and provide the approximate depth to the pipe for pipes up to 10' deep. The locatable signal from the sonde does not pass through metal pipes or solid metal barriers except for cast-iron. Access through a structure such as a manhole, catch basin, etc. or an open pipe within arm's reach is required for inspection service and locating. When inserted, the camera can be directed through the pipe to a maximum of 1000'. For large diameter pipe applications (24" and above) the crawler can be coupled with the carriage that allows for larger and more rugged wheels to be attached and a lift that raises the camera head up to 18" allowing for the camera to be centered in the pipe for lines up to 36" in diameter. If the project requires entry into confined spaces to gain access to the subject piping, this would need to be coordinated by the client and a permit obtained if required. Video and photos of the interior of pipes can be provided along with inspection reports. Inspection reports are available upon request prior to execution of field work.
